Engineered antibodies may help direct cancer-killing drugs toward tumors, but they do not make chemotherapy harmless or guarantee fewer side effects. Antibody-drug conjugates (ADCs) are designed to improve the therapeutic window by delivering a potent payload to cells bearing a target; clinical evidence shows that their risks vary by drug and can still be serious.
What is an antibody-drug conjugate?
An ADC combines a monoclonal antibody that recognizes a cancer-associated target with a cell-killing drug, often a chemotherapy payload. The antibody is intended to guide the payload toward cells bearing that target. This is one form of targeted therapy: the National Cancer Institute explains that targeted treatments act on proteins involved in cancer-cell growth, division, and spread, and that some monoclonal antibodies are combined with cell-killing substances such as chemotherapy drugs (NCI: Targeted Therapy for Cancer).
The goal is more focused delivery and a wider therapeutic index—not exposure limited exclusively to tumor cells. As Paolo Tarantino and colleagues wrote in a 2023 review in Nature Reviews Clinical Oncology, “Despite being designed with the rationale of expanding the therapeutic indices of conventional chemotherapies, most ADCs have a toxicity profile similar to that of their cytotoxic payload.” ADCs can also cause effects related to the target or other aspects of the treatment.

Do ADCs have fewer side effects than chemotherapy?
There is no class-wide answer that an ADC is safer than conventional chemotherapy. A 2023 systematic review and meta-analysis by Suzuki and colleagues pooled results from 20 randomized trials involving 10,075 patients. Compared with the trials’ control groups, ADC-based treatment was associated with higher odds of several all-grade adverse events, lower odds of febrile neutropenia, and no overall increase in high-grade adverse events.

These pooled results combine different ADCs and trial comparisons. The risks for a specific patient depend on the medicine, its payload, cancer type, dose and schedule, comparator, and patient factors. What side effects can ADCs cause?
Possible effects depend on the specific ADC and its payload. The pooled trial analysis found differences across fatigue, appetite loss, nausea, nerve symptoms, blood-cell effects, and liver-enzyme elevations. A 2023 clinical review also describes on-target and off-target toxicities, including potentially life-threatening effects that are not fully understood.

Antibody treatments as a broader group can be associated with allergic reactions, cardiovascular effects, inflammatory lung disease, or cytokine release syndrome, according to the NCI’s overview of monoclonal antibodies. These are general possibilities across antibody treatments; they should not be assumed to apply to every ADC. How is an ADC chosen for a patient?
The target and the cancer context matter. Biomarker testing can help determine whether a targeted therapy may be appropriate, but an ADC’s suitability depends on its indication and the individual treatment situation. An approval or treatment development for one cancer subtype does not establish that the same medicine is appropriate for another. The NCI describes ADCs among targeted-therapy approaches in its breast cancer research information, which is specific to that disease area (NCI: Advances in Breast Cancer Research).

- The cancer type, target, and biomarker eligibility.
- The ADC’s payload and its characteristic toxicities.
- Which adverse events occur, and how often they are severe, including grade 3 or higher events.
- The treatment setting, dose, schedule, and comparator used in the relevant trial.
- Evidence of cancer control alongside safety; toxicity alone does not establish the better treatment choice.
How could future ADCs reduce risk?
Researchers and clinicians are pursuing several ways to improve tolerability: optimizing dose and schedule, modifying the antibody, linker, or payload, identifying biomarkers that predict toxicity, and developing diagnostic tools to anticipate or detect adverse effects. These are active strategies, not guarantees that an ADC will avoid toxicity for a particular patient. Tarantino and colleagues discuss these approaches in their 2023 review, “Optimizing the safety of antibody–drug conjugates for patients with solid tumours”.
