No vitamin or dietary supplement has been shown to prevent or treat cancer. Evidence differs by nutrient, dose, formulation, and population, and some trials have found harm in specific groups. Supplements are not a substitute for cancer care; if you have cancer or are receiving treatment, share every supplement’s name, dose, and timing with your oncology team. A supplement prescribed for a documented deficiency or another medical reason is a separate matter.
Can vitamins prevent cancer?
The National Cancer Institute (NCI) says there is not enough evidence that multivitamins, minerals, or individual vitamins and minerals prevent cancer. Observational studies can find associations between nutrient intake and cancer, but those associations do not show that taking a supplement prevents a diagnosis. NCI’s Cancer Prevention Overview states: “There is not enough proof that taking multivitamin and mineral supplements or single vitamins or minerals can prevent cancer.”
Randomized trials also show why a nutrient’s reputation or a result in one group should not be generalized to everyone. In the ATBC trial, 29,133 Finnish male chronic smokers ages 50–69 received beta-carotene at 20 mg/day, alpha-tocopherol at 50 mg/day, both, or placebo for 5–8 years. The beta-carotene groups had higher lung cancer incidence (relative risk 1.18; 95% confidence interval 1.03–1.36) and total mortality (relative risk 1.08; 95% confidence interval 1.01–1.16). These findings concern supplements in the studied population; they do not show that beta-carotene-containing foods cause cancer. See NCI’s Lung Cancer Prevention PDQ.

The combination increased lung cancer risk and all-cause mortality, and the trial ended early. This result applies to that tested combination and high-risk population, not to every vitamin product or person. NCI summarizes the evidence in its Antioxidants and Cancer Prevention fact sheet and Lung Cancer Prevention PDQ.

Vitamin E is another example of a finding that is specific to the tested intervention. In extended follow-up of SELECT, 400 IU/day vitamin E alone was associated with an excess prostate cancer risk versus placebo (hazard ratio 1.17; 99% confidence interval 1.0004–1.36; P=.008). The NCI discusses this result in its Cancer Prevention Overview. These trial doses are not recommendations for use.
Does vitamin D prevent cancer or lower the risk of dying from it?
Most randomized trials summarized by NCI did not find that vitamin D, with or without calcium, lowered cancer incidence. Cancer mortality results are not uniform. In VITAL, which included more than 25,000 participants, vitamin D at 2,000 IU/day did not reduce cancer deaths overall. Analyses that excluded deaths in the first few years reported a reduction, but that secondary analysis should not be confused with the overall trial result. The NCI’s Vitamin D and Cancer fact sheet describes the evidence.

So the evidence does not establish vitamin D as a way to prevent cancer or reduce cancer deaths overall. A clinician may still recommend vitamin D for a deficiency or another medical indication; that is a different decision from taking it as cancer prevention or treatment.

Can vitamins treat cancer or prevent it from returning?
No vitamin, food, diet trend, mineral, supplement, herb, or combination has been proved to slow or cure cancer or prevent its return. NCI’s Diets, Supplements, and Cancer states: “But there is no proof that any nutrition trend, food, vitamin, mineral, dietary supplement, herb, or combination of these can slow cancer, cure it, or keep it from coming back.”

This is different from correcting a diagnosed nutrient deficiency or taking a supplement for another medical reason. It also means that a supplement claim should not be used to delay, stop, or change prescribed cancer treatment. Discuss treatment decisions with the clinicians responsible for your care.

The evidence about antioxidants during chemotherapy and radiation is mixed, and some studies have found worse outcomes. That does not mean every antioxidant necessarily interferes with every treatment; the relevant interaction depends on the supplement and the specific therapy. NCI discusses possible interactions in its patient information on Cancer Therapy Interactions With Foods and Dietary Supplements and its professional PDQ summary.

Before starting, stopping, or changing a supplement while receiving cancer treatment, give your oncology team the product name, ingredients, dose, and schedule. Do not change or delay cancer treatment because of a supplement claim. The available evidence does not provide a universal interaction list that can replace advice based on your particular regimen.

How to interpret vitamin-and-cancer claims
- Check the outcome. Preventing a first cancer diagnosis, reducing cancer deaths, treating an existing cancer, and preventing recurrence are different claims. Evidence for one does not automatically establish another.
- Check who was studied. A result in smokers or people exposed to asbestos cannot automatically be applied to people without those risk factors.
- Check the exact product and dose. Findings for a particular formulation, combination, and dose do not prove that a different product works or has the same risks.
- Distinguish association from cause. Observational links between nutrient intake and cancer do not, by themselves, show that a supplement changes cancer risk.
- Separate medical need from cancer claims. Treating a deficiency can be medically appropriate without being a cancer-prevention or cancer-treatment strategy.
