Dengue is a mosquito-borne viral disease. Most cases reported in the 49 continental states are linked to travel, but local transmission can occur; dengue is endemic in several U.S. territories and freely associated states. If you develop a fever or other symptoms after possible exposure, contact a healthcare professional and mention where you have traveled. Seek urgent care for severe abdominal pain, persistent vomiting, bleeding, or other warning signs—even if the fever is easing.
What dengue is and where it occurs in the United States
Dengue is caused by four related viruses and is spread primarily through bites from infected Aedes mosquitoes. A person can get dengue more than once. The Centers for Disease Control and Prevention (CDC) estimates that up to 400 million people are infected worldwide each year, although many infections cause no symptoms. CDC says about one in four infections are symptomatic.
In the 49 continental states, most reported cases are travel-associated, but mosquitoes can also spread dengue locally. CDC describes dengue as endemic in Puerto Rico, American Samoa, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau. These places are U.S. territories or freely associated states, not continental states. [CDC public health considerations]
In 2024, U.S. states and Washington, D.C., reported 359% more cases than the annual average for 2010–2023. Of the 2024 cases in that report, 97.2% were travel-associated and 2.8% locally acquired. Those figures describe 2024—not current 2026 incidence. CDC labels 2026 case data provisional; reporting delays, under-reporting, and differences in state and territorial reporting schedules limit comparisons. Warning signs can begin as the fever goes away. The critical phase typically occurs three to eight days after fever begins, so a falling temperature does not necessarily mean the risk has passed. CDC guidance says warning signs, severe dengue, or certain medical conditions may require hospital management. [CDC clinical care of dengue]
How dengue is treated
There is no specific antiviral treatment for dengue. CDC recommends medical evaluation, rest, acetaminophen for fever or pain, and plenty of fluids, such as water or drinks with added electrolytes. Avoid aspirin and nonsteroidal anti-inflammatory drugs (NSAIDs), including ibuprofen. Home care does not replace clinical evaluation when dengue is suspected. - Reduce mosquito breeding opportunities around your home.
- When traveling, choose a screened or air-conditioned room where possible. Use a bed net if those options are unavailable or if you sleep outdoors.
Before travel, review destination-specific advice and seek pre-travel care. After returning from an area with dengue risk, continue bite prevention for three weeks to reduce the chance of passing the virus to local mosquitoes. Watch for symptoms for two weeks after travel; if you develop a fever or other symptoms, tell your clinician about the trip. [CDC: Preventing Dengue]
