Neither chemical nor physical exfoliation is proven to work better for everyone with keratosis pilaris (KP). Dermatology guidance supports gentle surface exfoliation and ingredient-based keratolytics, with moisturizing to help manage dryness. Choose the approach your skin tolerates, and avoid harsh scrubbing: irritation can make KP worse. Treatment is optional and may improve the bumps or rough feel, but it does not cure KP.
What chemical and physical exfoliation mean for KP
KP causes small, rough bumps, commonly on the upper arms and thighs. They are associated with plugs of dead skin in hair follicles. KP is not contagious, and it does not need treatment unless it bothers you. The American Academy of Dermatology Association (AAD) overview and DermNet’s March 2022 overview describe the condition and its typical appearance.
“Chemical” exfoliation in this context means applying ingredients that help loosen or remove dead skin and follicular buildup. Physical exfoliation means gently rubbing the skin’s surface with a soft cloth or similar aid. A keratolytic moisturizer can do both jobs of delivering an active ingredient and moisturizing; the categories are not mutually exclusive.
How the options compare
| Approach | How it works | Examples in dermatology guidance | Main caution |
|---|---|---|---|
| Chemical / keratolytic | Helps loosen or remove dead surface skin and follicular buildup. | Lactic acid, glycolic acid, salicylic acid, other alpha hydroxy acids (AHAs), urea, and retinoids. | Follow product or clinician directions. Overuse can leave skin raw, dry, or irritated. |
| Physical | Gently removes some dead skin from the surface. | Soft washcloth, loofah, exfoliating sponge, or mitt. | Hard rubbing and harsh abrasives can irritate skin and worsen KP. |
| Moisturizing support | Helps address dryness and support skin comfort. | Cream or ointment; some moisturizers also contain urea or lactic acid. | Apply after bathing while skin is still damp, and reapply when skin feels dry. |
The AAD lists these ingredients and self-care approaches in its diagnosis and treatment guidance and self-care guidance. The available guidance does not establish a direct head-to-head winner. In practice, tolerability and ease of use are more useful decision points than the label “chemical” or “physical.”

Which should you choose?
Consider a keratolytic moisturizer if you want an ingredient-based routine
A product containing lactic acid, urea, or another recommended keratolytic may suit someone who prefers applying a lotion or cream rather than rubbing the skin. These ingredients are not interchangeable for every person, and a product’s directions matter. Start and use it as directed; do not assume that using more often or combining several exfoliating products will improve results.
Choose gentle physical exfoliation only if your skin tolerates it
A soft washcloth or exfoliating mitt can provide gentle surface exfoliation. Use light pressure rather than trying to scrub the bumps away. The AAD, DermNet, and the NHS guidance caution against vigorous rubbing or harsh scrubs because irritation may worsen KP.

Make moisturizing part of either approach
Exfoliation can leave skin dry, so moisturizing is not an optional finishing step. The AAD recommends applying moisturizer after bathing while skin is damp; its treatment guidance specifies within five minutes and at least two or three times a day. Its self-care guidance also recommends moisturizing after bathing and whenever skin feels dry. A cream or ointment is an option, including a moisturizer that contains a keratolytic.
A gentle routine to try
The following sequence reflects general AAD self-care advice, not an individual prescription. If you have persistent irritation, are unsure the bumps are KP, or have symptoms that concern you, ask a clinician for guidance.

- After bathing, exfoliate gently if you choose. Use a soft cloth or other gentle aid; do not scrub hard.
- Apply a keratolytic product according to its label or clinician’s directions. The AAD names ingredients such as lactic acid, urea, and salicylic acid among options used for KP.
- Moisturize while skin is damp. Use a cream or ointment, and reapply when skin feels dry.
- Adjust if your skin reacts. If the keratolytic makes skin too dry or irritated, the AAD advises pausing it for a few days before resuming according to directions.
The NHS also advises lukewarm bathing, mild unperfumed products, and avoiding scratching, picking, rubbing, and harsh scrubs.

KP care can improve appearance or symptoms, but it does not cure the condition; ongoing maintenance may be needed. Improvement takes time, and finding a tolerable routine can involve adjustment. The AAD recommends contacting a dermatologist if there has been no improvement after following a treatment plan for four to six weeks. That is a follow-up benchmark, not a promise that everyone will see a change within that period.
