There is no mouse proven to relieve arthritis across users. The best option is the one that fits your hand, lets you keep your wrist straight, and does not require painful gripping or movement. Rather than presenting an unsupported “top 15,” this guide compares the main mouse styles and explains how to assess specific models before buying.

What to look for in a mouse for hand pain

Choose by the movements your hand can comfortably make, not by an “ergonomic” label alone. A vertical mouse still needs you to move the device and operate its buttons. A trackball avoids moving the device across the desk, but you still move the ball and click. A device that is too large, too small, or made for the other hand can make your reach and posture worse.

  • Fit and handedness: Check that the model is intended for your operating hand and that your fingers can reach the controls without stretching. If possible, try it before committing to long-term use. OSHA recommends testing a device for fit and feel before purchase and extended use.
  • Movement: Consider whether the design relies on whole-arm movement, thumb movement, finger movement, or repeated clicking. Less desk travel does not necessarily mean less joint discomfort.
  • Wrist and arm posture: The pointer should sit close enough to the keyboard for your arm to stay near your body and your wrist to remain straight.
  • Control: Adjust pointer speed so the cursor can cross the screen without large wrist movements. It should still be controllable with a light touch.
  • Workspace: A trackball can help when there is little room to move a conventional mouse. Any pointer should be positioned so you do not have to reach awkwardly.

Which mouse style might suit you?

Conventional mouse

A conventional mouse moves across the desk as you guide the cursor. It may be a reasonable choice if you can hold it lightly, reach the buttons comfortably, and move it without bending your wrist or reaching away from your body. Size, shape, handedness, and button force matter more than the category name.

Vertical mouse

A vertical mouse changes the hand position compared with a conventional design, but it is not a treatment for arthritis. Logitech describes its MX Vertical as having a 57-degree angle and as suited to medium-to-large hands. Those are manufacturer statements about design and fit, not evidence that the mouse reduces arthritis pain. Check whether the size and button reach suit your hand before buying.

Trackball

A trackball stays in place while you move a ball to control the cursor, so it needs less desk surface and avoids repeatedly sliding the device. It still requires operating the ball and buttons. Thumb-operated models may be a poor fit if thumb motion is painful; the sources do not establish whether a particular trackball will suit an individual.

Examples include Logitech’s ERGO M575S, a wireless, thumb-operated trackball. Logitech markets it with a claim of 25% less forearm muscle strain, but that is a company claim, not an arthritis outcome. Consider the required thumb movement and try the controls if possible.

Touchpad or other pointer

A touchpad or fingertip joystick may be less fatiguing for some tasks, but there is no universal winner. OSHA recognizes these as possible alternatives, and Kaiser Permanente suggests trying a trackball or touchpad as a practical option for people with arthritis. Neither source guarantees that an alternative will reduce symptoms for every person.

Option What is established What to consider
Logitech MX Vertical Logitech lists a 57-degree vertical angle and says it is suited to medium-to-large hands. Manufacturer product page. Confirm the size and reach work for your hand. The company’s “4x less hand movement” statement is a marketing claim, not evidence of arthritis pain relief.
Logitech ERGO M575S A wireless thumb-operated trackball that stays in place as you control the cursor. Manufacturer product page. Assess whether the required thumb motion and button use are comfortable. Logitech’s 25% less forearm muscle strain figure is a company claim, not an arthritis outcome.
Evoluent VerticalMouse 4, regular or Small Evoluent says regular models are for hands about 7 inches (178 mm) or longer, measured from middle-finger tip to wrist crease; shorter hands may suit the Small model. Sizing guidance. The stated measurement is a manufacturer sizing guide, not a clinical threshold. Measure as described and check the intended hand and model details.

Set up the mouse to reduce awkward movement

  1. Place it close: Keep the mouse near the keyboard, on a surface that lets your arm remain close to your body.
  2. Keep your wrist straight: Avoid bending it up, down, or sideways while pointing and clicking.
  3. Adjust pointer speed: Choose a setting that lets you cross the screen without large wrist movements, while remaining controllable with light effort.
  4. Use a light grip: Avoid squeezing the device or pressing buttons harder than needed.
  5. Try before relying on it: OSHA advises testing a device before purchase and long-term use to assess fit and feel.
  6. Use supports carefully: A mouse pad with a palm rest may help promote a neutral wrist posture. When resting, contact should be with the heel of the hand or palm rather than pressure on the wrist during active mouse use.

Reduce uninterrupted mouse use

A different pointer does not eliminate repetitive work. OSHA recommends changing tasks, taking brief breaks, using keyboard shortcuts, and alternating hands where feasible. Its work-process guidance says, “Frequent short breaks are desirable.” These steps can reduce uninterrupted mouse use, but they are not a substitute for care when symptoms are concerning.

What the evidence does—and does not—say

Ergonomic features and manufacturer specifications can help you compare fit and movement demands; they do not establish that a mouse treats arthritis. Kaiser Permanente’s arthritis work-area advice suggests experimenting with a trackball or touchpad and notes occupational therapy as an option for adapting a work area. The advice is practical, not a promise of pain relief.

The 2026 APTA carpal tunnel guideline discusses clinician education about mouse use and alternatives such as arrow keys, touchscreens, or changing mouse hands. It addresses carpal tunnel syndrome, not every form of arthritis, and does not establish a best arthritis mouse. Its evidence update reports that no additional studies of nontraditional mice, mouse pads, tablets, or similar interventions met its inclusion criteria. Read the guideline.
