Yes. Atherosclerotic lesions can begin early in life, but the claim that nearly one in 10 young adults generally have “fatty, narrow arteries” is not established by the studies cited here. The often-quoted 10% figure refers to coronary artery calcium in a cohort whose mean age was 40.3—not a representative sample of young adults. Different studies measure different things, including autopsy lesions, carotid plaque, coronary calcium and heart attacks, so their percentages cannot be treated as interchangeable.

What does fatty plaque in an artery mean?

Atherosclerosis is the buildup of plaque in artery walls. That process can involve different types of lesions and affect different arteries. A fatty streak is not the same finding as a fibrous or complicated plaque, and a plaque does not automatically mean an artery is significantly narrowed, blood flow is obstructed, or symptoms are present.

The distinction matters because studies may examine the aorta, the coronary arteries that supply the heart, or the carotid arteries in the neck. For example, the American Heart Association describes carotid plaque as fatty buildup that can narrow neck arteries and may cause stroke; that is not the same measurement as coronary calcium or plaque found in an autopsy. Does one in ten young adults really have artery disease?

That broad claim is not supported by the findings described below. The 10% figure most relevant to the claim refers to coronary artery calcium reported for a CARDIA cohort at a mean age of 40.3 years. It is not a general prevalence estimate for young adults, and coronary calcium is an imaging finding—not a count of all fatty or narrowing artery lesions. A 2021 paper reviews that cohort result. 2021 paper discussing CARDIA

3M Littmann Cardiology IV Diagnostic Stethoscope, 6163, More Than 2X as Loud, Weighs Less, Stainless Steel Black-Finish Chestpiece, 27" Black Tube, Stem, and Headset
  • Diagnose and make decisions with confidence and hear subtle changes in patient status
  • Useful in critical care and challenging environments such as the ED, ICU, Cardiac ICU, step-down unit and other dynamic locations
  • More than twice as loud* as the next leading stethoscope. *Based on tests against globally-available comparable stethoscopes in an equivalent class using recorded heart sounds with diaphragm.
  • Allows you to more reliably and consistently hear heart sounds at lower frequencies (below 120Hz*) like Korotkoff sounds, Mitral Stenosis and S3 and S4 Gallops when compared to other leading stethoscopes.
  • Weighs less** than other stethoscopes. ** Based on published weights of globally-available comparable stethoscopes in an equivalent class.

A separate “nearly one in ten” statistic concerns familial hypercholesterolemia among young adults who had already suffered a heart attack, as described in a 2019 Brigham and Women’s Hospital report. That is a selected group of heart-attack patients, not a measure of how many young adults generally have plaque. Brigham and Women’s Hospital report

What different studies actually found

Evidence Who or what was studied What it can—and cannot—show
Japanese autopsy study (2001; autopsies conducted 1991–1995) Autopsy material from people aged one month to 39 years; 1,066 aortas and 974 coronary arteries were examined. Researchers classified fatty streaks, fibrous plaques and complicated lesions in the aorta and coronary arteries. Because this was an autopsy study, it does not estimate prevalence among living young adults. Study abstract
CARDIA result reported in a 2021 paper A cohort with a mean age of 40.3 years. Ten percent had coronary artery calcium. This is not a general young-adult estimate or a measure of every form of plaque. 2021 paper
Bogalusa Heart Study analysis (2021) 508 participants without clinical ASCVD or carotid plaque at baseline, followed for 12 years. Nearly one third of young adults with relatively low traditional risk-factor burden developed premature atherosclerosis. This is a result in a specific cohort, not a general-population rate. Study
2019 familial-hypercholesterolemia report Young adults who had suffered a heart attack. Nearly one in 10 met clinical criteria for familial hypercholesterolemia. The statistic applies to this selected patient group, not young adults generally. Report

These findings answer different questions. Autopsy work identifies lesions in examined arteries after death; coronary CT detects calcium; carotid ultrasound can identify plaque; and a clinical heart attack is an event. One result cannot be converted into another type of prevalence estimate.

Can atherosclerosis start before middle age?

Yes. The autopsy study documented lesions in people under 40, and the Bogalusa analysis found premature atherosclerosis developing during follow-up in some young adults who began without clinical ASCVD or carotid plaque. These findings show why “young” does not mean “impossible,” but neither establishes that a fixed proportion of all young adults have obstructive disease.

What should young adults do about prevention?

The 2026 ACC/AHA dyslipidemia guideline recommends dietary, physical-activity and weight optimization for adults older than 18 through age 39 to reduce cumulative exposure to atherogenic lipids and lifetime ASCVD risk. This is preventive guidance; it does not promise that lifestyle changes will prevent or reverse every lesion. 2026 ACC/AHA dyslipidemia guideline

The 2019 AHA/ACC primary-prevention guideline describes coronary artery calcium as a possible risk-refining tool for selected younger adults, while noting that more data are needed for those subgroups. That is not a recommendation for universal scans or routine imaging of all young adults. 2019 AHA/ACC primary-prevention guideline

A headline alone is not a reason to start medication or supplements, buy a home test, or seek an imaging scan. Prevention and testing decisions should be based on an individual’s circumstances and discussed with a qualified clinician.
