Baichuan-M3 is a medical language model designed to ask for missing clinical information before it reaches a conclusion. Baichuan says it models a multi-step clinical workflow—from history taking through testing and diagnosis—rather than treating a medical prompt as a question that always deserves an immediate answer. The developer’s reported benchmark results are promising, but they are not independent proof of clinical safety or improved patient outcomes.
What makes Baichuan-M3 different?
Many language-model interactions begin with a question and produce an answer in one turn. Baichuan describes M3 as an attempt to model parts of clinical decision-making: when the initial information is incomplete, the model should seek relevant details, build a coherent reasoning path, and avoid unsupported conclusions. The company calls these goals proactive information acquisition, long-horizon reasoning, and adaptive hallucination suppression. These are the developer’s stated aims, not established guarantees about how the system performs in clinical practice. Baichuan’s announcement and its technical report describe the approach.
In practical terms, the distinction is about workflow: a medical AI may need to ask what symptoms are present, how long they have lasted, or what relevant test results show before considering possible explanations. Baichuan’s framing emphasizes gathering information and moving through stages, rather than simply generating a fluent answer from a sparse prompt.

How Baichuan says it trains M3 to follow a clinical workflow
Stage-based reinforcement learning
Baichuan’s model card describes an approach called SPAR, expanded as Step-Penalized Advantage with Relative baseline. It divides the modeled workflow into four stages: history taking, differential diagnosis, laboratory testing, and final diagnosis. Baichuan says training uses rewards for performance at individual stages as well as for the process overall, with the goal of encouraging a more coherent sequence rather than rewarding only the final answer. The model card outlines the method.

Fact-aware reinforcement learning
Baichuan also describes Fact-Aware Reinforcement Learning, which checks generated medical claims against authoritative evidence during training. The stated purpose is to make factual support part of the learning loop, rather than relying only on whether a response resembles a preferred answer. The report does not establish that this eliminates hallucinations or ensures every output is grounded in evidence.
What results does Baichuan report?
The Baichuan-M3 team’s technical report, dated February 6, 2026, reports the following benchmark scores and hallucination rate. These are publisher-reported results; the available materials do not provide independent replication.
Rank #2
| Evaluation | Baichuan-M3 reported result | What it refers to |
|---|---|---|
| HealthBench-Hard | 44.4 | Score reported in the Baichuan-M3 technical report (2026). |
| HealthBench Total | 65.1 | Score reported in the Baichuan-M3 technical report (2026). |
| ScanBench clinical inquiry | 74.9 | Score reported in the Baichuan-M3 technical report (2026). |
| ScanBench laboratory testing | 72.1 | Score reported in the Baichuan-M3 technical report (2026). |
| ScanBench diagnosis | 74.4 | Score reported in the Baichuan-M3 technical report (2026). |
| Hallucination rate | 3.5% | Rate reported in the Baichuan-M3 technical report (2026). |
The model card describes HealthBench as a benchmark of 5,000 multi-turn medical conversations created by 262 practicing physicians from 60 countries. Baichuan says M3 improved by 28 percentage points over M2 on HealthBench-Hard and exceeded GPT-5.2 in the evaluations it compared. Those comparisons are Baichuan’s account; a benchmark result is not a measure of patient outcomes, and it does not by itself show that M3 is safe for routine clinical use.

Baichuan presents ScanBench as an end-to-end workflow evaluation spanning clinical inquiry, ancillary investigations or laboratory testing, and final diagnosis. Its model card said the benchmark was planned for later open release; the official materials cited here do not establish that a public release has occurred. See the technical report, model card, and GitHub repository README.

What are the model’s limits?
The Baichuan-M3 team states: “Baichuan-M3 is currently limited to episodic, text-based clinical scenarios and does not fully capture longitudinal disease management, multimodal clinical signals, or ultra-long-horizon reasoning across patient trajectories.” The report also identifies rare high-risk errors and limited explicit grounding in evidence-based sources as unresolved challenges. The technical report is the source for these limitations.
The evidence described here covers the model’s design, training methods, and developer-reported benchmark performance. It does not establish independent validation of patient outcomes, regulatory clearance, or that M3 should replace clinician judgment. That distinction matters: strong performance on selected evaluations cannot settle whether a system handles the variety, context, and consequences of real clinical care reliably.

Is Baichuan-M3 a consumer medical app?
The official materials describe software and technical deployment, not a ready-made consumer medical service. The model card provides instructions for tools including Transformers, vLLM, and SGLang, and gives an example deployment using eight H20 GPUs. That example indicates substantial infrastructure; it is not a stated minimum requirement for every version or deployment. A reader looking for a symptom checker or personal medical advice should not treat the model’s technical availability as evidence that it is an approved or suitable clinical product.
