An AI security reviewer that reports no problems for months may be finding nothing—or failing to detect the problems it was meant to catch. Silence alone is not evidence that software is safe. In one account, an author who relied on separate AI agents to write, test, and review hospital internal tools later received outside feedback identifying roughly eight defects over three days. That is one person’s experience, not a measure of AI review accuracy, but it is a useful warning: verify that your checks can catch known failures.

What happened when the reviewer stayed quiet

FromZeroToShip, who describes themself as a non-developer, wrote in a July 30, 2026 DEV Community article that AI produced most of the code for hospital internal tools. The author had separate agents implement, test, and inspect code for security issues. The security reviewer flagged almost nothing for months, and the author initially read that quiet output as reassuring.

Then outside commenters examined the systems and identified roughly eight defects over three days, according to the author. Reported examples included a check that verified the wrong condition, an exclusion guard that did not verify the actual shipping run, an exception list omitted from pass/fail evaluation, a manually performed drill with an expired result, and a scheduled job that had never been registered. These are the author’s reported examples; they were not independently audited.

The author first considered improving the reviewer prompt, then concluded that the implementer and reviewer shared the same underlying model and framing. In that setup, assigning different roles had not guaranteed an independent second opinion. Outside readers also lacked some of the author’s context and assumptions, which may have helped them see issues the author and agents overlooked. That is an interpretation of one episode, not proof that AI code review is inherently ineffective.

“Agreement inside the room is not evidence,” the author wrote. The practical lesson is narrower and more useful: a clean result means only that the checks which ran did not report a problem they were capable of detecting under those conditions.

Why a clean result is not a safety verdict

A test suite can pass while important behavior remains untested. A scanner can report no findings while missing a flaw outside its scope. A manual drill can appear successful but be stale, and a scheduled job can be absent even if its code looks correct. In each case, the output may be accurate about what was checked while saying little about what was not.

NIST’s Guidelines on Minimum Standards for Developer Verification of Software, published October 6, 2021, recommend using complementary verification techniques and explicitly do not claim to address the totality of software verification. OWASP makes the distinction bluntly in its living Secure Coding with AI Cheat Sheet: “Measure security confidence by adversarial testing results and independent analysis, not by ‘all tests pass.’”

The anecdote does not establish a general failure rate for AI reviewers: it gives no model version, controlled comparison, denominator, or sensitivity measurement. It does show why an uneventful report should prompt a question about detection, not an automatic conclusion about safety.

Prove that each check can detect the failures it claims to catch

A useful way to test a reviewer or other security check is to give it a known failure in a controlled setting and see whether it flags the issue for the right reason. NIST includes historical test cases designed to demonstrate the presence and later absence of bugs among its verification techniques. Applied to a review workflow, that suggests this validation loop:

  1. Choose a specific failure mode. Pick a defect the check is supposed to find, such as a missing authorization condition or a job that was never registered.
  2. Reproduce it safely. Introduce the failure in a test fixture or isolated environment rather than weakening a live system.
  3. Run the check. Confirm that the relevant reviewer, test, scanner, or monitoring step actually executes.
  4. Inspect the result. Verify that it detects the intended problem, not an unrelated warning that happens to turn the result red.
  5. Keep the case. Preserve the test so a future change cannot silently reintroduce the same defect without detection.

If a check does not catch the known failure, treat that as evidence about the check’s limits. Adjust its scope or configuration, add a more suitable test, or stop relying on it for that failure mode. A result is meaningful only in relation to what the check has demonstrated it can detect.

Use checks with different methods and scopes

No single review method covers every risk. NIST recommends a range of verification techniques, and OWASP’s AI secure-coding guidance calls for adversarial testing and independent analysis. A practical review process can combine methods that examine different parts of the system:

  • Threat modeling: identify assets, trust boundaries, likely attackers, and abuse cases before deciding what to test.
  • Automated tests, including negative cases: check expected behavior and deliberately exercise denied, malformed, or boundary inputs.
  • Static code scanning: look for recognizable insecure patterns in source code; interpret findings in context and do not treat a clean scan as proof of safety.
  • Dependency checks: examine third-party components and their known issues, which a review focused only on authored code may not cover.
  • Fuzzing where appropriate: supply varied or malformed inputs to find unexpected behavior in code that can be exercised this way.
  • Human review: ask a qualified reviewer to examine business logic and complex security implementations that automated tools may not understand.

OWASP’s Secure Code Review Cheat Sheet treats human review as part of a broader security approach, not a substitute for testing. Independence also matters: a reviewer who brings different context, methods, or assumptions may catch issues a closely coupled authoring and review process overlooks. Simply giving two agents different job titles does not demonstrate that independence.

Ask what the silence means

When an AI reviewer reports nothing, the useful follow-up is not simply “Can I improve the prompt?” Ask what code and dependencies it examined, what conditions it tested, whether it reran reliably, and whether it has ever been shown to detect known failures. Also ask whether another method or person can inspect the same risk from a different angle.

Months without findings may mean the code is sound in the areas checked. They may also mean those checks have not been challenged, do not cover the relevant behavior, or share assumptions with the process that produced the code. The author’s final question is worth keeping in mind: “when did your reviewer last tell you something you didn’t want to hear?” If the answer is “it doesn’t really find much,” that quiet output is still ambiguous—not a safety measure.
