For practical guidance, limit added and other free sugars—especially in sweetened drinks and processed foods—rather than treating sugar in whole fruit or plain milk as equivalent. But current general guidance does not establish a specific sugar limit that has been shown to improve gut microbiome health. The best-supported approach is to follow broad free-sugar guidance and choose fibre-rich foods; do not assume that “natural” means unlimited or that one sugar source is proven to harm or heal the gut.
What counts as added, free and naturally occurring sugar?
These terms describe different categories; they are not interchangeable. WHO’s free-sugar definition includes sugars added to foods and drinks by manufacturers, cooks or consumers, plus sugars naturally present in honey, syrups, fruit juice and fruit-juice concentrates. It excludes sugars intrinsic to whole fresh fruits and vegetables and sugars naturally present in milk. This is a classification for dietary guidance, not a claim that sugar molecules from different sources are chemically different. WHO’s 2015 guideline sets out the free-sugar definition.
- Added sugar: Sugar put into a food or drink during production or preparation. In the United States, packaged-food labels report added sugars separately.
- Free sugars: WHO’s broader category: added sugars plus sugars in honey, syrups, fruit juice and juice concentrate.
- Sugars in whole fruit and plain milk: These are not included in WHO’s free-sugar recommendation when intrinsic to the food.
So “natural” does not automatically mean “does not count.” Honey and juice may be described as natural, but their sugars fall within WHO’s free-sugar category.
Is fruit sugar bad for your gut?
WHO’s free-sugar guidance does not treat sugar intrinsic to whole fresh fruit as equivalent to sugar in juice. Whole fruit also fits within the food-pattern guidance to choose fruits, vegetables, whole grains and pulses as primary carbohydrate sources. That distinction is useful for everyday choices, but it does not prove that whole fruit directly improves the microbiome or that fruit sugar is harmless in every individual circumstance.

Fruit juice is different for this guidance: juice, including many unsweetened varieties, contains free sugars. “No added sugar” on a juice does not mean its sugars fall outside WHO’s free-sugar category. Honey and syrups count as free sugars as well.
What sugar limit should you use?
WHO recommends that adults and children get less than 10% of total daily energy from free sugars. It conditionally suggests reducing intake below 5% for additional benefits. These are general dietary recommendations, not limits validated specifically for gut microbiome outcomes. WHO’s sugar guideline focuses particularly on unhealthy weight gain and dental caries.

For a person eating about 2,000 calories per day, WHO’s 2026 healthy-diet fact sheet translates the 10% benchmark to 50 g, or about 12 level teaspoons. That is an illustration for that calorie intake, not a universal gram target. The below-5% suggestion is not a gut-specific prescription. See WHO’s healthy-diet fact sheet.

In the United States, the Dietary Guidelines for Americans 2020–2025 advise people aged 2 and older to keep added sugars below 10% of daily calories, and advise avoiding foods and drinks with added sugars for children younger than 2. This is U.S. guidance using the term “added sugars”; it should not be presented as an identical labeling or dietary definition worldwide. The cited edition covers 2020–2025. How to compare foods and read a U.S. label
For packaged foods in the United States, the Nutrition Facts panel lists added sugars. NIDDK recommends considering serving size alongside grams of added sugars and the % Daily Value; the ingredient list can help identify sweeteners, but does not tell you how much is present per serving. NIDDK’s guide to choosing healthy foods explains how to use the label.
- Check the serving size. The grams and % Daily Value apply to the listed serving, so compare like-sized portions.
- Look at added sugars. Use the grams and % Daily Value to compare products; do not infer the amount from the ingredient list alone.
- Consider the food form. Whole fruit and plain milk are not counted as free sugars in WHO’s guidance, while juice, honey and syrups are.
- Keep the geography in view. U.S. labels show added sugars; WHO’s free-sugar definition also includes certain sugars naturally present in juice, honey and syrups.
Why fibre and the overall food pattern matter
WHO’s carbohydrate guidance considers food sources, fibre content and how digestible carbohydrates release glucose. Its healthy-diet guidance recommends that carbohydrates come primarily from whole grains, vegetables, fruits and pulses. It also recommends at least 400 g of fruits and vegetables daily for people older than 10, and at least 25 g per day of naturally occurring dietary fibre for people older than 10. These are food-pattern recommendations, not evidence that fibre cancels sugar or that any one food repairs the microbiome. See the WHO carbohydrate guideline and its healthy-diet fact sheet.

What is known—and not established—about sugar and gut health
The guidance cited here does not establish a quantitative causal effect of added sugar compared with naturally occurring sugar on gut microbiome health, or a sugar threshold validated for microbiome outcomes. It therefore does not support claims that added sugar “feeds bad bacteria,” that natural sugar “heals the gut,” or that meeting a particular limit will treat gut symptoms. This is an evidence boundary, not proof that no relevant studies exist. WHO’s stated rationale for its free-sugar recommendation centers especially on weight gain and dental caries, while its carbohydrate guidance addresses broader carbohydrate quality and fibre.
Use free-sugar guidance as general dietary advice, not as a diagnosis or treatment plan for digestive symptoms.
