Hardened cockpit doors were introduced to make it harder for someone in the cabin to seize an aircraft. They are not a safeguard against every threat: when danger originates inside the flight deck, the same separation can make outside intervention harder. Available sources do not establish how often either scenario occurs or quantify the net effect on risk.
Why cockpit security changed after 9/11
The old response model assumed hijackers could be negotiated with
The 9/11 Commission described the FAA-approved “Common Strategy” before the attacks as cooperation and nonconfrontation: crews were expected to accommodate hijackers, land safely, and leave resolution to law enforcement or the military. That approach assumed hijackers had negotiable demands; it was not designed for suicide hijackings. The Commission also said FAA training gave crews no guidance on what to do if violence occurred.
The doors did not provide the same barrier they do now
Rules dating to the 1960s required cockpit doors to be closed and locked in flight, according to the Commission. But the doors also had to allow ready access in an emergency, and the rule was not always observed or vigorously enforced. The Commission connected that access model and the cooperative response strategy to the failure to anticipate the 9/11 attack method.

U.S. rules changed after the attacks
The Department of Transportation’s post-9/11 final-rule summary allowed airlines to modify flightcrew-compartment doors to delay or deter unauthorized entry. It also prohibited cabin flight attendants from possessing flightdeck door keys during flight. These are U.S. rule changes, not a complete account of aviation rules worldwide.
The Commission reported that, as of 9/11, 33 armed and trained federal air marshals were not deployed on U.S. domestic flights except when in transit to provide security on international departures. That is a historical figure, not a present-day count.

Two different threat pathways
A door intended to stop an unauthorized person in the cabin addresses a different problem from a threat originating with someone already inside the flight deck. The measures should not be treated as proof that the two risks are equally likely: the sources cited here provide no comparative probabilities.

What an attack from inside the cockpit changes
An Associated Press report described an in-flight attack on a FlyDubai pilot. The incident details should be regarded as provisional pending investigation; the report is not, by itself, proof of motive, cause, or a confirmed sequence of events.

AP quoted Matthew Borie, chief intelligence officer at Osprey Flight Solutions, warning that an aviation insider with harmful intent may know “where the most vulnerable points are and how to breach them.” That is an expert’s assessment of the insider threat, not an official finding about the FlyDubai incident.
The security problem is therefore not simply whether a cockpit door is strong. When an emergency involves a threat inside the flight deck, the people who might otherwise intervene are outside a barrier built to restrict access. AP quoted John Gradek, who manages McGill University’s Aviation Management Program, on the priority in a serious emergency: getting the aircraft to a suitable runway, “regardless of nationality, flight plan or politics.” The fragment describes his view of emergency priorities; it should not be read as a universal legal rule or a finding about this incident.

What newer secondary barriers do
A secondary barrier is a separate physical layer between the cabin and flight deck. The FAA’s final-rule record concerns an installed barrier on certain passenger-carrying airplanes, intended to protect the flight deck from unauthorized intrusion while the main cockpit door is open. An AP retrospective reported that newly manufactured passenger airplanes must have a retractable security gate between the cabin and flight deck as of that report’s publication.
The scope is aircraft- and jurisdiction-specific: the FAA material concerns U.S. rules, and it does not establish that every aircraft flying anywhere has the same equipment. Nor does an extra barrier address every possible threat from within the flight deck. The sources describe its purpose but do not provide a comparable figure for its effectiveness.

Does a locked cockpit mean pilots can be locked out?
A cockpit door that restricts access can make entry from the cabin harder, including for people who may need to respond to an emergency. But the material here does not establish the details of current airline procedures for access, emergencies, or a crew member who is locked out. It would be misleading to infer a universal procedure from the door’s design or from one incident report.
What can be concluded about safety?
The strongest supported conclusion is limited but useful: post-9/11 door changes were aimed at preventing an unauthorized person in the cabin from entering the flight deck, while a threat originating inside that space presents a different problem. A secondary barrier adds another layer on certain passenger aircraft when the main door is open. The sources do not show that cockpit protection is foolproof, that it has broadly failed, or how much either measure changes overall risk.
