AI models can identify certain tongue features and classify selected conditions in research datasets. That is much narrower than diagnosing health problems generally from a photo: the studies cover specific labels and patient groups, and their reported results do not establish reliable at-home diagnosis.
What tongue-image AI has actually been tested on
Researchers have trained image models for defined tasks, including distinguishing particular tongue lesions, recognizing visible attributes, and investigating selected diseases. The models are assessed against labels represented in their datasets; a condition outside those labels is not something the model has thereby shown it can detect.
| Study and task | Data and setting | Reported result and what it means |
|---|---|---|
| Tiryaki and colleagues, tongue-lesion classification (2024) | 623 clinic patients in Turkey; categories included coated, geographic, fissured tongue, median rhomboid glossitis, and normal tongues. [BMC Medical Imaging paper] | For normal-versus-lesion classification, ResNet101 reached 93.53% accuracy and fusion-based majority voting reached 95.15%. For five-way classification, VGG19 reached 83.93% and fusion reached 88.76%. These are results for the study’s categories and test data, not a general disease detector. |
| Selected gastrointestinal diseases (2024) | 2,167 tongue images from 949 patients. The paper investigated Helicobacter pylori infection, bile reflux, reflux esophagitis, gastric erosion, and duodenal erosion. [Frontiers in Medicine paper] | The authors report AUC 0.886, accuracy 0.849, and a true-positive rate of 0.965 for their model and study task. These figures apply to that five-condition investigation and cohort. |
| Dr. Tongue, remote tongue-sign recognition (AAAI 2025) | A research framework and dataset designed for telemedicine, focused on detecting multiple tongue attributes. [AAAI paper] | It addresses recognition of signs or attributes for remote assessment; the paper’s stated scope does not establish a consumer diagnostic product. |
| Oral-condition image classification (Scientific Reports, 2025) | 652 tongue images: 294 normal controls, 340 glossitis, and 17 oral squamous cell carcinoma (OSCC) images. [Scientific Reports paper] | The study examines these image classes. The 17 OSCC images are a small sample, so the result should not be read as proof of dependable cancer screening. |
| Smartphone tongue-image pipeline (2024) | A research paper studies a pipeline using smartphone-captured tongue images. [Applied Sciences paper] | Smartphone capture has been explored in research; this does not establish a validated consumer diagnostic app or endorse a particular phone or accessory. |
| Coronary artery disease feasibility study (2024) | 684 patients recruited from four hospitals in China for a disease-specific tongue-image study. [Study record at PubMed Central] | This is a targeted investigation of one disease question, not evidence of universal health screening from tongue images. |
Why a high accuracy figure does not prove a phone can diagnose you
Metrics answer different questions. Accuracy is the share of classifications that are correct in the evaluated data. A true-positive rate (also called sensitivity) measures how often a model identifies cases that have the target condition. AUC summarizes how well a model separates positive from negative cases across thresholds. They are not interchangeable, and none alone tells a reader whether a result is useful for personal medical decisions.

1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minutePerformance on a study’s test data also does not establish how the system will work in another clinic, population, or image-capture setup. Camera differences, lighting, image quality, and conditions absent from the model’s label set can all make a real-world photo a different task from the one evaluated. The figures above should therefore stay attached to each study’s specific categories and cohort.

Can you use a tongue photo for self-diagnosis?
No cited study establishes that consumers can reliably diagnose an illness by photographing their tongue. Studies have investigated image analysis for narrow classification and remote-assessment tasks; they do not demonstrate that a general-purpose phone scan can determine what is wrong with an individual. A research pipeline that accepts smartphone images is not, by itself, evidence of an approved or clinically validated diagnostic app.
If you have a concerning or persistent symptom, seek advice from a qualified health professional rather than relying on an AI tongue scan. A model’s output for a limited set of visual labels cannot rule out other causes or replace a clinical assessment.

To support use beyond a research dataset, a system would need evaluation on representative patients and images beyond the setting in which it was developed, with transparent reporting of its intended conditions, validation design, and limitations. Evidence that a model can classify images is distinct from evidence that using it improves decisions or health outcomes. The studies described here do not establish either broad diagnostic coverage or patient benefit.
