Can an intelligent knife tell if tissue is cancerous? In research studies, the iKnife system rapidly classified tissue by analyzing chemicals in the aerosol produced during electrosurgery. That is promising, but it does not mean the device is a proven, routinely available cancer test: the evidence described here does not show that it replaces pathology or improves patient outcomes.
How the iKnife analyzes tissue
An electrosurgical knife uses electrical current to heat and cut tissue, producing aerosol sometimes described as surgical smoke. In an iKnife setup, that aerosol is directed to a mass spectrometer, which measures chemical signals. Classification software compares the resulting molecular profile with reference patterns associated with different tissue types.
The method is called rapid evaporative ionisation mass spectrometry (REIMS). The phrase “smells cancer” is a metaphor: the system chemically analyzes tissue-derived aerosol; it does not literally smell tissue or recognize cancer visually. Imperial College London describes the original arrangement as an electrosurgical knife connected to a mass spectrometer and a reference library (Imperial College London, 2013).

What the studies have reported
Early operating-theatre feasibility work
In its 2013 account, Imperial College London said researchers used tissue from 302 surgery patients to build an initial reference library. In a subsequent 91 operating-theatre tests, the reported tissue classifications matched postoperative diagnoses. Surgeons could not see the iKnife readings during those tests, so the work demonstrated classification feasibility—not that acting on a live result changed surgical decisions or patient outcomes. The early library included samples from brain, lung, breast, stomach, colon and liver tumors; that range should not be read as equal validation or an approved use for each cancer type (Imperial College London, 2013).

Endometrial cancer pilot
A prospective, single-centre pilot examined Pipelle biopsy samples collected from patients undergoing clinical evaluation. The publication record reports 150 patients and 453 spectra. A 2022 NIHR Imperial Biomedical Research Centre summary reported 89% diagnostic accuracy and an analysis time of 1.8 seconds for this pilot’s model and samples—not for all cancers or for routine clinical use (NIHR Imperial BRC, 2022; Imperial College London publication record).
The pilot used a modified handheld diathermy connected to a mass spectrometer and analyzed ionic lipid species in surgical aerosol. Its summary identified larger datasets and more uniform sample conditions as needed. The reported accuracy and speed are specific to this study; they do not establish performance across other cancers, settings, or patient groups.

Breast cancer surgery trial
The REI-EXCISE trial studied iKnife assistance during breast cancer surgery. Cancer Research UK’s trial summary says recruitment ran from 2017 to 2019 and that results were reported in 2024, but the summary reviewed here does not provide detailed outcome measures. It therefore cannot support a claim about the system’s performance or patient benefit in that trial (Cancer Research UK, REI-EXCISE summary).

Fast chemical analysis in a study is not the same as a confirmed diagnosis available to patients in routine care. In particular, the reported 89% figure belongs to the endometrial pilot; it is not a general accuracy rate for the iKnife. The early theatre tests kept readings from surgeons, while the reviewed trial summary for breast surgery does not give detailed outcomes. Together, these limits mean the evidence presented here does not establish that clinicians can rely on iKnife results to guide treatment, or that use of the system improves survival, reduces recurrence, or replaces histopathology.

On 6 January 2023, Professor Sadaf Ghaem-Maghami described the technology’s potential for rapid-access clinics assessing abnormal vaginal bleeding. That statement expressed a hoped-for application, not proof that the proposed impact had been achieved (Imperial College London, 2023). The reports cited here do not establish regulatory or standards-body endorsement, routine availability, or current commercial status.

A 2024 Imperial College Healthcare NHS Trust report described an AI system that analyzes medical scans to help classify lung tumors. That is an imaging-based “virtual biopsy,” not the electrosurgical iKnife and its REIMS analysis of aerosol (Imperial College Healthcare NHS Trust, 2024).
