Colorlib’s Free Contact Form v5 HTML/CSS Template is a front-end design, not a complete email-delivery system. It includes a service dropdown that reveals additional options and a budget selector, but visitors’ submissions will not reach you until you connect the form to a server-side handler, CMS, or form service. Colorlib lists the template as released in May 2021 under CC BY 3.0.

What Contact Form v5 includes

Colorlib describes Contact Form v5 as a clean, minimal form for collecting project inquiries. Its distinctive interaction is a service dropdown: selecting services reveals additional options. The design also includes a budget selector. These are features Colorlib describes on its Contact Form v5 listing; they do not establish that the fields or behavior will suit every site.

Colorlib’s contact-form collection associates v5 with agency websites. Treat that as a suggested use case, not a guarantee of fit. Consider whether you want visitors to choose services and indicate a budget, and whether the template’s visual style belongs on your site.

Does the form send email by itself?

No. The HTML and CSS provide the visible form, but the markup alone does not deliver submissions. To receive messages, connect the form to a server-side script, a CMS form handler, or a hosted form service. Without that connection, a visitor may be able to fill in and submit the form without the message being delivered.

How to make Contact Form v5 send submissions

Colorlib’s 2026 guide to making an HTML contact form recommends setting a form action, using POST, and validating submitted data on the server. The guide names PHP mail, Nodemailer, SendGrid, and Formspree as possible approaches. These are examples, not integrations verified for this particular template; you will need to configure the handler or service you choose.

  1. Choose where submissions will go. Decide whether your site will process requests with its own server-side code, a CMS handler, or a form service.
  2. Set up the receiving endpoint. Configure the form’s action to point to the handler or service endpoint, and use the POST method as Colorlib’s guide recommends. Follow the selected handler’s setup requirements.
  3. Validate on the server. Check submitted values again on the receiving side rather than relying only on browser-side validation.
  4. Test the complete path. Submit a test message and confirm that the receiving endpoint accepts it and that the message reaches the intended destination.

What to check when editing the fields

When you add, remove, or rename fields, keep each input’s name and id aligned with its corresponding label. The name is used when data is submitted, while the label helps identify the field for visitors; inconsistent edits can make fields harder to use or process. Colorlib’s guide recommends making these changes together.

Color, font, and spacing adjustments belong in the stylesheet. After changing the layout, review the form on a phone: Colorlib recommends stacking side-by-side content on narrow screens and making controls comfortably tappable. These are publisher recommendations; no independent testing of the template is claimed here.

What the listed license means for use

Colorlib’s official listing identifies Contact Form v5 as released in May 2021 and lists a CC BY 3.0 license. Before redistributing the download or using it in a client project, inspect the package’s included license and asset notices. The listing does not establish that any third-party assets bundled with the download share the template’s license.

Is Contact Form v5 a good fit?

It is worth considering if you want a project-inquiry form with service selection and a budget question, and you are prepared to connect a submission handler and review the mobile layout. If you only need a basic message form, or need a ready-to-receive integration, check the fields and setup requirements against your needs before adopting it. The preview and publisher’s suggested agency use case alone cannot establish how well it will fit your site.
