An Apple Watch hypertension notification is a prompt to check your blood pressure with a cuff and follow up with a healthcare professional—not a blood pressure reading or a diagnosis. In Apple’s pivotal study submitted to the FDA, the alert feature detected 41.2% of participants who met the study’s hypertension definition. Many participants with hypertension received no alert, so a quiet watch cannot reassure you that your blood pressure is normal.
Can Apple Watch detect high blood pressure?
Apple says the feature analyzes optical heart sensor data across 30-day evaluation periods to look for patterns associated with chronic high blood pressure. It does not measure blood pressure in mm Hg or provide a blood pressure value. Apple states that the feature is not intended to diagnose, treat, or manage hypertension, and that not everyone with hypertension receives a notification. Apple’s support page explains the alerts and their limits.
Eligibility is limited to people aged 22 or older who have not been diagnosed with hypertension and are not pregnant. Compatible watch and iPhone software and regional availability are also required. Check Apple’s current feature-availability page for supported locations and requirements.
How accurate are Apple Watch hypertension alerts?
The FDA 510(k) summary for Apple’s 2025 submission reports a pivotal study in which 2,229 adults without a prior hypertension diagnosis enrolled. Participants wore an Apple Watch for 30 days and used an FDA-cleared home blood pressure monitor as the reference. The primary analysis included 1,863 participants who provided at least 15 days of usable data. The FDA summary reports these study-level estimates:

| Measure | Study result | What it means |
|---|---|---|
| Overall sensitivity | 41.2% (95% CI 37.2–45.3) | The feature alerted about 4 in 10 participants who met the study’s hypertension definition; most were not alerted. |
| Overall specificity | 92.3% (95% CI 90.6–93.7) | Most participants who did not meet the study’s hypertension definition did not receive an alert. |
| Sensitivity for stage 2 hypertension | 53.7% (95% CI 47.7–59.7) | The feature alerted about half of participants in this group. |
| Specificity among normotensive participants | 95.3% (95% CI 93.7–96.5) | Most normotensive participants did not receive an alert. |
Sensitivity and specificity describe results across the study group; they are not a guarantee of what will happen for an individual. The figures also do not tell you the chance that a particular alert means you have hypertension. That depends on factors including your health and the prevalence of hypertension in the population being considered. A 2026 JAMA Network Open commentary summarizes the study results as 41.2% of participants with undiagnosed hypertension receiving an alert, while 58.8% did not; 7.7% of participants without hypertension had an inappropriate alert.
Can you trust a normal result if your watch did not alert you?
No. Because the feature missed many participants with hypertension in its pivotal study, no notification does not rule out high blood pressure. Keep up with routine cuff-based blood pressure checks and follow your clinician’s advice rather than using the watch as a substitute for screening.

What should you do after a notification?
- Measure with a cuff. Use a home blood pressure monitor authorized for your market and suitable for you. The FDA warns that unauthorized monitors may give inaccurate results; see its guidance on blood pressure devices.
- Record readings twice daily for seven days. This is the follow-up schedule Apple recommends after a hypertension notification. See Apple’s instructions for what to do after an alert.
- Share the readings and notification with a healthcare professional. Discuss the pattern and next steps. Do not use the watch alert to diagnose yourself or to start, stop, or change medication.
What the alert can—and cannot—establish
The FDA cleared Apple’s software feature through the 510(k) pathway. That is not the same as FDA approval of a cuffless blood pressure monitor, and the watch does not provide cuff-equivalent blood pressure readings. A peer-reviewed 2026 commentary discusses limitations in the validation approach and the need for evidence about real-world clinical impact and follow-up pathways. The evidence described here does not establish that the feature improves diagnosis rates or health outcomes.
