An AI model incident response plan should define reportable events, provide a reporting and triage path, assign decision authority, explain containment and evidence handling, and specify who receives updates inside and outside the organization. It must cover the deployed system—not only the model—and include recovery, user remediation, monitoring, and plan maintenance.
NIST AI RMF 1.0 provides a useful voluntary structure, but it does not create a universal incident definition, recipient list, or notification deadline. Those details depend on your system, contracts, sector, jurisdictions, and the people affected.
What an AI incident response plan does
An AI incident response plan is the operational playbook for detecting, assessing, containing, communicating, and recovering from harmful or unexpected behavior in an AI-enabled system. The scope should include the model, prompts and input pipelines, retrieval or training data, tools and integrations, human review, user interface, access controls, and downstream decisions. A model can behave as designed while the surrounding application exposes sensitive data or takes an unsafe action.

NIST AI RMF Manage 4.1 calls for post-deployment monitoring that includes feedback, appeal and override, decommissioning, incident response, recovery, and change management. The framework also says that “Risk treatments, including response and recovery, and communication plans for the identified and measured AI risks are documented and monitored regularly.” See the NIST AI RMF Core.

NIST AI RMF 1.0 was released on January 26, 2023. NIST describes the framework as voluntary and says it is being revised; its Playbook offers suggested actions rather than a mandatory checklist. Check the current framework page and the AI RMF Playbook for updates before approving your plan.
Define scope and the incident trigger
List the systems and deployments covered
Start with an inventory that identifies each AI system, model and version, deployment environment, business owner, technical owner, intended use, user groups, geographic reach, connected tools, and populations that could be affected. Record whether the organization operates the system itself, provides it to another deployer, or relies on a third-party provider. A single plan can set common rules, but each system needs its own contacts, rollback method, data considerations, and severity thresholds.
Specify what must be reported
Write trigger categories in plain language and tailor the threshold to the system’s potential impact. Candidate categories include:
Free tools Windows power users keep installed
One-click scans. No signup required.
- Materially incorrect, unsafe, discriminatory, or misleading outputs in a consequential use.
- Unexpected model behavior, prompt-injection success, jailbreaks, data leakage, or unauthorized tool actions.
- Performance degradation, drift, or a failure of monitoring, guardrails, access controls, or human review.
- Security, privacy, availability, or integrity events affecting the model or its surrounding application.
- Adversarial activity, abuse, or a compromise of model, data, credentials, or infrastructure.
- Near misses in which controls prevented harm but reveal a credible path to recurrence.
- Material negative impacts on users, workers, customers, or affected communities, even when no technical fault has been confirmed.
NIST describes post-deployment monitoring as a way to identify degradation, attacks, unexpected behavior, near misses, and impacts. Your plan should say who decides whether an observation crosses the reporting threshold and require staff to report uncertainty rather than wait for proof.
Build intake and triage procedures
Offer more than one reporting route
Provide a monitored route for users and operators, an internal route for engineers and support staff, and a method for relevant third parties to report concerns. Depending on the deployment, this could include an in-product report function, support queue, security disclosure address, privacy channel, and emergency on-call number. Every route should identify the system or service, preserve the original submission, and generate a tracking identifier.

Capture the first report consistently
The intake record should capture the reporter’s contact preference, date and time with time zone, system and model version, deployment or tenant, user role, prompt or input context where lawful, observed output or action, expected behavior, suspected impact, affected people or data, and any immediate containment already taken. Preserve relevant screenshots or artifacts, but minimize collection of personal data and restrict access to those who need it.
Use organization-defined severity levels
NIST does not prescribe universal AI severity labels. Define levels that map to response authority and deadlines. The following structure is illustrative and should be adapted to your risk appetite:
| Level | Typical condition | Initial action and escalation |
|---|---|---|
| Critical | Ongoing or likely severe harm, active compromise, sensitive-data exposure, unsafe automated action, or a failure affecting many people. | Activate the incident lead immediately; invoke emergency containment and executive, legal, privacy, security, and communications review. |
| High | Credible material harm or security, privacy, or reliability impact with contained scope or uncertain extent. | Assign a lead promptly, restrict exposure, preserve evidence, and notify accountable leadership and specialist owners. |
| Moderate | Repeatable harmful behavior, significant degradation, or a near miss requiring corrective work but no confirmed severe impact. | Track to a named owner, investigate, apply a mitigation, and review whether the threshold should be raised. |
| Low | Isolated error, minor quality issue, or suspected weakness without material impact. | Log, assess trends, provide a user remedy where appropriate, and feed the issue into monitoring and improvement. |
For each level, specify the time to acknowledge, who can change the level, the maximum time before leadership review, and the conditions that allow closure. Do not let a lack of certainty delay containment when potential harm is high.
Assign roles and decision authority
A plan is unusable if it names only a generic “response team.” Assign a primary incident lead and deputies, maintain current on-call details, and define authority for technical and organizational decisions. The exact organization chart will vary; this map applies NIST’s responsibility and information-sharing outcomes to a practical operating model.
| Role | Core responsibilities | Decisions or inputs |
|---|---|---|
| Incident lead | Declares the incident, sets objectives, coordinates work, maintains the timeline, and controls status updates. | Severity, escalation, work priorities, closure recommendation. |
| Model or AI engineering owner | Analyzes model, prompts, evaluations, versions, guardrails, and behavior changes. | Rollback, configuration change, retraining or model replacement. |
| Security and operations | Investigates compromise, access, infrastructure, availability, and abuse; applies technical containment. | Credential revocation, network isolation, rate limits, service pause. |
| Product or system owner | Assesses workflow, user impact, dependencies, and business continuity. | Feature disablement, customer handling, alternate process. |
| Privacy and legal | Assesses personal data, privilege, contracts, regulatory duties, and preservation requirements. | Notification obligations, wording, retention and disclosure constraints. |
| Communications and support | Prepares consistent notices, handles questions, and records user reports and remediation requests. | Audience, channel, cadence, approved language. |
| Accountable executive | Resolves risk acceptance and resource conflicts for serious incidents. | Extended shutdown, public position, major customer or partner decisions. |
State explicitly who may pause the system, restrict access, override an automated decision, roll back a release, disable a tool, or decommission a deployment. If emergency authority is delegated, document the conditions and require a retrospective review.

Contain the event and preserve evidence
Choose the least harmful effective containment
Containment should stop or reduce exposure while preserving a path to investigate. Depending on the failure, options include disabling a risky feature or tool, switching to a previously approved model, reducing permissions, adding human approval, rate-limiting requests, isolating a tenant, blocking a prompt or data source, or taking the service offline. Record who authorized the action, its time, expected side effects, and the conditions for reversal.
Preserve a defensible timeline
Secure logs and records before routine retention deletes them. Relevant evidence can include prompts or inputs, outputs, model and configuration hashes or versions, retrieval documents, tool calls and parameters, safety-filter decisions, authentication and access logs, monitoring alerts, user reports, deployment history, and communications. Synchronize timestamps and record whether an artifact is original, redacted, or derived. Collect only what is necessary, apply access controls, and follow privacy, security, contractual, and legal-hold requirements. NIST supports incident tracking and recovery processes but does not prescribe one universal AI evidence schema.
Separate facts, hypotheses, and decisions
Use the incident record to distinguish observed behavior from assumptions about cause. Log every significant decision, the evidence available at that time, alternatives considered, approver, and expected outcome. This prevents later confusion when the model, data, application, or human process may each contribute to the event.
Decide who to notify and what to say
Internal notification
Notify the incident lead and the role owners needed for the severity level: model or system owner, security and operations, privacy or legal, communications and support, and accountable leadership. Keep an escalation matrix with primary and backup contacts, time-zone coverage, secure contact methods, and authority limits. A contact list that is not tested and maintained is not a response capability.
External notification
Identify audiences before an incident occurs. Depending on the event, they may include affected users and communities, a customer or deployer, the model or infrastructure provider, data partners, contractors, regulators, or law enforcement. NIST AI RMF Manage 4.3 states: “Incidents and errors are communicated to relevant AI actors, including affected communities.” Read the outcome in the AI RMF Core.

Do not treat that sentence as a universal recipient list. Determine recipients from the system’s role in the supply chain, contracts, affected locations, sector rules, data involved, and incident facts. Privacy or legal review should establish whether a notice is required, permitted, or restricted, while communications should ensure that affected people receive a usable explanation and a way to seek help or appeal.
Use a staged communication process
- Confirm the minimum facts: what happened, when, which system or version was involved, what is being done now, and what remains unknown.
- Set the audience and channel: choose direct user notice, status page, customer communication, regulator contact, public statement, or another appropriate route.
- Explain protective actions: tell recipients what they should stop doing, change, preserve, or submit for review.
- Provide remediation: include support, appeal, correction, refund or replacement process where relevant, and a contact for follow-up.
- Schedule updates: state when the next update will arrive, even if the investigation is incomplete.
Keep messages accurate under uncertainty. Avoid claiming a root cause or scope that has not been established, and coordinate public wording with contractual and regulatory communications.

Set return-to-service conditions
Define objective gates for resuming service: containment is effective, the vulnerable path is fixed or acceptably mitigated, access and configuration are reviewed, required approvals are complete, affected users have a support route, and monitoring can detect recurrence. For high-severity events, require an independent reviewer or accountable executive to approve restoration.
Offer appeal and correction
If an AI output influenced a person’s access, eligibility, employment, safety, finances, or other consequential outcome, provide a route to human review and correction. Record the cases affected, decisions reversed, and people still awaiting remediation. NIST’s Manage 4.1 outcome explicitly includes feedback, appeal, and override.
Run a root-cause and corrective-action review
Analyze technical, data, process, human-factors, and governance contributors rather than stopping at the first software defect. Assign corrective actions with owners and due dates; examples include evaluation changes, new guardrails, permission reduction, revised training, supplier controls, user-interface changes, or retirement of the deployment. Monitor the restored system for recurrence and document whether residual risk is accepted, transferred, reduced, or requires decommissioning.

Maintain and exercise the plan
Keep the plan available to designated staff through an access-controlled location and a usable emergency copy. Review it after model, data, integration, organizational, contractual, or regulatory changes, and after every material incident or exercise. NIST SP 800-171 Rev. 3 assessment material calls for distributing an incident response plan and updating it to address changes or implementation problems; see the assessment material.
The sources do not establish a universal exercise frequency. Set one appropriate to your risk and change rate, then test contacts, declaration criteria, evidence access, pause and rollback authority, notification approval, and user-remediation paths. Record findings and assign fixes rather than treating an exercise as complete when the meeting ends.
A practical plan template
Adapt the following headings into your controlled document. Fill each field with system-specific information rather than copying generic language.
- System profile: name, purpose, model and version, owner, deployer, providers, data types, users, locations, dependencies, and intended decisions.
- Trigger definition: reportable errors, attacks, degradation, near misses, impacts, and severity thresholds.
- Intake: reporting channels, required fields, tracking identifier, accessibility, and privacy protections.
- Command structure: incident lead, deputies, role owners, on-call contacts, escalation times, and emergency authority.
- Containment: pause, override, rollback, isolation, access reduction, tool disablement, and decommissioning procedures.
- Evidence: logs and artifacts to preserve, retention and legal-hold rules, access controls, and timeline format.
- Communications: internal audiences, affected communities, customers, providers, authorities, approval path, channels, and update cadence.
- Recovery: return-to-service gates, monitoring, appeal and correction, root-cause review, corrective actions, and residual-risk acceptance.
- Maintenance: document owner, review triggers, exercise approach, version history, and distribution list.
Legal and jurisdictional limits
There is no single NIST-defined deadline for notifying people or authorities about an AI incident. Requirements can depend on the organization’s location, the affected person’s location, sector, personal-data type, contractual terms, supplier role, and the incident’s facts. The NIST material is U.S. federal guidance and does not replace legal analysis. Have qualified counsel or the organization’s compliance lead map applicable notification, privacy, safety, cybersecurity, employment, consumer-protection, and contractual rules before an incident occurs.
How to judge a template or existing plan
When reviewing an internal document or third-party template, check whether it covers:
- Both model behavior and the surrounding deployed system.
- Clear, organization-defined severity and escalation thresholds.
- Named authority to pause, override, roll back, restrict, or decommission.
- Reporting and communication routes for users and affected communities.
- Evidence preservation, privacy controls, recovery gates, and post-incident monitoring.
- Alignment with legal, security, privacy, accessibility, customer, and supplier obligations.
- Current contacts, version information, distribution, and a process for learning from exercises and incidents.
A plan that lacks any of these elements may describe principles without giving responders a safe way to act. Use NIST AI RMF as a voluntary baseline, then make the thresholds, authorities, contacts, and notifications specific to each deployment.
