A caption or thumbnail can help you find a clinical image, but neither proves that you can access its original pixels, crop it responsibly, or reuse it. Those are three separate questions: what a search system indexes, what image data you can open, and what the rights and applicable policies allow. For a clinical or scientific crop, begin with an authorized source file, preserve the original, and keep enough context to interpret the image.
What does image search use: captions, metadata, pixels, or all three?
It depends on the search system. General web image search can use textual clues such as a page’s title, caption, filename, and surrounding text; other systems may also index visual content or attributes such as image modality. A result’s appearance alone does not tell you which signals produced it.
Google documents image-search operators that can help narrow results, but they have limits: src: finds pages that reference a supplied image URL, while imagesize: filters by image dimensions. Neither guarantees a complete collection or access to the original file. See Google’s image-search operators documentation. Google also describes textual image context in its image SEO guidance.
Biomedical literature search offers a different set of examples: some systems make captions searchable and show thumbnails, extracted figures, subfigures, or modality information. A caption is useful context, but it is not the image itself. A study of concept-based indexing evaluated 84,846 radiology figure captions from 9,004 English-language peer-reviewed articles. In that specific 2009 study, estimated precision was 0.897 (95% confidence interval 0.857–0.937) and estimated recall was 0.930 (95% confidence interval 0.838–1.000) for the sampled concepts and caption corpus—not for image search generally. In a random sample of 10,000 queries, concept-based retrieval returned results not found by simple keyword matching for 5,535 queries (55%). The study record describes the scope and methods.

Does a thumbnail let you crop the full-resolution image?
No. A thumbnail is a preview, not proof that the complete image file is available or that you have permission to use it. Some biomedical search workflows have linked thumbnails to higher-resolution figures, but this depends on the particular system, source, and access rules. A 2008 paper on Yale Image Finder reported a collection of more than 140,000 images from more than 34,000 open-access biomedical papers; those are historical publication figures, not a current service count. The paper record describes that system.
Before cropping, open the source page or repository and determine whether it provides the original image file at sufficient resolution. The visible preview may be compressed, resized, or only one panel of a larger figure. If the source requires institutional credentials or other authorization, a public search result does not bypass those controls.

Where do clinical images come from?
Public web search, biomedical article search, open-access datasets, and hospital imaging repositories are different routes to images, with different access conditions. A hospital PACS is not the same thing as a public image-search index: clinical images may be held in systems with institution-controlled access and privacy safeguards. The DIANA paper describes an interface to hospital PACS for research and clinical AI applications; it does not establish that patient images are exposed through ordinary public search. Read the DIANA system record.
Open-access case-report datasets also illustrate why text and pixels should be treated separately. A 2023 dataset paper reports 135,596 images and 96,428 clinical cases, with images paired with labels and captions. Its preprocessing included removing borders and splitting some compound figures into panels. Those reported dataset contents do not grant permission to reuse images beyond the dataset’s own terms. The dataset paper record describes its scope.

When is cropping scientifically responsible?
The HHS Office of Research Integrity says cropping is usually acceptable when it removes irrelevant edges or focuses attention without changing context. It cautions against crops that conceal information conflicting with a favored interpretation, recommends retaining the raw original, and warns that enlarging an image with few pixels can create artifacts. Read Guideline #4.
For a clinical or scientific image, check the rules that actually govern its use: journal image policies, institutional requirements, and the clinical system’s procedures. The cited guidance does not establish one universal cropping rule or resolution threshold for every clinical use. A crop intended to illustrate a finding should retain the landmarks, controls, scale, or surrounding anatomy needed to understand what is shown. Do not use enlargement to imply detail that is absent from the source pixels.

- Start with an authorized source. Obtain the image from its original host or approved repository rather than treating a search preview as the working file.
- Keep an untouched original. Save the source image separately so edits can be checked against it.
- Make the smallest justified crop. Remove irrelevant margins or direct attention without hiding pertinent context or contradictory evidence.
- Check the result at its intended size. Look for lost context and artifacts, especially if the crop is enlarged.
- Follow the applicable policy. Confirm the relevant journal, institution, and repository requirements before sharing or publishing.
Does image search give permission to crop or republish?
No. Search placement, a caption, metadata, or a license filter does not itself grant reuse rights. Google says its usage-rights information comes from image hosts or providers and recommends checking licensing requirements. Verify the exact terms with both the original image host and the license provider; check attribution requirements, whether modification is allowed, commercial-use limits, and whether your intended use is covered. Google’s guidance is available in Find images you can use and share and Find Google Image details.

How to assess an image-search result before using it
| Question | What to verify |
|---|---|
| What made the result searchable? | Whether the system indexes captions, page text, filenames, metadata, visual content, modality, or some combination. This varies by system. |
| What does the result display? | Whether it is a thumbnail, extracted figure, subfigure, or link to a source image. A preview alone does not establish full-resolution access. |
| Can you access the needed pixels? | Open the authorized source and check the file’s resolution, context, and any access controls. |
| Can you crop and reuse it? | Confirm the host’s and rights holder’s terms, then apply relevant institutional and publication policies. |
