A typical ankle sprain affects ligaments on the outside of the ankle; a high ankle sprain affects the ligaments connecting the tibia and fibula just above the ankle joint. Pain above the ankle after the foot twists outward can raise suspicion of a high sprain, but symptoms overlap and appearance alone cannot show whether the joint is stable. A clinician can assess for fracture or syndesmotic injury and decide whether imaging is needed.
What is the difference between an ankle sprain and a high ankle sprain?
| Feature | Typical lateral ankle sprain | High ankle sprain |
|---|---|---|
| Injured structure | Ligaments on the outer side of the ankle. | The distal tibiofibular syndesmosis: ligaments joining the tibia and fibula above the ankle joint. |
| Common mechanism | The ankle rolls inward; inversion and internal rotation are typical mechanisms. | The foot rotates outward or the ankle is forced upward into excessive dorsiflexion. |
| Pain pattern | Often centered around the outside of the ankle. | May be felt above the ankle and worsen with weightbearing, pivoting, or dorsiflexion. |
| Why stability matters | Severity varies, and symptoms alone do not establish the extent of injury. | The syndesmosis helps keep the tibia and fibula aligned under load. Widening between the bones or an associated fracture can change treatment. |
These are different injury patterns, but neither pain location nor swelling reliably determines severity. The 2018 ankle-sprain clinical practice guideline and the ROAST consensus on ankle sprain assessment describe mechanisms and clinical examination as parts of evaluation; a clinician may palpate relevant structures and perform specific tests.
How can you tell if it might be a high ankle sprain?
There is no dependable at-home symptom check that confirms a high ankle sprain. Pain or tenderness above the ankle after an outward twist, particularly when weightbearing, pivoting, or bringing the foot upward hurts, is a reason to seek an assessment. Pain can also radiate up the leg.
Swelling and bruising may be less pronounced than expected. A relatively normal-looking ankle therefore does not rule out a syndesmotic injury. Conversely, outer-ankle pain after an inward roll is common in a lateral sprain, but patterns can overlap.

Clinicians consider how the injury happened, examine and palpate the ankle and lower leg, and may use tests such as the squeeze or external-rotation test. These maneuvers can provoke pain and should not be used as self-diagnosis tests.
When should you get medical care?
Arrange prompt medical evaluation after an ankle injury if you cannot bear weight, have severe pain, see a deformity, have marked tenderness directly over bone, or suspect the joint may be unstable. These signs can warrant assessment for fracture or significant ligament injury. If you have severe symptoms or are unsure whether the injury is urgent, contact a medical professional or local urgent-care service.

Do you need an X-ray or MRI?
Diagnosis starts with the injury history and a clinical examination. Imaging depends on what the clinician needs to find out; not every sprain needs an MRI.
- X-ray: Useful when fracture is a concern and to assess the position of the tibia and fibula. The American Orthopaedic Foot & Ankle Society’s FootCareMD overview describes standard ankle views and imaging of the whole leg when relevant to the assessment.
- MRI: Can show soft-tissue ligament injury. The 2018 guideline reports MRI sensitivity of 93%–96% and specificity of 100% in its referenced diagnostic context; those figures are not a guarantee of performance for every patient or ankle MRI. The guideline says MRI may be considered when a high-grade ligament injury, syndesmotic injury, occult fracture, or persistent symptoms are suspected.
- CT: Can provide a better assessment of bone and fibular position when that is the clinical question, according to FootCareMD.
The choice of views or additional imaging is a clinical decision. A radiograph can help identify fracture or widening, while MRI is useful for soft tissue; one does not replace the other in every situation.

How are the two injuries treated?
Typical lateral ankle sprain
Once the diagnosis is established, a brace or tape combined with exercise is a common approach. The 2018 guideline favors supervised exercise-based rehabilitation over passive treatments alone and says ankle braces may be considered to help prevent recurrence. In some acute cases, a clinician may recommend a short period of immobilization to help manage pain and swelling, followed by progressive exercise.
High ankle sprain without widening or fracture
A stable syndesmotic injury may be treated without surgery, with protection or immobilization followed by progressive rehabilitation. The National Athletic Trainers’ Association position statement lists non-weight-bearing, a boot, cast, or brace as possible immobilization approaches; the choice and duration depend on the injury and functional recovery. A removable walking boot is a clinician-directed option for selected cases, not a universal requirement.

High ankle sprain with widening or fracture
If imaging shows syndesmotic widening (diastasis) or an associated fracture, surgery may be considered. A specialist’s assessment of the injury’s stability and alignment guides that decision; symptoms alone cannot establish whether surgery is needed.
How long does a high ankle sprain take to heal?
There is no single recovery timeline. FootCareMD says return to normal activity may take 6–8 weeks, while full recovery can take a year or longer in some cases. These are broad source-specific estimates, not a prediction for an individual or a date that automatically clears someone for sport. High ankle sprains often take longer than typical lateral sprains, especially when the injury is unstable or treated surgically.
