Hydrocortisone, a medication related to the body’s stress hormone cortisol, is being studied in two different ways: soon after trauma to try to prevent later PTSD, and alongside therapy for people who already have PTSD. Neither use is established routine treatment. Early prevention studies show mixed, preliminary results; for established PTSD, the 2023 VA/DoD guideline recommends against hydrocortisone alone and says evidence is insufficient to recommend for or against adding it to psychotherapy.
What “stress hormone treatment” means
Cortisol is a hormone the body produces. Hydrocortisone is a pharmaceutical medication used in the clinical studies discussed here. The evidence does not establish that over-the-counter cortisol products or at-home cortisol testing treat PTSD or help choose a treatment. The 2021 review of hydrocortisone studies and the VA’s medication guidance concern clinical treatment, not consumer products.
Hydrocortisone soon after trauma: possible prevention, not routine care
Prevention research asks whether giving hydrocortisone shortly after a traumatic event can reduce later PTSD symptoms or diagnoses. These studies differ in the kind of trauma, patient population, timing, dose, and outcome measured, so a positive result in one group does not establish a general preventive treatment.
What pooled studies found
A 2021 systematic review and meta-analysis included eight studies, nine effect sizes, and 362 participants. Across the included trials, the authors reported fewer PTSD symptoms with hydrocortisone (standardized effect d = 0.96; 95% CI 0.22–1.69; p = 0.011) and lower PTSD incidence (reported log risk ratio = 0.85; 95% CI 1.12–1.59; p = 0.023). The preventive-study subgroup showed a statistically significant effect (d = 1.50; 95% CI 0.30–2.69; p = 0.014), while the subgroup treating established symptoms did not (d = 0.28; 95% CI −0.11 to 0.66; p = 0.161). The authors cautioned that few studies qualified and study quality was limited; the pooled results are preliminary, not a basis for routine treatment. Read the 2021 review.

Why individual trials do not give a simple answer
One randomized trial enrolled 64 adults with traumatic physical injuries and tested a 10-day low-dose hydrocortisone course begun within 12 hours. Its authors reported fewer PTSD and depression symptoms and better health-related quality of life in the hydrocortisone group during the first three months. That small early-intervention study applies to acutely injured patients; it does not show that hydrocortisone prevents PTSD for trauma survivors generally. See the injury trial.
In a separate randomized, placebo-controlled emergency department study, 118 people with acute stress symptoms received one IV dose within six hours of trauma or placebo. At 13 months, the groups did not differ overall in PTSD prevalence or symptom severity. A 2019 review of 19 randomized trials found that most studies were methodologically weak. Its positive hydrocortisone-prevention evidence came from people with severe physical illness, which limits how confidently those findings can be applied to other trauma survivors. Read the 2019 review.
Hydrocortisone added to therapy for established PTSD
Augmentation means adding a medication to a treatment such as prolonged exposure therapy; it is different from trying to prevent PTSD immediately after trauma. In a randomized, double-blind trial involving 60 U.S. veterans with PTSD, participants received 30 mg oral hydrocortisone or placebo before prolonged exposure sessions. Symptoms improved over time in both groups, but the intention-to-treat analysis found no measurable overall improvement in symptoms or secondary outcomes from hydrocortisone. A 2014 pilot trial randomized 24 veterans to hydrocortisone or placebo before prolonged exposure sessions and associated hydrocortisone with greater symptom reduction, which the authors attributed to better treatment retention. Its small sample makes it preliminary, particularly alongside the later, larger trial’s null overall result. See the pilot trial.
What current PTSD treatment guidance recommends
The 2023 VA/DoD guideline gives a Strong Against recommendation for hydrocortisone as monotherapy—meaning treatment with hydrocortisone alone. Recommendation 20 states: “We recommend against treating PTSD with divalproex, tiagabine, guanfacine, risperidone, benzodiazepines, ketamine, hydrocortisone, or D-cycloserine, as monotherapy due to the lack of strong evidence for their efficacy and/or known adverse effect profiles and associated risks.” Separately, the guideline finds insufficient evidence to recommend for or against hydrocortisone augmentation. The distinction matters: the monotherapy recommendation does not settle every research question about adding hydrocortisone to therapy. Read the VA/DoD guideline.

The guideline recommends specific trauma-focused psychotherapies over pharmacotherapy and identifies paroxetine, sertraline, or venlafaxine as medication options. A 2019 APA guideline summary likewise strongly recommended several psychotherapies and conditionally recommended medications; it does not establish stress hormones as routine PTSD treatment. When comparing a study or treatment claim, check whether it concerns prevention shortly after trauma or established PTSD; whether participants had acute medical illness or injury; whether hydrocortisone was used alone or added to psychotherapy; and whether the evidence comes from a small pilot, a larger randomized trial, or a pooled review. Also note what was measured—symptoms, diagnosis, treatment retention, or another outcome—and the length of follow-up. These differences explain why promising early-intervention findings cannot by themselves justify treating established PTSD with hydrocortisone.
