Colorectal cancer (CRC) is rare in children and teenagers, but it can be diagnosed late and may have features that are more common in younger patients than in older adults. The evidence needs a careful distinction: early-onset CRC is rising in many countries among adults under 50, while the material available here does not establish a population-wide rise in CRC specifically among children. Persistent symptoms such as rectal bleeding, unexplained iron-deficiency anemia, ongoing abdominal pain, or a lasting change in bowel habits deserve prompt medical assessment—but they do not, by themselves, mean a child has cancer.
Is colorectal cancer rising in children and teens?
CRC in children and adolescents remains uncommon. The National Cancer Institute (NCI) says that childhood and adolescent cancer overall is rare and that its overall incidence has increased slowly since 1975. That broad trend is not a child-specific CRC rate, however, and it should not be presented as proof that CRC is rising among children at the same pace as it is among adults under 50.
The clearest international increase described in recent evidence concerns early-onset CRC, generally meaning diagnoses before age 50—not childhood cancer. An American Cancer Society (ACS) analysis using data through 2017 found incidence rising in 27 of 50 countries and territories. In 14 countries, rates among young adults rose while rates in adults ages 50–74 stabilized. The highest recent early-onset rates in that analysis were 14–17 cases per 100,000 in Australia, Puerto Rico, New Zealand, the United States, and South Korea, as reported by the ACS in 2024. Those figures cover adults ages 25–49 and are not estimates for children.

What pediatric and adolescent studies do show is why recognition matters. In a 2025 four-institution retrospective cohort of 34 patients ages 10–22, the median age at diagnosis was 19. At diagnosis, 74% had disease at least T3, 29% had metastatic disease, and 71% had one or more positive lymph nodes. This small clinical cohort describes the patients treated at those institutions; it cannot tell us how often CRC occurs across the entire population.

How can CRC in younger patients differ from typical older-adult CRC?
Not every child or young adult has a distinct tumor type. But pediatric and adolescent cases have higher frequencies of several histological and molecular features than are typical in older adults, so it is misleading to assume the disease always follows the same pattern.
Histology and tumor location
The NCI’s current pediatric cancer summary reports mucinous adenocarcinoma in 40%–50% of pediatric and adolescent colorectal lesions, compared with about 15% of adult lesions. Younger patients also have more tumors with signet-ring-cell components. These are tumor descriptions made by pathologists; they do not determine an individual patient’s outlook on their own.
A SEER analysis of 5,350 patients ages 15–39 diagnosed from 2010 through 2015 found right-sided tumors in 28.6% overall. The share was 38.3% among those ages 15–19 and 27.3% among those ages 35–39. These age-group figures come from that registry analysis and should not be generalized to every pediatric patient.

Molecular features and inherited risk
Younger patients have higher frequencies of microsatellite instability (MSI) and mismatch-repair (MMR) gene variants than older patients, according to NCI summaries. Some small genomic comparisons have also reported differences in genes including MYCBP2, BRCA2, PHLPP1, TOPORS, and ATR. Those findings are not all validated, and they should not be treated as a standard diagnostic signature for every young patient.
Inherited syndromes matter, but they do not explain every case. Pediatric CRC cohorts include people with Lynch syndrome, familial adenomatous polyposis (FAP), and Li-Fraumeni syndrome. In one pediatric series summarized by NCI, nearly 30% had a known predisposition syndrome; that proportion is from a particular series, not a universal estimate for all children with CRC. When appropriate, clinicians may recommend specialist genetic counseling and tumor testing. Consumer genetic tests are not a substitute for evaluation by a medical team.
Which symptoms should prompt medical assessment?
Symptoms that persist, recur, worsen, or occur together should be discussed promptly with a clinician. ACS researchers have highlighted rectal bleeding, abdominal pain, altered bowel habits, and unexplained weight loss in young people. NCI’s pediatric summary also lists blood in stool, iron-deficiency anemia, weight loss, decreased appetite, and an abdominal mass—particularly in descriptions of right-sided tumors.

- Rectal bleeding or blood in the stool that persists or returns.
- Unexplained iron-deficiency anemia, or fatigue that is being assessed alongside possible anemia.
- Ongoing abdominal pain or a palpable abdominal mass.
- A sustained change in bowel habits.
- Unexplained weight loss or decreased appetite.
These symptoms have many possible causes, and common benign conditions are far more frequent than CRC in children. A symptom is not a diagnosis. A clinician can decide whether examination, blood tests, imaging, or referral is warranted based on the child’s history and findings.

Symptoms may be attributed to more common problems, and CRC is rare in young people, which can complicate recognition. Registry and clinical studies also show substantial advanced disease at diagnosis, though their age ranges and patient groups differ.
An NCI-cited National Cancer Database analysis included 531,462 patients with colon cancer diagnosed from 2004 through 2016, including 947 patients age 25 or younger. In that analysis, stage III disease was recorded in 44.4% of the very-young group versus 33.4% of older patients; stage IV disease was recorded in 27.5% versus 15.3%, respectively. These are registry comparisons from that period, not a prediction for an individual child.

The separate 2025 multicenter cohort of 34 patients ages 10–22 reported that, at a median follow-up of 2.2 years, 50% were alive with no evidence of disease, 15% were alive with disease, 26% had died, and 9% had unknown status. The small sample and limited follow-up mean these outcomes should not be used as an individual survival estimate.

There is no definitive explanation. An NCI expert review published in 2025 describes ongoing investigation of obesity, alcohol, diet and environmental exposures, microbiome disruption, bacterial toxins, and birth-cohort effects. Researchers have cautioned that strong epidemiological evidence does not establish an individual link between many of these factors and early-onset cancers, and findings about specific genetic contributors have been conflicting.

For families, the practical point is that no single food, chemical, infection, or parenting practice has been established here as the cause. Avoid blaming a child or family, and do not treat a proposed risk factor as an explanation for a particular diagnosis.

What happens if a clinician suspects CRC?
Assessment is guided by the child’s symptoms and clinical findings. Depending on the case, the work-up may involve imaging, colonoscopy, biopsy, staging, tumor molecular testing, and evaluation of possible inherited risk. A biopsy establishes what the tissue is; staging assesses how far disease has spread. The treating team determines which tests are appropriate and in what order.
NCI recommends that children and adolescents with cancer be referred to medical centers with multidisciplinary teams experienced in pediatric disease. Because adolescent and young-adult patients may need both pediatric and adult expertise, coordination across those specialties can help guide care. Treatment is selected for the individual patient by the treating team; the evidence summarized here does not support one treatment plan for every young person with CRC.
How to interpret the evidence without overreading it
Studies use different definitions of “young”: under 21, age 25 or younger, ages 10–22, or ages 15–39. “Early-onset CRC” often refers to diagnosis before age 50 and includes adults. Those groups are not interchangeable, and a study of patients treated at several hospitals cannot establish the population incidence of childhood CRC.

Persistent red-flag symptoms merit timely clinical review, while causes and individual risk require careful assessment rather than assumptions.
