Bounded agent repair is a policy for limiting how many times an agent can revise work after review. But a repair limit is not evidence that review demand has fallen. To measure that outcome, teams need a defined first-pass acceptance rate and linked records that follow changes from rejection through repair or human disposition.
What first-pass acceptance and bounded repair mean
In an article by James Smith for AFT Group, first-pass acceptance means a change completes its configured review and evidence path without another implementation pass. It is an outcome measure—not a measure of effort, elapsed time, code volume, number of attempts, or code quality in the abstract.
Bounded repair treats another implementation pass as a limited exception rather than an open-ended loop. In the described policy, only blocking or major findings trigger repair. Each risk band has a hard-capped repair budget, while minor observations do not automatically send work back to implementation. The article does not publish the severity criteria, risk-band definitions, or cap values, so those details cannot be inferred.

When a budget is exhausted, a person takes over disposition. They might narrow the requirement, settle a disputed rule, split the change, or reject the implementation approach. The cap is presented as a safety boundary, not a throughput target. These are policy statements in the article, not independently verified industry findings.

Why a repair count does not measure review demand
First-pass acceptance and repair-cycle distribution describe different events. The former asks whether an eligible change was accepted without another implementation pass. The latter describes how many repair cycles occurred. A distribution showing only a no-repair category cannot, on its own, explain changes rejected at another gate.
The two measures cannot be reconciled unless they use aligned cohorts and denominators and account for all relevant outcomes. In the source article, available records do not fully account for rejected, repair-ineligible, abandoned, or human-routed changes. A count of repair executions therefore cannot establish how much review work occurred or how many changes failed first-pass acceptance.

These are complementary measures, not interchangeable rates. Combining them before their cohorts and denominators align risks treating missing outcomes as if they were successful or irrelevant.
Define the acceptance rate before calculating it
A usable first-pass acceptance rate is the number of eligible changes accepted without another implementation pass divided by all eligible changes entering the configured review and evidence path, for a stated cohort and measurement period. This is a proposed definition; the source provides no underlying counts from which to calculate a rate.

- Denominator: all eligible changes entering the configured path in the same cohort.
- Cohort and period: state which changes are included and the time window covered.
- Inclusion and exclusion rules: explain what makes a change eligible and what is excluded.
- Abandoned and human-routed work: specify how each is handled rather than silently dropping it.
Without those choices, a percentage can appear precise while answering an unclear question. The article explicitly omits a previously reported 91.7% figure because its numerator, denominator, cohort, and measurement period are undocumented; it should not be treated as a verified result.
Link each change from rejection to final disposition
To measure review demand, teams need more than a count of accepted changes or repair attempts. Records should preserve the lifecycle and the reasons behind transitions. The article describes an intended workflow, not a verified event schema:

- A change enters the configured review and evidence path.
- It is accepted, or rejected and assessed for repair eligibility.
- If eligible and budget remains, it may receive bounded repair.
- If it is ineligible or its budget is exhausted, it goes to human disposition or re-specification.
- The record captures the final disposition.
At minimum, track separate states for accepted, rejected, repair-eligible, repaired, abandoned, and human-routed work. Preserve why a rejected change was or was not eligible, and link each event to the same change through its final disposition. Then test whether the states are mutually exclusive and collectively exhaustive: every in-scope change should land in one accounted-for outcome, without double counting.

Keep the ambiguity explanation as a hypothesis
The article proposes that incomplete acceptance conditions, unresolved business rules, unclear ownership of side effects, or plans that defer too much design may create pressure for a second implementation pass. It labels this a working hypothesis; it does not quantify the relationship.

Nor does the article establish that approved planning improves first-pass acceptance. Plans are selected partly according to risk and complexity, so a simple comparison of planned with unplanned changes could reflect which work received a plan rather than the plan’s effect.
What evidence supports an effectiveness claim
The source, published on DEV Community on August 28, 2026, is attributed to James Smith for AFT Group and links to the original AFT Group Engineering Insights article. It presents an operational policy model, not an independent evaluation. The article says there is no sourced change trace from rejection through final disposition and no aligned cohort count to assess review demand, acceptance outcomes, or defect-escape effects.
Accordingly, it does not establish that bounded repair reduces review demand, lowers defect escape rates, or improves acceptance. Those are effects to measure, not conclusions supported by the described policy. As Smith writes, “The objective is not to make agents better at looping. It is to stop ambiguity reaching review.”
