Haptic VR gloves can add touch-like cues to surgical simulation, but they do not make a virtual procedure feel identical to operating on a patient. SenseGlove NOVA gloves were used in a neurosurgical training pilot to provide vibration and finger resistance; separate studies suggest haptics can improve some training outcomes, while also showing that benefits depend on the task and the feedback remains incomplete.
What are the gloves, and what do they add to VR?
SenseGlove NOVA is a wearable haptic device used alongside a VR headset and VIVE tracking. In the neurosurgical simulator pilot, the setup also used SenseCom software to connect the gloves to an Unreal Engine environment. The glove combines a soft wearable with a hub, vibrotactile actuators and mechanisms that resist finger movement.
Those features provide two kinds of cues: vibration can signal an event, while finger resistance can make a virtual interaction feel less like moving an unimpeded hand through empty space. They do not amount to a complete reproduction of touch or surgical force. The distinction matters: a headset can show a convincing scene, but realistic-looking imagery and realistic physical feedback are separate parts of simulation.
How were NOVA gloves used for neurosurgical training?
A 2024 pilot study in Frontiers in Robotics and AI integrated the gloves into an external ventricular drain (EVD) training simulator. Thirty neurosurgery residents used the system at the 2022 Canadian Neurosurgery Rookie Bootcamp.

The simulated task included drilling a burr hole, placing a catheter, responding to pressure changes and receiving a performance score. This is evidence that a glove-based haptic system can be incorporated into a specific neurosurgical training exercise. It is not, by itself, evidence that trainees who used the gloves became better surgeons, or that the simulator reproduces the full sensations of an operation.
Do haptic gloves improve surgical skills?
Evidence from other procedures offers a more direct test of whether haptics can affect performance. A 2022 randomized controlled trial in Annals of Medicine and Surgery compared haptic and non-haptic VR for tibial bone drilling among 31 trainee doctors. The haptic group performed better on several measured outcomes, although it took longer to drill:

The trial supports a task-specific conclusion: adding haptic feedback was associated with better results on the reported drilling measures, but not with faster completion. It tested tibial drilling, not the NOVA EVD pilot, so its outcomes should not be treated as proof of effectiveness for every glove, simulator or surgical procedure.

Six trials found haptic VR more effective for skill training; two found no significant task differences, and the survey reported decreased realism. Taken together, the findings are promising but mixed rather than a guarantee that haptics improve every form of surgical training.

What still feels unlike real surgery?
The NOVA pilot primarily used vibration and finger resistance. Its authors noted that the gloves lacked full kinesthetic feedback—the richer sense of forces and movement through the hand and body—and called for more controllable, quantitative rendering. A separate paper on laparoscopic platforms likewise identifies realistic tissue simulation and force calculation as unresolved challenges.
That gap helps explain why “more immersive” should not be confused with “physically faithful.” A vibration cue or resistance at the fingers can make an interaction more informative, but it does not establish that the trainee is feeling the same tissue behavior, instrument forces or tactile detail as in an operating room. One survey in the 2020 review even reported decreased realism, a reminder that adding hardware does not necessarily make every simulation feel more convincing.

How to judge a surgical haptic simulator
For hospitals and training programs comparing systems, the glove alone is not enough to judge value. The training task, software, tracking and evidence all shape what the system can teach. Useful questions include:

- What feedback does it provide? Distinguish vibration cues from mechanical force or resistance, and check whether the system offers both.
- Which outcomes have been validated? Look for measured performance results, not only reports that a setup was demonstrated or felt immersive.
- Does it work with the intended simulator? Confirm tracking, software and procedure compatibility; the NOVA pilot used VIVE tracking, SenseCom and Unreal Engine.
- Which procedures are covered? Evidence for one task, such as tibial drilling or EVD training, does not establish performance in another.
- Can practice be repeated consistently? Ask whether the simulator can deliver controllable, quantitative feedback and comparable task conditions across sessions.
- What is the institutional cost? The review characterizes haptic simulation as expensive and nascent; the available evidence here does not establish a current purchase price or total cost of deployment.
For a training decision, controlled comparisons are more informative than a technology demonstration, but even a positive trial answers only the question it tested. The strongest case for a system comes from validated outcomes on the procedures and skills the institution actually needs to teach.
