No general test can detect brain damage before it happens. Current tests are used after a suspected head injury to assess what may have happened and help guide care. Because symptoms can take hours or days to appear, seek emergency help for danger signs rather than waiting for a test.

What “detecting brain damage before it happens” really means

There is an important difference between predicting an injury before an event and detecting or assessing injury after it. The clinical guidance covered here concerns traumatic brain injury (TBI), including concussion, after a blow, jolt, or other suspected injury. It does not establish a way to forecast that brain damage is about to occur.

A concussion is a brain injury, even when symptoms are described as mild. Symptoms may begin right away or appear hours or days later, and they can change over time. A person who seems well immediately after a head impact may still need assessment if symptoms develop.

After a suspected injury, a health care provider considers what happened, the person’s symptoms, and a neurological examination. Imaging or other tests may be appropriate depending on the clinical situation. No scan, blood test, app, or wearable should be treated as a stand-alone way to rule out every TBI or predict future injury.

How clinicians assess a suspected head injury

Evaluation starts with the circumstances of the injury and the person’s symptoms and functioning. A provider may examine neurological function and order imaging or other tests when indicated. Depending on the injury, evaluation can also include cognitive, speech, swallowing, or other functional assessments.

What each tool can and cannot do

Tool What it can help assess Important limit
Clinical history and neurological examination The event, symptoms, and changes in neurological function. It is part of a provider’s overall assessment, not a forecast of injury before an event.
CT scan Quickly checks for bleeding, bruising, or other visible damage when clinically indicated, including concern about bleeding. It does not detect every TBI. A scan that does not show a problem does not rule out concussion.
MRI Provides more detailed images and may be used in follow-up. It does not detect every TBI and is not a general pre-injury prediction test.
GFAP/UCH-L1 blood test Can aid a clinician’s decision about whether a CT scan is needed after suspected mild TBI. It is not a stand-alone diagnosis, a home screening test, or a way to predict injury before impact.

CDC guidance says a scan is not necessary to identify every mild TBI or concussion. A provider may consider CT when someone is at risk of bleeding after an injury; the decision depends on the clinical assessment. FDA also cautions that CT and MRI cannot detect all TBIs and that no FDA-cleared or approved medical product is intended to diagnose or treat TBI by itself, without provider judgment and other diagnostic information.

What the blood biomarker test is used for

The GFAP/UCH-L1 test described by NICHD detects proteins released into the bloodstream after mild concussion. Its clinical role is to help a provider identify patients whose injury is unlikely to appear on a CT scan and support a decision about whether CT is needed. It does not establish a diagnosis by itself, and it cannot detect damage before an injury occurs.

Other blood biomarkers have also been studied, but research findings should not be mistaken for consumer screening claims. In a 2020 NIH report on neurofilament light chain research, Leighton Chan, M.D., M.P.H., then chief of the NIH Clinical Center’s Rehabilitation Medicine Department, said: “Currently, there are no validated blood-based biomarkers to provide an objective diagnosis of mild traumatic brain injury or to predict recovery.” That statement describes the research context reported in 2020; it is not a claim about every later development.

A 2022 NIH report on the TRACK-TBI study described GFAP and UCH-L1 measurements in nearly 1,700 TBI patients from 18 U.S. trauma centers. Injury-day levels were associated with six-month outcomes. Those results concern risk assessment after an injury, not prediction of damage before an event, and do not establish a consumer test’s accuracy.

When to get emergency care

Do not wait for symptoms to worsen or for a test if emergency danger signs appear after a head injury. CDC advises seeking emergency care immediately for signs such as:

  • A worsening headache that will not go away
  • Repeated vomiting
  • A seizure
  • Weakness or numbness
  • Slurred speech or unusual behavior
  • Unequal pupils or worsening confusion
  • Loss of consciousness or difficulty waking

For a child, inconsolable crying or refusing to nurse or eat after a head impact are also danger signs. If symptoms appear later or change over time, contact a health care provider for assessment; emergency signs call for immediate care.

Can an app, wearable, or consumer test tell you?

The FDA’s guidance does not support using an app, wearable, or other device as a substitute for clinical judgment and appropriate diagnostic information. A device’s claim that it can identify brain damage in advance should not be treated as an established medical diagnosis or a reason to delay care. Prevention measures may reduce injury risk, but they are not detectors and cannot guarantee that an injury will not happen.
