A breath test for valproic acid (VPA) is an intriguing research idea, but the 2011 study behind “Breathe out for personalised medicine” did not establish a clinical test or a replacement for blood monitoring. Researchers detected a VPA-related signal in breath and found that it tracked free VPA in blood in a small group of patients. The method relied on specialist mass spectrometry; the sources available here do not establish that it is routinely used or commercially available today.
What “Breathe out for personalised medicine” describes
The title comes from 2011 coverage of a proof-of-principle study by Gamez and colleagues. The researchers explored whether a chemical produced during VPA metabolism could be measured in exhaled breath and used to follow drug levels without taking a blood sample for each breath measurement. That is an experimental analytical approach, not a consumer breathalyzer or a validated way for patients to adjust medication.
The primary paper, “Real-time, in vivo monitoring and pharmacokinetics of valproic acid via a novel biomarker in exhaled breath”, appeared in Chemical Communications in 2011. Its authors described the work as “only a first example of how EESI-MS can be used for following the metabolism of drugs in vivo.”

A proposed metabolite marker
The team used extractive electrospray ionization mass spectrometry (EESI-MS) to analyze exhaled breath. The proposed marker was 4-hydroxy-VPA-gamma-lactone, detected as a signal at m/z 143. The paper also considered 5-hydroxy-VPA-delta-lactone as a less favored alternative assignment, so the exact identity was not presented as wholly settled.

Laboratory equipment, not a home test
EESI-MS was performed with a minimally modified electrospray source on a commercial quadrupole time-of-flight mass spectrometer. The authors noted that, in their setup, breath analysis avoided sample storage and pretreatment. But breath analysis also presents analytical challenges, including separating, identifying, and quantifying compounds consistently. The study therefore demonstrated a specialist laboratory method, not a simple sensor patients could use at home.
What the study found—and what the numbers mean
The reported association was promising within the study dataset, but the sample was small and all participants in the comparison were male. The reported results were:

- The marker was consistently detected across 36 measurements from 6 patients receiving VPA.
- Signals in 18 measurements from 3 controls were comparable to background.
- The breath signal was linearly associated with the free fraction of VPA measured in blood: R² = 0.89, p < 0.01.
- The association with weight-adjusted daily dose was weaker: R² = 0.76, p < 0.01.
These values describe correlations in this small 2011 study; they are not measures of diagnostic accuracy or evidence that the method works across a broader patient population. The paper also reported estimated VPA half-life values of 7 to 10.5 hours in several volunteers. That was pharmacokinetic tracking, not validation of a dosing algorithm.
Why measuring VPA can matter
VPA is strongly protein-bound, and the free fraction is the pharmacologically relevant portion. The 2011 paper explains that factors such as serum albumin and displacement by some other strongly protein-bound medicines can affect that fraction. Therapeutic drug monitoring can help clinicians assess drug levels when considering dosage and side effects. The paper described blood tests every few weeks for new patients initially while the therapeutic window is established; that is background from the study, not patient-specific guidance.

The breath approach aimed to follow a metabolite signal associated with free VPA, rather than merely infer exposure from the prescribed dose. Whether a breath measurement could reliably support clinical decisions would require validation beyond the small study.
Is this breath test used in care today?
The cited sources do not establish clinical validation, regulatory status, routine use, or current commercial availability for this specific VPA breath technique. A 2022 paper, “Personalised therapeutic management of epileptic patients guided by pathway-driven breath metabolomics,” provides later research context on breath metabolomics in epilepsy, but it does not establish that the 2011 VPA test has become a clinical service or replacement for blood monitoring.

For the original study, the distinction is important: it showed that a breath signal could be measured and related to free VPA in blood under research conditions. It did not show that a patient or clinician can use a breath result today to determine a safe or effective dose. Decisions about VPA monitoring and treatment remain matters for the prescribing clinician.

A practical clinical test would need to show more than a relationship in a small initial sample. Important questions include whether the metabolite can be identified and quantified reliably, whether results hold in larger and more representative groups, and whether the breath measurement adds dependable information about the clinically relevant free-drug fraction. The available sources do not answer those implementation questions for this specific technique.
