You pasted a file and asked why the totals are wrong. The answer may be fluent and internally consistent—and still miss the key fact because only part of the file reached the model. Before rewriting the prompt or judging the model’s reasoning, trace the input from the original source through reading, indexing, retrieval and conversation context. As Serguey Asael Shinder puts it, “Before you debate the output, prove the input arrived.”
How an answer can be coherent but incomplete
A tool reporting that it processed a file does not, by itself, show that the relevant passage was available to the model. A file reader may extract only part of a document; an index may omit an oversized file; retrieval may return too few passages; or a system may drop older conversation turns to make room for newer ones. These are possible failure modes, not universal behavior: the details depend on the product and its configuration.
The distinction matters. A model can reason consistently from the subset it received and still produce the wrong result for the full source. Rephrasing the question may not help if the missing information never entered the model’s context.

Where to look in the input path
- Source file: Confirm that the original document is the one you intended to provide and that it contains the fact or section at issue.
- File reader: Check the extracted text or parsed content, if the product exposes it. Look for a configured page, character, or other processing limit and for parsing errors.
- Index: If the workflow searches an index, verify that the file was indexed and that the relevant section is present. Large files may be skipped or handled differently by a particular system.
- Retrieval results: Inspect the passages actually fetched for the question, rather than relying only on a summary or the final answer. Retrieval may return a limited set of relevant passages, not the whole document.
- Conversation context: If the information was given earlier in a long chat, check whether that turn is still available to the model. Context is finite, and systems may manage it by trimming or otherwise prioritizing content.
- Model synthesis: Only after checking the preceding stages should you treat the answer as evidence about the model’s reasoning over the intended material.
These stages are not identical across products. OpenAI describes file search as a tool that searches uploaded files and returns relevant information (OpenAI file search documentation). Anthropic’s context documentation describes the finite context available to a model and the need to manage it (Anthropic context-window documentation). Neither general description establishes the exact limits or behavior of every application using those capabilities.

A practical way to verify what arrived
- Pick a known endpoint or identifier. Choose something specific and easy to check in the source, such as the document’s last line, the name of its final function, or a distinctive phrase near the end.
- Ask the system to identify it from the material it received. For example: “What is the last line of the file you can access?” Treat the reply as a check, not proof; compare it with the original yourself.
- Inspect the extracted or fetched content. If the tool displays parsed text or retrieved passages, confirm that the relevant section appears there. A correct-sounding summary is not a substitute for checking the underlying material.
- Compare counts or lengths where available. Check whether the reader reports pages, characters, tokens, or passages, and compare those values with what you expect. A mismatch can point to a delivery or processing issue, though a count alone does not prove which content was omitted.
- Reduce the input to a complete unit. When a broad dump is being truncated or selectively searched, provide one complete function or a smaller, self-contained section and ask the same question against it.
Shinder’s essay gives the practical check this way: “Ask it to quote the last line of what it received, and compare that with the file.” A wrong or missing endpoint is a useful warning that the input path needs investigation; a matching endpoint does not guarantee that every middle section arrived.
What to do when the check fails
- If the extracted text stops early, check the reader’s configured limits or parsing status and retry with a smaller complete section.
- If the source parses but the relevant passage is absent from search results, inspect the retrieval results and try a narrower query or a smaller source unit.
- If a document is missing from search entirely, check indexing status and any file-size or processing restrictions exposed by the system.
- If the information was in an earlier chat turn, restate or reattach the relevant material so it is available in the current context.
These are diagnostic branches, not guarantees that a particular fix applies. The useful evidence is the content visible at each stage and whether it includes the fact the answer needed.

Separate delivery failures from reasoning failures
If the relevant fact is present in the material the model received and the answer still overlooks it, the problem may be reasoning, instruction-following, or ambiguity in the question. If the fact is absent from the received material, improving the prompt alone cannot supply it. A wrong answer does not establish which case occurred; tracing the input path does.
