Bone-conduction headphones turn an audio signal into vibrations that travel through tissue and skull bone to the cochlea. Unlike ordinary headphones, they do not send sound through the ear canal and eardrum. Most consumer models leave the ear canal open, so outside sounds remain accessible—but that does not guarantee awareness of every hazard or protect hearing from excessive volume.
How do bone-conduction headphones work?
A small transducer rests against the cheek or temporal-bone area and vibrates in response to an electrical audio signal. Those vibrations pass through surrounding tissue and bone to the cochlea, the inner-ear structure that converts movement into signals the auditory system processes. The inner ear is involved in both bone- and air-conduction hearing.
With ordinary headphones, sound moves through the air in the ear canal, vibrates the eardrum and middle-ear structures, and reaches the inner ear. Bone conduction changes the route to the cochlea; it does not send sound directly to the brain or create a separate sense of hearing. When might you use them?
Consider them when you want to listen without sealing the ear canal and value access to surrounding sound—for example, while walking, running, or exercising. The open-ear design is a trade-off: it can make ambient sound easier to hear, but background noise can compete with the audio. Headphones can also distract or mask sounds, so open ears are not a guarantee of safety. Use judgment around traffic and follow local rules.

Fit and sound are personal, and the useful choice depends on your surroundings. Try a pair in the setting where you expect to use it rather than assuming one style will suit everyone. Open-ear designs can also leak sound, particularly in quiet places or at higher playback levels, which may matter to people nearby.
Are they safe for hearing?
Bone conduction still delivers vibrations to the inner ear. Bypassing the ear canal and eardrum does not make loud listening harmless, and the evidence cited here does not establish a bone-conduction-specific safe volume threshold. Keep playback at a sensible level and reduce it if it becomes uncomfortable; do not treat the open-ear design as hearing protection.

Can you swim with bone-conduction headphones?
Check the exact model’s water rating and instructions. Resistance to sweat or rain does not establish that a device is suitable for submersion. For example, Shokz says its OpenRun is IP67-rated but does not recommend it for underwater swimming. Product specifications can change, so check the current manufacturer listing before relying on them.
Are consumer headphones the same as bone-conduction hearing aids?
No. Consumer headphones are for listening to audio; they should not be presented as treatment for hearing loss. Bone-conduction hearing aids are medical devices intended for particular hearing conditions. The FDA notes that they can bypass problems in the outer or middle ear, and the American Academy of Otolaryngology–Head and Neck Surgery says bone-conduction hearing devices are appropriate, and sometimes preferred, for certain cases of conductive or mixed hearing loss, subject to regulatory indications.
