Not in the sense of having a younger brain overall. A 2021 study of people with type 1 narcolepsy found age-related differences in specific MRI measures and brain regions, including cortical thickness and folding. It did not calculate a whole-brain biological age or show that narcolepsy makes the brain age more slowly.
What the “youthful brain” finding actually means
The phrase refers to patterns in particular brain measurements that differed between participants with narcolepsy and healthy controls. It is not a validated brain-age score, an estimate of how many years younger someone’s brain is, or evidence that every part of the brain looks or functions younger.
The 2021 study assessed cortical thickness, local gyrification (the degree of folding in the brain’s surface), and resting-state functional connectivity. It compared 51 people with type 1 narcolepsy with 60 demographically matched controls using anatomical and resting-state MRI scans. Tang et al., Cerebral Cortex (2021).
What the 2021 study found
Cortical thickness differed in adolescents
Among adolescent participants with narcolepsy, the researchers reported reduced cortical thickness in bilateral frontal cortex and the left precuneus. Cortical thickness is one structural measure; a difference in it does not, by itself, establish a person’s overall brain age.

Gyrification differences appeared in both age groups
The study reported increased gyrification in selected regions among both adolescent and adult participants with narcolepsy. In four regions, the age-related reduction in gyrification was greater among adolescents with narcolepsy and less pronounced among adults. Increased gyrification was also associated with reduced long-range functional connectivity.
These findings are regional and depend on which measure and age group are considered. They do not amount to a single, uniform “narcolepsy brain” pattern.

Why the findings do not prove slower brain aging
The study was cross-sectional: it compared people of different ages at one point in time rather than tracking how the same individuals’ brains changed. Its participants ranged from 10 to 39 years old, and the authors noted that the sample was limited. A cross-sectional age pattern cannot establish an individual’s lifetime trajectory or show that narcolepsy slows brain aging.
The authors suggested hypocretin deficiency as one possible influence on brain development, but presented it as speculation, not an established explanation. They wrote: “We can only speculate that the possible mechanisms behind this may be hypocretin deficiency in narcolepsy impact brain development and maturation via different neurotrophic factors as well as having a direct or indirect activation of neural network activity.” They called for research that directly measures age-related brain changes after hypocretin deficiency at different stages of life. The authors’ discussion and limitations also recommend larger samples across a broader age range and longitudinal designs.

How other imaging studies fit in
Other studies have reported differences in distinct measures, populations, and brain regions. Those results add context, but they cannot be combined into a single biological-age verdict.

What readers can conclude
The evidence supports the narrower conclusion that type 1 narcolepsy is associated with differences in certain regional brain-imaging measures and in how some measures vary with age. It does not establish that people with narcolepsy have younger brains overall, age better, or experience a general slowing of brain aging. The reason for the observed patterns remains uncertain.
