To create a survey in Qualtrics, start a survey project, add and arrange questions in the Survey Builder, set respondent paths in Survey Flow if needed, preview and publish the survey, then choose a distribution method. Before sharing it, check privacy and repeat-submission settings: an anonymous link does not automatically prevent collection of IP address and location data.

1. Start a survey project

Create a new survey from the Qualtrics catalog, or start from an existing survey, a file, or the library. A project is the workspace for building the survey, distributing it, and working with its analysis and reports. The choices and navigation you see can depend on your account. See Qualtrics’ survey project guide.

2. Add and organize questions

Open the project’s Survey tab and use the Survey Builder to create questions. Questions are grouped into blocks, and a new project starts with a default question block.

  1. Select Add new question.
  2. Choose a question type.
  3. Edit the question text and answer choices to match what you need to ask.
  4. Repeat for the remaining questions, then arrange questions or blocks in the order respondents should see them.

Qualtrics numbers questions in the order they were created, not necessarily the order in which they appear to respondents. Changing a question type after responses have been collected may invalidate earlier responses to that question, so settle question types before opening the survey. 3. Choose a simple sequence or conditional paths

If everyone should answer the same questions in the same order, arrange the blocks in the Survey Builder. If respondents should see different content based on their answers, use Survey Flow to create branches or add other flow elements, such as embedded data or randomization. Qualtrics describes Survey Flow as the place to customize the order in which respondents experience survey elements. The Survey Flow guide explains the available elements.

4. Configure the experience and access

Use the survey navigation to review settings such as Look and Feel, Survey Options, quotas, and translations. Configure only what your study requires, and check access and privacy settings before collecting responses. Available controls may depend on your account permissions. Qualtrics lists these areas in its survey project documentation.

5. Preview, revise, and publish

Preview the survey before launch and test the respondent route, including any branches. Make corrections before distributing it. When you are ready, publish the changes: saving an edit alone may not update the version respondents see through the live link. 6. Select how to distribute the survey

Depending on your account and project permissions, Qualtrics offers distribution methods that include email, SMS, and a single anonymous link. Choose based on how respondents will access the survey and whether you need to manage a known contact list or share a link broadly. For an anonymous link, open Distributions, retrieve the link, and share it through your chosen channel.

Activating a survey to obtain a reusable link opens it to responses. Do this when you are ready for data collection to begin. See Qualtrics’ anonymous link guide and distribution overview.

7. Check what “anonymous” means for your survey

An anonymous link does not by itself collect names or email addresses unless the survey asks for them. However, Qualtrics says IP address and location are recorded by default. If that is not appropriate for your study, review the Anonymize Responses option for removing certain personal data, along with security options for limiting repeat submissions. Check the actual settings in your project before launch; an anonymous link should not be treated as a guarantee that no identifying or location-related data is collected. 8. Review collected responses

After responses arrive, use Data & Analysis to inspect or edit individual responses, Results for a dashboard of collected results, and Reports to create reports suitable for PDF output. Access to these tabs and functions can depend on permissions. Qualtrics describes them in its survey project guide.
