Yes—but only as a discovery and summarization aid. An AI literature-search answer can help a clinician locate candidate studies, guidelines and review articles. It cannot establish that a citation is real, that a summary is accurate, that the evidence is complete, or that a finding applies to a particular patient. Before an AI-derived claim affects care, independently verify the source, appraise the evidence, compare it with applicable guidance, and apply clinical judgment, patient preferences, privacy rules and local governance.

What an AI literature-search result is—and is not

Literature-search systems that use generative AI may translate a clinical question into search terms, retrieve papers, rank results, summarize findings, classify studies or show citations. Those functions can reduce the time needed to begin an evidence search. The result is still an interpretation produced by a tool, not a peer-reviewed evidence review.

The American Speech-Language-Hearing Association (ASHA) warns that AI-generated evidence summaries are not peer-reviewed or critically appraised and should be treated as a starting point rather than a definitive source. Its guidance also identifies practical failure modes: a system may oversimplify a study, draw a conclusion from an introduction or discussion instead of the reported results, or provide a citation that is fabricated, mismatched or inaccurately described. See ASHA’s guidance on using AI to support evidence-based practice.

That distinction matters in patient care. A generated paragraph can sound precise while omitting a study’s exclusion criteria, confidence intervals, adverse events, comparator, follow-up period or limitations. A genuine paper can also be irrelevant to the patient in front of you. The output therefore helps you find and organize evidence; it does not replace source reading, critical appraisal or shared decision-making.

What clinicians may reasonably use AI search for

Use depends on the tool, its validated purpose and your organization’s rules. Commonly defensible uses are preparatory and administrative rather than autonomous treatment decisions.

  • Expanding a search: Generate synonyms, related conditions, intervention names, study-design terms and possible subject headings to test in an established database.
  • Finding candidate sources: Identify potentially relevant primary studies, systematic reviews, guidelines, safety alerts and ongoing-study records.
  • Orienting yourself to a topic: Obtain a provisional map of concepts before reading the underlying literature.
  • Classifying or screening: Use AI assistance to sort records for human review, provided the screening decision is checked and the workflow is auditable.
  • Extracting structured fields: Ask for population, intervention, comparator, outcomes, follow-up and study design as a reading aid, then verify every field against the paper.
  • Visualizing evidence: Use supported tools to organize study relationships or evidence gaps, without treating a visualization as an appraisal.

The National Institute for Health and Care Excellence (NICE) describes potential AI assistance with evidence identification, search-strategy generation, study classification, screening and visualization. It characterizes automated data extraction as less established. This position statement concerns evidence generation, not an endorsement of using an AI answer to select treatment for an individual patient: NICE’s position statement development page.

Why a citation and a summary still require checking

A citation may be nonexistent or mismatched

Open each citation that matters. Confirm the title, authors, journal, year, DOI or database record and publication type. Then locate the passage or table supporting the exact claim. A paper that discusses an outcome in its background is not evidence that the study demonstrated that outcome. If a link fails, details conflict or the cited passage does not support the statement, remove the claim or find the original source yourself.

A true statement may be presented with the wrong strength

AI can turn an association into a causal claim, a relative effect into an absolute one, a statistically significant result into a clinically important benefit, or an exploratory subgroup into a general recommendation. Check the design and the authors’ own wording. In a randomized trial, inspect allocation, blinding, attrition, analysis population and prespecified outcomes. In an observational study, look for confounding, selection bias and the limits of causal inference.

A list of papers is not a complete evidence base

A generated result set does not prove that all relevant evidence was found. It may miss older studies, negative results, non-English literature, unpublished work, updates, corrections or evidence outside the tool’s indexed sources. The Centers for Disease Control and Prevention’s ACIP GRADE Handbook explains why systematic methods define a question, document search and selection criteria, appraise studies and record search dates. Those methods reduce selective citation and improve decision reliability. See Chapter 6 of the ACIP GRADE Handbook.

A verification workflow before an AI result affects care

  1. Frame the clinical question. State the population, intervention or exposure, comparison and outcomes (PICO) where appropriate. Add timing, setting, severity, comorbidities and harms that matter to the decision. A vague prompt such as “best treatment” encourages a vague and potentially selective answer.
  2. Record the search context. Note the tool name and version if shown, the date, exact prompt, filters, databases or source collections and whether patient information was entered. This makes the process reproducible and helps you detect changes when a tool is updated.
  3. Inspect the source list before reading the synthesis. Open every citation supporting a recommendation, safety statement, contraindication or numerical effect. Verify that the article exists and that its bibliographic details match.
  4. Read the primary evidence. Check the methods, population, comparator, outcomes, statistical analysis, harms, follow-up and limitations. Confirm that the quoted result appears in the results section or a clearly identified table, rather than being inferred from background or discussion.
  5. Find higher-level evidence. Search for current systematic reviews, evidence reports and professional guidelines. Check their search date, inclusion criteria, risk-of-bias methods, conflicts of interest and whether newer studies could change the conclusion.
  6. Compare the AI claim with the whole evidence set. Look for contradictory trials, differences in effect size, important adverse outcomes and evidence certainty. Do not average disagreement into a confident sentence; investigate why studies differ.
  7. Test applicability to the patient. Ask whether the patient resembles the study population in age, disease severity, comorbidities, medications, pregnancy status, socioeconomic context and care setting. Consider whether the measured outcome matches what this patient values.
  8. Obtain the appropriate clinical and governance review. Discuss uncertainty with the relevant clinician, pharmacist, specialist, methodologist or local evidence team. Escalate a conflict between AI output and the primary source, guideline or clinical assessment rather than silently choosing the AI answer.
  9. Document and correct. Before placing information in a record or acting on it, correct errors and retain enough information about the source, tool and verification process to satisfy organizational policy. Never copy unverified generated text into the chart.

How to appraise the evidence you found

Start with design and question fit

Identify whether the source is a guideline, systematic review, randomized trial, cohort study, case-control study, diagnostic study, qualitative study, case report or commentary. The design determines which questions it can answer. A case series may describe an uncommon adverse event but cannot establish comparative effectiveness. A diagnostic-accuracy study may not tell you whether using the test improves outcomes.

Check internal validity

  • Were participants selected and analyzed in a way that limits selection and attrition bias?
  • Was the intervention compared with an appropriate control, and were co-interventions similar?
  • Were outcomes defined in advance, measured consistently and reported completely?
  • Are effect estimates accompanied by uncertainty, such as confidence intervals?
  • Were harms and discontinuations reported as carefully as benefits?

Separate statistical from clinical importance

Ask how large the absolute benefit or harm is, over what period and for which baseline risk. A relative reduction can appear impressive when the underlying event is rare. A statistically detectable difference may be too small to change a patient’s decision, while an imprecise estimate may still warrant caution when the potential harm is serious.

Evaluate reviews and guidelines as methods documents

For a systematic review, inspect the databases searched, last search date, eligibility criteria, duplicate screening, risk-of-bias assessment, handling of heterogeneity and certainty grading. For a guideline, check the evidence review, panel composition, conflicts of interest, recommendation strength and update status. A generated summary that omits these details is not a substitute for reading them.

Patient-centered care cannot be outsourced to a search result

Evidence is one part of a decision. The Australian Commission on Safety and Quality in Health Care’s AI Clinical Use Guide – Guidance for clinicians, Version 1.0 (August 2025), says clinicians should review AI outputs in the patient’s context and remain accountable for records and results that will be acted on. It states: “You remain responsible for confirming the accuracy and completeness of records, results or documentation that will be acted on by you, other clinicians or patients, now and into the future.” Read the full guide at the Australian Commission’s AI Clinical Use Guide.

Apply the evidence alongside the patient’s goals, risk tolerance, culture, health literacy, ability to access treatment, preferences about testing and treatment burden, and informed consent. An AI system does not inherently know which outcome the patient considers most important or whether a recommendation is feasible in the local service. Shared decision-making remains a conversation between the patient and care team.

Privacy, consent and local governance

Do not paste identifiable patient information into a tool merely to obtain a better search. Determine what data the service retains, where it is processed, whether it is used for model improvement, who can access it and how deletion works. Use de-identified or synthetic details when they are sufficient, and follow your organization’s approved systems and information-security policy.

Before clinical use, confirm the tool’s intended purpose, evidence base, limitations, organizational approval, incident-reporting route and any applicable medical-device or privacy requirements. The Australian guide’s references to Ahpra, the Therapeutic Goods Administration and Australian privacy law are specific to Australia; clinicians elsewhere must follow their own jurisdiction’s rules. A product’s performance can also change after a model, retrieval source or interface update, so approval should include ongoing monitoring rather than a one-time check.

Dimensions for evaluating a tool in a clinical workflow

No single accuracy rate can be applied to all AI literature-search systems. Performance depends on the tool and version, query, evidence collection, specialty, task and patient population. Evaluate the specific system for the intended use.

Evaluation area Questions to ask
Intended use Is the proposed activity within the tool’s documented and validated scope, or is it being stretched into treatment advice?
Source coverage and freshness Which databases and publication types are included? How current are records, corrections and guideline updates?
Citation traceability Can a reviewer open the original source and see the passage supporting each material claim?
Evidence appraisal Does the workflow expose design, bias, certainty, harms and applicability, or only produce fluent summaries?
Specialty and population fit Has it been assessed on the languages, diseases, demographics and care settings in which your team will use it?
Privacy and retention What patient information is permitted, retained, shared or used for service improvement?
Governance and regulation Who approved it, what instructions apply, and how are incidents, updates and withdrawal handled?
Auditability Can the organization retain prompts, citations, reviewer corrections and the final decision when policy requires?
Bias and accessibility Could language, indexing, demographic representation or interface design disadvantage a patient group?

The Society of General Internal Medicine recommends that clinicians critically appraise evidence and utility and incorporate generative AI thoughtfully; its position statement is not a validation study of every current product. See the Society’s recommendations for generative AI in medicine.

What to do when the AI answer conflicts with the evidence

If the citation cannot be verified

Stop using that claim. Search the title and authors in a trusted bibliographic database, check for a retraction or correction, and rerun the question with a clearly defined PICO. If no genuine source supports the statement, treat it as an unsupported output, not as a lead to cite.

If the source is real but the summary is wrong

Use the paper’s actual methods and results, not the generated wording. Record the discrepancy if it could affect care, notify the responsible team or tool owner through the local incident process, and avoid reusing the flawed summary for another patient.

If sources genuinely disagree

Check differences in population, intervention dose, comparator, outcome definition, follow-up and risk of bias. Seek the most current guideline or systematic review and obtain specialist or evidence-methodology input when the decision is high stakes. Clinical judgment should be informed by the underlying evidence, not overridden by a fluent output.

A quick pre-use checklist

  • The question identifies the population, intervention or exposure, comparison and outcomes.
  • Important citations exist, match their bibliographic details and support the exact claims made.
  • Primary studies have been read far enough to verify design, results, harms and limitations.
  • Current systematic reviews and applicable guidelines have been checked for search date and methods.
  • The evidence is applicable to this patient’s characteristics, setting and preferences.
  • Patient data were handled in an approved, privacy-preserving workflow with required disclosure or consent.
  • A clinician reviewed and corrected the output before it informed care or documentation.
  • The tool, prompt, sources and decision process are documented when policy or the clinical situation requires it.

The practical boundary

AI can make the first pass through a large literature faster, but speed is not proof. The safe boundary is to let the system propose sources and organize information while clinicians verify citations, appraise methods, search for missing or conflicting evidence, and decide with the patient. Responsibility for the resulting recommendation and record remains with the clinical team.
