Apple Watch can do far more than count steps, but the headline “a doctor at Apple” is not verified by Apple’s published material. The nine functions below are documented by Apple and should be treated as alerts, estimates, records, or reflection tools—not a complete medical diagnosis. Availability depends on your watch model, watchOS version, paired iPhone, and country or region.

What these nine Apple Watch health features actually do

Apple Watch health tools fall into four categories: measurements, notifications, estimates, and manual logs. A notification can tell you that a reading or pattern deserves attention; it cannot establish the cause. An estimate, such as a sleep stage or fertile window, is calculated from available sensor data and is not a clinical measurement. A log is information you enter for later review.

Apple’s health-feature guide and its health overview make clear that not every Apple Watch supports every function. Before relying on any feature, check the exact model, installed watchOS, paired iPhone requirements, and regional availability.

1. High- and low-heart-rate notifications

Apple Watch can notify you when your heart rate is unusually high or unusually low while you appear to be inactive. These alerts are intended to prompt you to pay attention and, when appropriate, seek medical advice; they are not a diagnosis of a heart condition. Apple documents the controls in its health-feature support guide.

How to use the alerts

  1. Open the Watch app on the paired iPhone and review the Heart settings for high- and low-rate notifications.
  2. Choose thresholds that make sense for you and follow Apple’s on-screen explanation of when an alert can occur.
  3. If an alert is unexpected, recurrent, or accompanied by chest pain, fainting, severe shortness of breath, or other urgent symptoms, contact a clinician or emergency service rather than waiting for another reading.

The watch samples heart rate using its sensors, but a single notification does not explain whether exercise, medication, illness, stress, or another factor caused the change. Do not disable prescribed care because the watch has not alerted you.

2. Irregular-rhythm notifications that may suggest AFib

An irregular-rhythm notification looks for a pattern that may be suggestive of atrial fibrillation (AFib). Apple describes it as a screening-style notification, not confirmation that you have AFib. The feature can be useful when it prompts a conversation with a health professional, especially if you save the date and circumstances of the alert.

What the notification means

  • It indicates that the watch detected an irregular pattern during its checks.
  • It does not identify every episode, rule out AFib, or diagnose another rhythm disorder.
  • A clinician may need a medical history, examination, and formal testing to interpret the result.

Apple lists irregular-rhythm notifications alongside other heart tools in its official guide. Regional approval and model support can change, so the setting may not appear on every watch.

3. The ECG app

On compatible models and in supported regions, the ECG app records an electrical tracing that Apple says is similar to a single-lead electrocardiogram. You can create a PDF of the result and share it with a doctor, as described by Apple in Why Apple Watch.

Recording a reading

  1. Open the ECG app on the watch.
  2. Rest your arm on a table or your lap, keep still, and place a finger from your opposite hand on the Digital Crown when prompted.
  3. Wait for the recording to finish, then review the classification shown by the app.
  4. Use the Health app on the iPhone to review or export the PDF if a clinician asks for it.

A single-lead tracing is not the same as a multi-lead clinical ECG. The app cannot replace an examination, laboratory work, or emergency assessment. If you have severe symptoms, seek urgent care even if the tracing appears normal.

4. Sleep schedules, history, and estimated sleep stages

Apple Watch can record sleep duration, show recent sleep history, and let you set a sleep schedule and sleep-duration goal. Apple’s materials also present estimated time in sleep stages. These stages are estimates derived from sensor signals; they are not equivalent to a sleep-laboratory study.

Getting useful sleep data

  1. Set a consistent sleep schedule and a sleep-duration goal in Apple’s sleep tools on the iPhone or watch.
  2. Wear the watch snugly overnight and charge it before bedtime or during another part of the day.
  3. Review trends over several nights instead of treating one night as a diagnosis.

Short wear time, a loose fit, charging gaps, illness, alcohol, travel, and irregular bedtimes can all affect the record. The Apple health guide explains the sleep functions and their setup requirements.

5. Possible sleep-apnea notifications

Apple’s September 2024 technical overview says compatible Apple Watch models can track breathing disturbances during sleep. If the disturbance pattern reaches a level associated with moderate to severe sleep apnea, the watch can provide a notification of possible sleep apnea. Apple’s explanation is available in the September 2024 technical PDF.

What to do after an alert

  1. Review the breathing-disturbance information and make sure the watch was worn consistently for the nights being analyzed.
  2. Arrange a discussion with a qualified health professional, who can decide whether a sleep study or other evaluation is appropriate.
  3. Do not interpret the notification as proof that you have, or do not have, sleep apnea.

The feature is a pattern-based notification, not a measurement of airflow in a clinic. Its availability depends on supported hardware, software, and regional authorization. Apple’s PDF uses the word “possible” deliberately.

6. Vitals: an overnight personal baseline

The Vitals app brings several overnight measurements together and compares them with your personal typical range. Apple says the displayed metrics can include heart rate, respiratory rate, wrist temperature, sleep duration, and, where available, blood oxygen. If multiple metrics are outside the typical range, the app may notify you and show possible contextual factors. See Apple’s current health-feature guide for the supported set.

Why a personal range matters

Vitals is not comparing everyone with one universal “normal” number. It learns your usual overnight pattern, so a deviation can be more meaningful than an isolated value. That still does not identify the cause: a hard workout, altitude, travel, medication, infection, or poor sleep may shift several metrics at once.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Apple notes a specific limitation for Blood Oxygen: it is unavailable in the Vitals app on certain Apple Watch models purchased in the United States on or after January 18, 2024, identified by particular part-number endings. The restriction does not apply to every watch or every country, so check the model and purchase region rather than assuming Blood Oxygen is present.

7. Cycle Tracking, fertile-window predictions, and retrospective ovulation estimates

Cycle Tracking records menstrual-cycle information and uses it to provide period and fertile-window predictions. Apple says heart-rate data can improve those predictions. Supported watches can also use wrist-temperature data for period predictions and retrospective ovulation estimates. Apple warns that retrospective ovulation estimates are unavailable in some countries or regions; consult the availability information in the support guide.

Using the feature responsibly

  1. Enter period dates and other requested cycle information consistently in the Health app.
  2. Wear a supported watch overnight when you want wrist-temperature data to contribute.
  3. Use the predictions as planning information, not as contraception or diagnostic certainty.

Cycles can vary because of stress, illness, postpartum changes, perimenopause, medication, and many other factors. Persistent changes, severe pain, unusually heavy bleeding, or pregnancy concerns require professional advice.

8. Medication, vitamin, and supplement logging

The Medications app is a record-keeping tool. Apple says you add medications, vitamins, and supplements in the iPhone Health app, then log doses from the watch. It can help you see whether an entry was made and review a history, but it does not prescribe treatment, check interactions, or confirm that a dose was swallowed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Setting up medication logs

  1. Add each medication, vitamin, or supplement in the Health app on the iPhone, including the information the app requests.
  2. Open Medications on the watch when you take an item and record it as instructed.
  3. Review the history in Health and keep your clinician or pharmacist’s instructions as the authority for timing, dose changes, missed doses, and interactions.

Protect the data with your device passcode and review who can access Health information. Never double a dose simply because a log is missing; ask a pharmacist or clinician what to do.

9. State of Mind logging

State of Mind in Apple’s Mindfulness app lets you record a momentary emotion and a daily mood. Over time, those entries can help you notice relationships between mood, routines, sleep, exercise, or stressful events. Apple presents this as logging and reflection, not as a mental-health diagnosis.

Making the log useful

  • Log at roughly consistent times so entries are easier to compare.
  • Add context when the app allows it, such as work pressure, social events, exercise, or poor sleep.
  • Look for patterns over weeks rather than treating one entry as a conclusion.

If you feel unsafe, hopeless, or at risk of harming yourself or someone else, contact local emergency services or a crisis service immediately. A mood log cannot provide urgent assessment or treatment.

How to check whether a feature will work on your watch

There is no universal “all health features” switch. Use this checklist before buying a watch or troubleshooting a missing option:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Check Why it matters What to verify
Watch model Sensors and regulatory approvals differ by model. Exact model name and generation in the Watch app or device settings.
watchOS version New functions can require a minimum operating-system version. Software Update on the paired iPhone and the watch’s About screen.
Paired iPhone Setup, storage, and some analysis occur on the iPhone. Compatible iPhone software and an active pairing.
Country or region Medical features can require local authorization and may be withheld. Your Apple Account and device region, plus Apple’s current availability notes.
Data and wear time Estimates and baselines need enough usable readings. Snug fit, regular charging, and consistent overnight wear where required.

Apple’s support page is the best place to recheck changing compatibility details: Get started with health features on Apple Watch.

What these features cannot establish

None of the nine functions is a substitute for a clinician’s history, examination, or diagnostic testing. Notifications can miss events and can also be triggered by circumstances unrelated to disease. Estimates depend on sensor quality and consistent use. Logs are only as complete as the information you enter.

Apple also announced hypertension notifications in 2025 and cautioned that they will not detect every instance of hypertension. The company said it expected the feature to notify more than one million people with undiagnosed hypertension in its first year; that number is an Apple forecast, not an independently verified outcome. Read the announcement at Apple Newsroom, and check current model and regional support before assuming the alert is available.

A practical routine for using Apple Watch health data

  1. Turn on only the notifications and logs you understand and are prepared to act on.
  2. Wear the watch consistently enough to establish useful personal trends, especially for sleep and Vitals.
  3. Record relevant context—exercise, illness, travel, medication changes, or unusually poor sleep—when reviewing an alert.
  4. Export or show ECG and trend information to a clinician instead of self-diagnosing from an app label.
  5. Seek urgent care for severe symptoms regardless of what the watch reports.

The practical value of Apple Watch is continuity: it can collect a long-running record, surface a pattern, and make it easier to start a conversation with a professional. Its limits are just as important as its sensors.
