Recovery after Achilles tendon surgery usually moves from protecting the repair to restoring walking, calf strength and balance, then gradually returning to running and sport. There is no single reliable date for everyone: “normal activity,” starting sport and regaining a pre-injury level are different milestones. Follow your surgeon’s and physical therapist’s plan rather than using a general timeline to change weight-bearing, boot settings or exercises.
What recovery involves
The repair needs protection while healing, followed by carefully increased movement and loading. Rehabilitation then works toward a steady walking pattern, stronger calf muscles, balance and, eventually, the demands of running or sport. The exact sequence depends on the repair and clinical findings; Mass General Brigham describes its post-operative interventions as individualized to the patient and repair (Mass General Brigham rehabilitation protocol).
Early protection
After surgery, the ankle is protected in a splint, boot or brace. Crutches may be used to keep weight off the ankle or limit how much weight it bears. MedlinePlus advises following the care team’s instructions about the boot and crutches; the surgeon determines when and how weight-bearing can increase (MedlinePlus, Achilles tendon repair aftercare). Do not adjust heel wedges or start bearing more weight on your own.

As recovery advances, the clinical plan may allow more weight-bearing and ankle movement. A 2022 GAIT Study Group consensus reported an average post-operative non-weight-bearing period of 2.3 weeks and recommended keeping the foot pointed downward (plantarflexed) during the first four weeks, without moving the ankle beyond neutral during that period. These are consensus recommendations for the context considered by the panel, not instructions for every patient (GAIT Study Group consensus, 2022).

Strength and control
Rehabilitation develops calf strength, balance, walking quality and single-leg control. In the same 2022 consensus, concentric two-leg heel raises were recommended after six weeks, while stretching and eccentric exercise were not recommended before 12 weeks. Your own protocol may use different timing; do not begin an exercise or stretch just because a general milestone has passed.
Running and sport
Running and sport demand more than being able to walk comfortably. They require adequate strength, balance and control, and the treating team may advance activity according to function rather than elapsed time alone. Published estimates differ because sources describe different endpoints and populations.

How long does Achilles tendon surgery recovery take?
Use these figures as broad, source-specific estimates—not promises or a personalized schedule. In particular, being able to resume ordinary activities does not mean the tendon is ready for sport or for a return to pre-injury performance.

An older archived American Academy of Orthopaedic Surgeons guideline summary described return to sport within 3 to 6 months after operative treatment as an option for people who participate in sports. It is an older option, not a current universal expectation; other published guidance gives later estimates, and functional readiness matters (AAOS archived guideline summary).

What should be in place before returning to running?
A 2026 modified Delphi consensus study proposed seven criteria for return to running after Achilles tendon repair. The authors note that evidence-based criteria remain limited, so these are expert-consensus proposals rather than a validated clearance checklist (Modified Delphi consensus study, 2026).
- No pain in daily life.
- No pain during or after rehabilitation sessions.
- Walking without a limp.
- Ability to walk on tiptoes.
- Ability to perform ten single-leg heel rises.
- Good single-leg balance.
- Psychological readiness to run.
Ohio State’s 2022 guideline likewise describes progression by functional readiness rather than time alone. Your treating team should decide whether your symptoms, examination and function support the next stage.

When to contact the surgical team
Mass General Brigham advises contacting the referring physician for fever, persistent numbness or tingling, excessive drainage from the incision, uncontrolled pain or other concerning symptoms. Use the contact instructions from your own surgical team if symptoms occur.
