A good human-in-the-loop (HITL) is a defined working relationship between people and a machine-learning system: people may label data, correct predictions, review recommendations, make decisions, or monitor outcomes. To make that relationship useful, specify what the human is responsible for and authorized to do, give them the context and tools to act, test the combined workflow, and use operational evidence to improve it. A reviewer’s presence alone does not guarantee safe or fair results.
What does human-in-the-loop mean in machine learning?
Human-in-the-loop describes an arrangement in which people take a specified role in some part of an AI system’s lifecycle or decision process. That role might involve preparing training data, correcting model output, reviewing a recommendation before it is used, making the final decision, or watching a deployed system for problems. These arrangements differ in how much authority people have and how directly they participate.
There is no single level of human involvement that fits every application. NIST recognizes configurations ranging from fully manual to fully autonomous and notes that some applications may need human oversight while others may not. The choice should follow the intended use and the consequences of mistakes, not the assumption that adding a reviewer automatically improves outcomes. How do you decide what role the human should have?
Start by describing the actual setting in which the system will be used. Record its purpose, assumptions, requirements, affected people, data sources, and operating conditions. Involve the people who understand the model and its domain, as well as human-factors, governance, testing, and operational expertise. Include affected communities where relevant. NIST’s framework recognizes that AI systems involve different actors across design, deployment, operations, and testing; its AI RMF Core provides related outcomes and actions.

Then choose the human’s role explicitly. A person labeling examples needs different access and authority from an expert reviewing a consequential recommendation or an operator monitoring a live service. NIST’s AI RMF human-AI interaction guidance states: “Human roles and responsibilities in decision making and overseeing AI systems need to be clearly defined and differentiated.”
- Label or correct: People classify examples or amend inaccurate outputs. Define how their work is checked and how corrections reach the appropriate data or evaluation process.
- Review a recommendation: A reviewer examines the model’s output before it is acted on. Specify what evidence and context are visible, what the reviewer can change, and what happens when they disagree.
- Make the decision: The human, rather than the model, has decision authority. Make clear whether the model is advisory and how its recommendation should be considered.
- Monitor the system: Operators observe performance and incidents after release. Give them escalation procedures and a practical route to pause, restrict, or raise concerns according to the organization’s process.
For each role, document who is responsible, who has authority, what they can change, and how a case moves to someone with more expertise or decision authority. Do not describe a reviewer as responsible for an outcome if they lack the authority or information needed to influence it.

How should you choose between manual, reviewed, and autonomous workflows?
Compare configurations against the setting rather than applying a universal rule. The axes below are practical questions for design, not a NIST scoring model.

|---|---|
| What is the consequence of an error, and can it be reversed? | Higher-impact or difficult-to-reverse outcomes may call for stronger review, escalation, or a manual decision path. |
| What is the human authorized and able to do? | Review is not meaningful if people cannot correct, reject, delay, or escalate an output under the workflow. |
| What context and time does the reviewer have? | A person needs enough relevant information and time to assess the case; a nominal approval step may not provide effective oversight. |
| What expertise and training are required? | Match the task to the reviewer’s proficiency and provide procedures suited to the work. |
| How does the process behave under workload and edge cases? | Check what happens when queues grow, information is missing, or a case falls outside ordinary conditions. |
| What evidence will be monitored after release? | Decide in advance which errors, overrides, incidents, and feedback will trigger investigation or reassessment. |
A human-reviewed workflow and a human-on-the-loop workflow are not interchangeable: in the former, a person typically reviews an output before it is used; in the latter, a person monitors a system that may act without checking each case. Define the specific arrangement in operational terms rather than relying on the label alone.

What should a human reviewer be able to see and change?
Give the reviewer an intervention path that works in the real interface and process. They should be able to inspect the model output in context, correct or reject it where appropriate, and route a consequential or uncertain case for further consideration under the organization’s process. Specify who receives an escalation and what happens while it is pending.
Review also needs a route for affected people to challenge an outcome and seek redress where the system’s use calls for it. NIST’s human-centred design best-practice document discusses embedding human interaction to label or correct inaccuracies and calls for remediation processes that allow affected people to challenge and obtain redress for outcomes: NIST Human Factors guidance.

Set expectations for operators and practitioners: what they need to know about system capabilities and limitations, what proficiency the work requires, and which procedures they should follow. The AI RMF Core calls for defining, assessing, and documenting operator and practitioner proficiency and human-oversight processes. A review screen or policy that offers no meaningful option to act is not a substitute for that work.

Evaluate the human-AI workflow, not just the model in isolation, when people’s decisions materially affect the result. Document the test sets, measures, and tools; assess performance in conditions resembling deployment; and include representative human evaluation where applicable. Consider the full path from model output through review, correction or escalation, to the resulting action. NIST’s AI RMF Core covers measurement and evaluation, while its AI RMF Playbook suggests actions for achieving framework outcomes.

Decide locally what evidence would indicate that the process needs attention. For example, teams can examine whether reviewers can identify and correct relevant errors, whether escalations receive appropriate follow-up, and whether the workflow performs as intended under realistic workload and edge cases. NIST does not establish a universal confidence threshold or effect size for HITL effectiveness, so set measures for the system’s use and test them rather than borrowing an unsupported cutoff.
How do you monitor and improve the loop after release?
Make oversight an ongoing operational process. Set up channels for feedback and appeals, record incidents and errors, monitor production behavior, and schedule periodic reassessment. Track the frequency and rationale for human overrides where useful: NIST identifies both as information that can inform analysis. Review this evidence alongside workflow performance and relevant changes in use or operating conditions.
Use monitoring to decide whether the process needs investigation, adjustment, or escalation. Ensure the people who collect evidence can route it to those responsible for system evaluation and governance. NIST’s AI RMF organizes risk-management work into Govern, Map, Measure, and Manage; its Playbook offers suggested actions based on AI RMF 1.0. NIST says the Playbook will be updated after the framework itself is revised, so consult the official materials for the current version. The framework is voluntary guidance, not proof that a particular workflow is legally required everywhere: NIST AI Risk Management Framework.
