A reliable agent review pipeline combines deterministic checks, rubric-based evaluation, repeatable regression tests, production monitoring, and human approval for consequential decisions. A completed run is not proof of a correct run: an agent can finish while choosing the wrong tool, missing relevant context, violating an instruction, or producing an unsafe result. Review the evidence in its traces, evaluate behavior against explicit criteria, and block promotion when a critical check fails.
What should an agent quality pipeline evaluate?
Evaluate the workflow, not just whether it returned an answer or completed without an error. A useful review asks whether the agent solved the task, chose and used appropriate tools, followed instructions and policy, handled handoffs correctly, and met the system’s performance requirements.
OpenAI’s agent-evaluation guidance identifies questions such as whether the agent picked the right tool, made a handoff when it should have, and followed instructions. Microsoft’s evaluation overview separates concerns including orchestration, knowledge, tools, safety, performance, and system correctness. Treat these as distinct evaluation dimensions: a better result in one can conceal a regression in another.

Use the run trace as evidence. A trace records the path through model calls, tool steps, guardrails, and handoffs, so reviewers can investigate how the final result was produced. A final answer alone may not reveal a bad tool selection or a failed handoff. Preserve enough input, output, and workflow context to diagnose failures and reproduce them, subject to your organization’s privacy and access controls. The official guidance cited here does not establish a universal retention period or privacy configuration.
How should checks be divided between rules and graders?
Separate properties with crisp expected values from qualities that require interpretation. Keep each check inspectable: specify what evidence it reads, what condition passes, and what a failure means for the maintainer.
| Check type | Good fit | Example evidence and result |
|---|---|---|
| Deterministic assertion | A requirement has a clear, repeatable expected value. | Required metadata is present; the output matches a required schema; a tool is on the allowlist; permissions stay within a defined boundary; or a review artifact exists. Report the failed assertion and the relevant evidence. |
| Rubric-based or AI evaluation | The result depends on context, instruction-following, or design judgment. | Assess whether the answer follows instructions, whether a selected capability is appropriate, or whether a design pattern is present. Return a criterion-level judgment with an explanation that reviewers can inspect. |
| Human review | A decision is consequential, subjective, policy-sensitive, or requires final authorization. | A reviewer approves, corrects, or supplies input before the workflow continues or an action is authorized. |
Microsoft’s documented Agent Review Pipeline combines deterministic configuration checks with AI evaluation of design patterns and instruction compliance. Its report includes scores and failed patterns, and a configurable threshold controls acceptance or rejection. This is an example implementation, not evidence that a single model-graded score proves overall quality. Keep separate criteria visible at the promotion gate rather than relying on an opaque blended score.
How do you build a repeatable regression evaluation?
Start with representative behavior
Build a fixed dataset from common requests, edge cases, and known failures—not only idealized examples. AWS recommends curating representative runs into a stable dataset; OpenAI recommends moving from individual trace inspection to datasets and evaluation runs for repeatable benchmarks and prompt comparisons. Include enough context to test the behavior in question, while applying your own privacy and access controls.

Compare like with like
Run the same cases through the baseline and candidate versions with the same evaluators. This makes a difference in results more useful as evidence about the change under test, rather than a side effect of different inputs or grading criteria. Keep the dataset and evaluator versions identifiable so the comparison can be reproduced.
Track criterion-level results
Report results by dimension, not just as one aggregate. Depending on the system, track task correctness, tool selection and execution, retrieval or grounding quality, safety and boundary adherence, plus relevant latency or cost targets. Define those targets for the use case; the cited documentation does not establish universal pass rates or performance thresholds.
What happens at the promotion gate?
Make the promotion decision explicit: which checks are mandatory, which failures block release, who can override a result, and what evidence is attached to the decision. A threshold can help automate a gate, but set it for the agent’s risk and use case rather than copying a vendor example as a universal quality target. Microsoft’s documentation mentions a default threshold of 60 in customization instructions; that is a product configuration example, not a generally valid target.
Use hard blocks for clear violations such as disallowed output structure or a permission-boundary failure. For rubric-based results, define how uncertain or borderline judgments are handled. A policy-sensitive or consequential action may need a human checkpoint even when automated checks pass.

Google Cloud describes a human-in-the-loop pattern in which the workflow pauses for a person to approve, correct, or provide input. That added interaction brings operational and architectural complexity, so place it where human judgment or authorization changes the decision—not as a vague final sign-off on every run. Limit agent write and approval capabilities through explicit permissions and controlled outputs. GitHub’s agentic workflow documentation describes read-only permissions by default, declared safe outputs for writes, and approval control over merges; its page labels the feature public preview and subject to change.
How should production failures improve the test suite?
Evaluate a sample of live traffic and monitor quality signals for declines after release. AWS documents online evaluation of sampled traffic and describes returning failing traces to the development loop. A production failure should enter triage, where the team decides whether it warrants a regression case, an evaluator change, a policy update, or an incident response.

Do not automatically promote every trace into a test. Some are duplicates, sensitive, or unrepresentative. Review and sanitize examples under your organization’s data-handling requirements before adding them to a durable dataset. Once a representative failure becomes a test, verify that the test reproduces the behavior and that its pass condition captures the intended correction.
Which orchestration pattern fits the review workflow?
The workflow that runs quality checks also needs to be evaluated. Choose a pattern based on how predictable the steps are, whether checks depend on one another, and how much latency, coordination, and maintenance the team can support.
| Pattern | Useful when | Main tradeoff |
|---|---|---|
| Deterministic sequence | Steps are known, stable, and should run in a predictable order. | Less flexible when the task path varies. |
| Parallel checks | Independent evaluations can run at the same time. | Results need coordination and aggregation. |
| Iterative refinement | Open-ended output benefits from multiple improvement passes. | Additional cycles can increase evaluation time and cost. |
| Human checkpoint | A critical action or subjective judgment needs approval. | An external review interaction adds operational complexity. |
| Custom orchestration logic | Branching and fine-grained conditional control are required. | Development, debugging, and maintenance are more demanding. |
Google Cloud’s agentic-system guidance describes sequential and parallel patterns for predictable tasks and custom logic for fine-grained conditional workflows. Compare candidates on determinism, flexibility, latency, evaluation capacity and cost, observability, security boundaries, review burden, and how easily the team can add regression cases.

What does a concrete review pipeline look like?
Microsoft’s cross-system promotion gate
Microsoft’s Agent Review Pipeline documentation describes one implementation using Power Platform Pipelines, Power Automate, GitHub Actions, Dataverse, and Copilot Studio. The documented flow starts a pre-deployment step, dispatches an action, retrieves and parses a Dataverse solution, runs deterministic and AI-driven evaluations, calculates a score, and produces a PDF report. A callback returns the result so the pipeline can approve or reject the stage against a configured threshold.
The documented setup requires a custom pipeline host, the review solution, a GitHub repository, a Microsoft Entra ID app registration, and Copilot Studio credits. Microsoft also notes that its AI review stages consume Copilot Studio messages, so teams planning frequent evaluations need to account for capacity. These are requirements and considerations for that Microsoft-specific design, not prerequisites for every agent review pipeline.
Repository-centered workflow
GitHub documents agent instructions written in Markdown and compiled into a hardened workflow file, with isolated execution, threat detection, role-based restrictions, read-only defaults, and declared safe outputs for writes. Because the documentation labels this feature public preview and subject to change, confirm its availability and current behavior before making it a release control.
How do you know whether a change actually improved behavior?
Evaluate the candidate against the same representative cases, baseline, and graders; inspect changes by criterion; then examine traces for failures that summary scores cannot explain. A promotion is justified when required checks pass and any remaining tradeoffs are acceptable under the system’s risk policy. Keep the evidence attached to the release decision, and feed reviewed production failures into the next regression cycle.
The cited official guidance supports this layered design but does not establish a single benchmark proving that one pipeline pattern is most effective. Treat product integrations, preview labels, and capacity requirements as implementation-specific details that can change; verify them in the relevant current documentation when adopting a vendor workflow.
