If your AI keeps giving the same apology, or you’re wondering whether an assistant’s previous answer can shape its next search, the short answer is yes. In one documented case, a voice avatar built on retrieval-augmented generation (RAG) reused its own failed answer as part of the next search query. The apology’s generic wording pulled that search away from the user’s topic, so the next answer failed in the same way, and the cycle repeated.
What the user saw
The case comes from a first-person engineering write-up on DEV Community, dated September 16, 2026. The author states that the post was originally published in Japanese at forge.workstyle.tech. In the example log, a user asked whether the avatar could hear them, asked again, and then said “I can hear you.” Across four consecutive turns the avatar returned nearly the same apology, each time saying that the corresponding page description could not be found.
How the loop formed
The author traced the behavior to an earlier change meant to help with vague follow-ups such as “Tell me more about that.” A follow-up like that carries almost no searchable topic on its own, so the system began adding the previous user utterance and the assistant’s answer to the next retrieval context. That change worked for ordinary follow-ups, but it also carried failed answers forward.
In the logged case, the prior answer was “Can you hear me? I apologize, but the corresponding description was not found.” The author’s diagnosis was that this sentence contained no user topic and introduced generic phrases such as “corresponding description.” Those words then pulled the next search toward generic text instead of the subject the user cared about. The mechanism runs as follows:

- A turn fails to find grounded content, and the assistant returns an apology.
- The apology is saved as conversation context.
- The next user turn is combined with that apology to form the search query.
- The query matches little that is relevant, so the answer is another ungrounded apology.
- That new apology becomes context for the following turn.
The author summarizes the design lesson in one line: “Designs that return output to input amplify when they fail.”
The fix: gate reuse on grounding, not on wording
The corrective rule is factual rather than phrase-based. The assistant’s answer is passed into later retrieval context only if that turn was grounded, meaning the answer was actually supported by retrieved page content. The author’s reasoning is that apology wording is an unreliable boundary. Deployments use different templates, a phrase filter can miss new variants, and a filter can also discard a useful answer that happens to contain an apology.
Update grounding state where it cannot be lost
The author also found an implementation flaw. The grounding state had been updated during best-effort conversation-log recording, which meant a logging exception could prevent the state from updating at all. The author moved the update to the end of the turn so that the decision about reuse no longer depends on the logging path.

Mark the apology path explicitly
The apology response also needed a marker, so that the system would not reuse it later even if the surrounding turn was handled differently. Marking the response as ungrounded at the point it is produced makes the later check a simple read of stored state rather than an inference from text.
Two states that should not sound the same
The case also separates two conditions that the system had been collapsing. Page text may not be available yet, or the page text may have been read and contain nothing that matches the question. The author reports that the system had been returning a “not found” response without having logged that the page text had actually loaded. The two conditions call for different responses and different logs.
| State | What it means | What the case shows |
|---|---|---|
| Page text not available | The page content was not loaded, so no search over it could have succeeded. | Reported as “not found,” which misdescribed the cause. Exact replacement wording: not stated in the post. |
| Page text loaded, no match | The content was read, and nothing in it answers the question. | The load was not recorded, so logs could not show which of the two states had occurred. |
Checks before shipping a similar fix
The author’s write-up measures the design against five criteria. Four are issues the author encountered in the case; the fifth is one the post does not report evidence for.

| Criterion | Status in this case |
|---|---|
| (a) Was the prior assistant answer grounded? | Central to the failure. Ungrounded answers were reused. |
| (b) Does the follow-up query include topic-bearing language? | Missing in the failing turns; the query carried generic apology phrases instead. |
| (c) Was the page text actually loaded? | Not logged, so “not found” was reported for an unknown state. |
| (d) Does grounding metadata survive logging failures? | Not initially. The author moved the update so logging errors cannot block it. |
| (e) Is retrieval quality measured on representative follow-up turns? | Not reported in the accessible write-up. |
In practice, a team adapting this approach would ask the same five questions of its own system, and would log each of the first four states per turn so that a repeated failure can be traced.

What the reported metric does and does not show
The post says that an earlier retrieval improvement moved an internal measurement from 0.602 to 0.741, as reported by the author in 2026. The accessible excerpt does not identify the metric, the dataset, the sample size, or the evaluation protocol. The figure should be read as the author’s internal result, not as a standardized benchmark or an independently verified gain.

The author’s principle applies whenever generated output becomes input to a later operation. Examples include a conversation summary that is fed into the next summary, generated examples that are reintroduced into training data, and a search result that becomes the next query. In each case the practical habit is to check the provenance and evidence status of generated content before reusing it. These examples are the author’s analogies; the sources used for this article do not evaluate them independently.

What broader studies say about apologies and recovery
A 2025 review of AI apology studies treats an apology as one possible trust-repair step, with mixed findings across apology components and contexts. It reports that optimistic language about reform can raise trust at first but frustrates users when the system does not improve, while a more realistic admission of limitation was found to be less frustrating and more believable in the studies it discusses. The review also notes that long-term research on repeated AI apologies is limited.
A separate 2021 exploratory study of conversational assistants found that “cannot help” responses do not guide the user’s next question. In that study, moving on without an explicit acknowledgment of the misunderstanding was the most successful recovery strategy. This is adjacent evidence about conversational recovery. It does not test retrieval contamination, and it does not validate the fix described above.

The account rests on one author’s case report, and only an excerpt of it is publicly indexed. No independent replication of the grounding-gated fix, and no larger benchmark of it, was found. The reported effect should not be read as a causal size or as a proven best practice. Whether the pattern generalizes to other RAG systems is not established. The apology and recovery studies above address how users respond to apologies, not how apology text behaves inside retrieval queries, so the two streams should be kept apart.
Taken together, the case shows a concrete failure: a generic apology stored as context can steer the next search, and the same loop can continue until grounding status, not apology wording, decides what the system carries forward.
