There is no proven universally best ingredient for keratosis pilaris (KP). Urea, lactic acid, and salicylic acid are all used in creams to loosen built-up surface cells; a moisturizer helps with the dryness that can accompany KP or its treatment. The limited studies offer some clues, but they do not predict which option will work best for you.
What do these ingredients do for keratosis pilaris?
KP is a usually harmless condition that causes rough bumps around hair follicles. The American Academy of Dermatology (AAD) lists urea, lactic acid, and salicylic acid among keratolytic options: ingredients that help loosen accumulated surface cells. Moisturizers serve a complementary purpose by easing dryness and supporting the skin barrier.
These treatments can improve the skin’s appearance, but they do not cure KP. Ingredient names alone also do not establish how effective a product will be; formulation, concentration, how you use it, and your skin’s tolerance all matter. AAD self-care guidance and Mayo Clinic treatment guidance describe these options and their limits.
Is urea or lactic acid better for keratosis pilaris?
There is no direct comparison here between urea and lactic acid, so the available studies cannot establish which is better. They examined different concentrations, treatment periods, and study designs.

Urea: supportive but preliminary evidence
A 2024 open-label study evaluated 20% urea cream applied once daily for four weeks. Thirty adults aged 18–65 completed the study. It reported significant improvement in measured skin smoothness and texture, with no significant adverse events. Because the study had no comparison group and follow-up was short, it does not prove that 20% urea is the right concentration for everyone or establish how it compares with other ingredients. The 2024 study in the Journal of Drugs in Dermatology provides the details.
Lactic acid: one comparative trial
Lactic acid is an alpha hydroxy acid. In a 2015 randomized study, participants applied 10% lactic acid cream twice daily for three months. The study reported a mean lesion reduction of 66% from baseline. That is an average result for that study’s participants and regimen—not a personal forecast, proof of lasting improvement, or recommendation that the same strength suits every reader.

Salicylic acid: another option in the same trial
Salicylic acid is also listed by the AAD and Mayo Clinic as a keratolytic option. In the same 2015 study, participants applied 5% salicylic acid cream twice daily for three months; mean lesion reduction from baseline was 52%. This result does not show that salicylic acid is inferior for everyone: it compares specific formulations and conditions in one study. The 2015 randomized comparison reports both study results.
What does the evidence say about the best ingredient?
The most direct comparison available here is that single 2015 randomized trial of 10% lactic acid and 5% salicylic acid. The separate urea study was uncontrolled, so it cannot be compared directly with either treatment. A 2022 review described lactic acid and salicylic acid as promising options for people aged 12 years and older, while calling for higher-quality randomized studies with long-term outcomes. That age range describes whom the review addressed; it is not evidence that every product is suitable for every person in that group. The 2022 review indexed by PubMed outlines the evidence limitations.

None of these sources establishes a winner by skin type, severity, body area, or age. A practical choice depends on the product’s directions and intended use, whether its formulation is moisturizing enough for you, and whether your skin tolerates it. If you are unsure which product or strength is suitable, a clinician can help you decide.
How to use KP creams without over-irritating your skin
- Follow the product label. Use the amount and frequency directed; applying more often or layering products is not a shortcut. The AAD warns that overuse of keratolytics can leave skin raw and irritated.
- Test a new product on one affected area. Mayo Clinic recommends trying a new product on a small area first. Acid-containing creams may sting or inflame skin.
- Moisturize after treatment. Apply a moisturizer after using a keratolytic. For routine care, the AAD recommends a thick, oil-free cream or ointment on damp skin within five minutes of bathing.
- Keep skin care gentle. Avoid vigorous scrubbing and picking at bumps; scrubbing can irritate skin and worsen KP.
- Pause if your skin reacts. If you develop irritation or excessive dryness, stop the keratolytic for a few days and let your skin recover. Follow the product’s age restrictions; Mayo Clinic notes that acidic creams are not recommended for young children.
Why maintenance may be needed, and when to seek care
KP can persist, may improve over time, and may return when treatment stops. The AAD says it cannot be cured and often requires a maintenance plan. If home care is not helping or the bumps bother you, consider seeing a dermatologist. Seek professional assessment if bumps are painful, markedly inflamed, spreading, or you are unsure whether the rash is KP; copy alone cannot diagnose a skin condition.
