A wearable-data model developed by researchers at the Icahn School of Medicine at Mount Sinai forecast activity one hour ahead and used those forecasts to identify 15-minute sedentary bouts in women with chronic pelvic pain disorders. It is a prediction framework, not a proven treatment: the study did not test whether movement prompts reduce sitting or improve pain or quality of life.

What the wearable AI study found

The 2026 study, “Robust forecasting of sedentary bouts in chronic pelvic pain disorders for on-device learning and real-time deployment,” analyzed wearable data from 134 participants with chronic pelvic pain disorders. Mount Sinai’s institutional summary also describes 61 healthy comparison participants. Participants wore Fitbit devices for up to 90 days; the paper’s methods identify the Fitbit Inspire 2.

The researchers trained personalized models using approximately ten days of an individual’s data, then forecast physical activity one hour ahead. They assessed whether the forecasts could identify sedentary bouts lasting 15 minutes during waking hours. The aim was to explore whether everyday wearable data might support timely detection of prolonged sitting, not to diagnose pelvic pain or recommend a treatment.

How the models worked—and who was studied

The study compared online and offline learning approaches. Models that drew on recent activity and recurring daily patterns performed best. The paper reports that relatively simple approaches performed well; Mount Sinai says these approaches were comparable to more computationally intensive deep-learning methods. This suggests that greater model complexity was not necessary for the forecasting task tested, not that simpler algorithms are always better for health applications.

The parent study’s eligibility criteria limit how broadly the results should be applied. Participants were assigned female at birth, menstruating, ages 18–64, and had chronic pelvic pain for at least six months plus a self-reported surgical or clinician diagnosis of a chronic pelvic pain disorder. The paper also lists pregnancy-related exclusions and active major comorbidities among exclusions. The findings should not be treated as established for all women or all people with pelvic pain.

What the reported alert numbers mean

At a conservative operating point, the paper reports approximately one true alert per day compared with 0.6 false alerts. These figures describe the model’s sedentary-bout alert performance, not patient outcomes. They do not show that an alert was delivered to participants in a clinical intervention, nor that responding to one reduced sitting or changed symptoms.

In practical terms, the framework attempted to flag periods when a person’s predicted activity pattern suggested a prolonged sedentary bout. The reported trade-off matters: any real-world system would need to balance useful alerts against interruptions that are unnecessary or poorly timed. The study’s alert metric alone cannot establish what balance users would prefer or what would be clinically beneficial.

What this does not prove—and what comes next

The study does not establish that sitting causes chronic pelvic pain, that taking movement breaks treats an underlying pelvic pain disorder, or that buying a consumer wearable improves health outcomes. It evaluated forecasting from wearable data, not the effect of behavior-change prompts.

Mount Sinai identifies prospective clinical trials as necessary to determine whether personalized movement prompts based on predictions can reduce sedentary time, improve symptoms, or enhance quality of life. Until those outcomes are tested, the work is best understood as a technical foundation for a possible just-in-time adaptive intervention—not evidence that such an intervention works.
