The question bank below is a practical aid—not a universal template. Follow the target repository’s current contribution guide and PR template first.
Start with the repository’s contribution rules
Open-source projects set their own conventions for code style, tests, development setup, and pull-request process. Before changing code, read the repository’s README and contribution guide, then check for a PR template, code of conduct, license information, and security-reporting guidance where relevant. GitHub’s guide to contributing to a project recommends reviewing project-specific expectations before submitting.
Confirm that the project accepts the kind of contribution you intend to make and follow its setup instructions. A reusable question bank should complement—not replace or override—required template fields and project-specific instructions. If the project already provides a reviewer-notes section, use it.
Make the patch understandable before asking for review
Write the PR description so a maintainer can understand the change before reading its implementation. State the problem or requested outcome, why it belongs in this repository, the scope of the patch, and the behavior users or maintainers should expect. Link a relevant issue or discussion when one exists; check it for duplicate work or an active contributor before proceeding.

Keep the patch focused on the smallest change that addresses the problem. In the description, point reviewers to the best place to start and identify files or behaviors that merit particular attention. That gives reviewers a useful route through the change without asking them to infer its boundaries from the diff.
Use this reviewer question bank selectively
Adapt these prompts to the repository’s checklist. Answer straightforward questions in the PR description; ask the maintainer only when a real decision or direction is unresolved. A question should supply enough context to make a useful answer possible, not transfer the author’s investigation or implementation work to the reviewer.

- What user, maintainer, or project problem does this patch address?
- Is the change already requested or discussed in an issue, and is anyone else working on it?
- What is the smallest behavior or code change that solves the problem?
- What observable behavior should change, and what should remain compatible?
- Which relevant edge cases or failure paths did you consider?
- Which project-specific tests or checks did you run? What could not be tested, and why?
- Does the change require documentation, release notes, migration notes, or examples?
- Are there design or scope decisions where maintainer direction could prevent rework?
- What known limitation, follow-up, or out-of-scope issue should the reviewer know about?
- Where should the reviewer start, and which files or behaviors deserve focused attention?
Use questions to surface choices such as compatibility expectations, edge-case behavior, or project fit. If no answer is needed, write the item as a note instead—for example, identify a known limitation or say that documentation is unchanged. Apache Hop’s pull-request review guide offers a useful ordering principle: establish that a contribution is well described and has consensus to proceed before spending effort on detailed code review.
Show what you validated—and what you did not
Report the project-specific tests, checks, or manual validation you actually performed, along with any relevant result. If something could not be tested, say what it was and why. Do not imply that a check passed when it was not run, and use the project’s testing guidance rather than assuming a generic test command is sufficient. GitHub’s contribution guidance covers project-specific setup and contribution expectations; Open Source Guides also recommends making PR context and reviewer notes clear in How to Contribute to Open Source.

Consider documentation, examples, release notes, or migration notes as part of the change’s impact. State whether they need updating, and include the relevant update or explain why one is not needed. Keep this factual: reviewers need to know what changed and what remains unverified, not a claim of completeness unsupported by the checks performed.
Put reviewer notes where maintainers will see them
Place the question bank in the PR description or the repository’s designated reviewer-notes field. Preserve every required template field, and do not bury essential rationale in a comment that reviewers may miss. If the change is still seeking early feedback, open it as a draft or clearly mark it as work in progress. Open Source Guides describes draft or WIP PRs and a labeled “Notes to Reviewers” section as ways to communicate review status and context in its contribution guide.

Once review begins, keep the conversation and follow-up in the existing contribution thread. Avoid force-pushing a submitted PR after review starts, since doing so can make changes harder to follow; GitHub’s contribution guide advises against it. Make clear which feedback you addressed and which points remain open.

There is no single review template that applies to every project. A useful approach fits the repository’s process and helps reviewers move through the right questions in a sensible order.

These are comparison axes, not extra mandatory fields. Apache Hop’s review guide explicitly separates checking a contribution’s description and acceptance from detailed code-quality review. Git’s reviewing guidelines encourage concise review-state descriptions linked to relevant threads and distinguish out-of-scope suggestions. Creative Commons’ pull-request guidelines show another project-specific approach, including requesting a review if none is assigned automatically. Use such examples to understand different practices, not to impose another project’s process on your target repository.
