Your LLM API can keep returning successful responses while its answers stop meeting your product’s requirements. Availability checks catch outages; application-specific evaluations catch behavior changes. To investigate a shift, keep representative test cases, run them repeatedly, preserve traces, and compare results alongside the versions of your prompts, graders, and test data.
Why a successful API response can still be a failure
An HTTP success tells you that a request completed. It does not tell you whether the model followed your instructions, used a tool correctly, returned valid structured output, or gave an answer your application can safely use. Those are product-level behaviors, and they need product-level checks.
Model responses can also vary from run to run. OpenAI’s model optimization guidance says output is non-deterministic and behavior can change between model snapshots and families. That makes a single successful call—or a single benchmark result—weak evidence that behavior remains stable.
What published evidence says about behavior changes
A 2023 study by Lingjiao Chen, Matei Zaharia, and James Zou compared March and June versions of GPT-3.5 and GPT-4 across seven task areas: math, sensitive or dangerous questions, opinion surveys, multi-hop knowledge-intensive questions, code generation, US Medical License tests, and visual reasoning. On its prime-versus-composite task, GPT-4’s reported accuracy fell from 84% for the March version to 51% for the June version. Those figures apply to the paper’s tested versions, prompts, and task; they are not a reliability rate for current models or a claim about every vendor.

The same study found changes in different directions depending on the task: GPT-4 became less willing to answer sensitive questions and opinion surveys, while doing better on multi-hop questions. Both tested models made more code-formatting mistakes in June. A change can be an improvement in one dimension and a regression in another. The authors concluded that the behavior of the “same” LLM service can change substantially over a relatively short time, underscoring the value of continuous monitoring. Read the study.
Build an evaluation around your application
Do not start with a generic leaderboard. Start with the behaviors your product depends on, then build a repeatable check for each one. OpenAI recommends representative test data and iterative evaluation; Anthropic describes an eval as an input paired with grading logic and notes that repeated trials matter because outputs vary. Its guidance also emphasizes evaluating an agent together with its harness—the surrounding instructions, tools, and workflow.

1. Choose representative cases
Collect examples that reflect real inputs and meaningful risks: ordinary requests, ambiguous wording, edge cases, and cases where a wrong answer has a high cost. Include examples of required tool use, refusal behavior, or structured output if those are part of the application contract. Add new cases when production failures reveal gaps.
2. Define pass criteria before running tests
Specify what counts as success for each case. Depending on the task, criteria might include factual correctness judged against a known answer, a valid JSON schema, the required tool call, a safe refusal, or an acceptable final outcome. Keep the criterion close to the behavior being tested; one aggregate score can hide a serious regression in a small but important category.

3. Run multiple trials
Repeat each case enough times to see whether results are stable. Record both the pass rate and the kinds of failures, rather than treating one run as definitive. There is no universal number of trials or alert threshold established by the cited guidance; choose them according to the risk and variability your application can tolerate.
4. Preserve versions and compare like with like
Keep the model identifier and settings, prompt version, test dataset version, and grader version with each run. When comparing a new result with a baseline, use the same cases and grading logic where possible. If any of those inputs changed, mark the comparison accordingly rather than attributing the difference automatically to the model.

Compare task-level outcomes and error categories. If you operate more than one provider or model version, use the same application-specific cases and criteria for each. Track tool-use and format compliance, repeat-run variability, and latency or cost when they matter to your product. The cited sources do not establish a current provider ranking or a universally best model.

A lower pass rate is a signal to investigate, not proof that the model changed. The cause could be model behavior, ordinary output variation, a prompt or grader edit, or a failure in a tool or other workflow component.

OpenAI describes traces as end-to-end records of model calls, tool calls, guardrails, and handoffs, and recommends trace graders for identifying workflow-level regressions. Its agent documentation also describes using datasets and evaluation runs for repeatable comparisons. For each run, retain the input, the response, relevant model identifier and settings, tool activity, and the versions of the prompt, dataset, and grader. Comparing those records helps distinguish a changed answer from a changed workflow or test.
Make monitoring a repeatable operating loop
- Establish a baseline. Run the representative dataset and retain its outcomes, error categories, and traces.
- Rerun after relevant changes. Evaluate when you change a model selection, prompt, tool, or other part of the workflow, and periodically according to your product’s risk.
- Investigate before blaming a vendor. Check whether the prompt, grader, data, settings, or tools changed; then inspect traces and repeated outcomes.
- Update the dataset. Turn confirmed failures and newly discovered edge cases into test cases, and version the dataset and prompts. OpenAI’s dataset guidance recommends adding edge cases over time and versioning prompts.
- Set application-specific alerts. Choose thresholds based on the impact of a failure in your product. The cited documentation does not prescribe one universal threshold or monitoring cadence.
OpenAI’s dataset guide currently states that its Evals platform is scheduled to become read-only on October 31, 2026, and shut down on November 30, 2026. Those dates are subject to change; check the current guide before relying on that platform for an ongoing evaluation process.
