Colorlib’s Contact Form V19 is a free, minimalist HTML contact form built around a budget dropdown for project enquiries. It can help a business qualify incoming leads, but the downloaded front end does not send email by itself: submissions need a backend or form service.
What Contact Form V19 is designed to collect
V19 pairs a round call-to-action button with a structured budget selector. In Colorlib’s September 28, 2026 roundup, the selector appears between the email and message fields and offers example bands spanning $2,000 to over $25,000. Those figures are sample options, not recommended prices; you can customize the choices or relabel the selector for a service, department, or another intake category. Colorlib’s Contact Form V19 page identifies the template, while its 2026 roundup describes the current dropdown and rebuilt download.
When the dropdown is useful
A free-text contact form lets visitors explain what they need in their own words. V19 adds a structured question before the message, which can make it easier for an agency or service provider to sort project leads. If you only need a basic “send us a message” box, a budget question may add unnecessary friction; use it only if the answer will inform how you respond.

What the current download includes
Colorlib describes its rebuilt forms as dependency-free HTML and CSS with a small amount of vanilla JavaScript. It says the templates can be used standalone or in Bootstrap 4, 5, or 6 projects without loading jQuery, Bootstrap files, or plugins. Colorlib reports that the V19 download is 190 KB, compared with 1.3 MB before the rebuild. These are publisher-reported specifications, not independent performance measurements; file size alone does not establish how quickly a finished page will load.

How to make form submissions reach you
The template supplies the front end, not the service that receives and delivers entries. To handle submissions, connect the form’s action URL to a server-side handler or form service. Colorlib’s FAQ names a PHP mail script, a serverless function, and services such as Formspree, Basin, and Web3Forms as possible approaches. Choose an endpoint that you control or have configured, then verify that it accepts the submitted fields and delivers them to the right destination. Do not expect the downloaded HTML form to email you automatically.
License and required attribution
The V19 page labels the template CC BY 3.0. Colorlib’s current roundup says personal and commercial use is allowed under that license as long as the credit link is kept. Follow the attribution displayed with the download and review the license terms for your specific use; Colorlib’s stated guidance does not remove the need to preserve the credit link.

Make sure you have the right V19
Colorlib has a separate page titled “Contact Form v19” at this lowercase-v19 URL. That is a different design, described as a basic message form with a Google Maps background and autocomplete—not the budget-picker form covered here. The names are similar, so check the page URL and preview before downloading.
The target page displays a March 6, 2021 post date alongside a “Released: 2025” detail. The separate lowercase-v19 page shows an April 21, 2018 post date and “Released: April 2026.” Those conflicting page details do not provide a reliable release chronology; the September 28, 2026 roundup is the clearest source for the current rebuilt download description.
