Prioritize zero-day work by combining evidence of exploitation with the affected system’s exposure and importance—not by sorting vulnerability scores alone. Confirm that your software and versions are affected, identify where the vulnerable service is reachable, then choose a safe fix or mitigation. Verify the change and keep monitoring: a patch closes a vulnerability, but it does not prove that the system was not already compromised.

What should determine the patch order?

Use the following factors together. They are a decision framework, not a universal score formula; a lower technical severity can still warrant faster action if an exposed system supports an essential service.

  • Exploitation evidence: Put confirmed active exploitation near the top. Consider a Known Exploited Vulnerabilities (KEV) catalog listing, credible vendor or government advisories, and exploit activity in your own telemetry. Record what the evidence is and when it was observed. A missing KEV entry is not proof that a vulnerability is not being exploited; NIST notes that KEV lists may not be comprehensive.
  • Exposure: Establish whether the affected system is reachable from the public internet, reachable only through internal networks, or not reachable in its deployed configuration. Check whether the vulnerable service or feature is enabled. Publicly exposed outdated software, misconfiguration, and default credentials can increase risk, as CISA explains in its Internet Exposure Reduction Guidance.
  • Technical impact: Check the advisory for what successful exploitation could allow, whether authentication is required, and whether the vulnerable feature is active. These details vary by vulnerability; do not infer them from the “zero-day” label.
  • Asset consequence: Consider whether the system supports safety, essential operations, identity, sensitive data, revenue, or important downstream dependencies. CISA’s Cross-Sector Cybersecurity Performance Goals checklist recommends prioritizing more critical assets when handling known exploited vulnerabilities.
  • Remediation and mitigation: Weigh patch availability, testing needs, operational windows, rollback options, and whether a workaround actually blocks the attack path. Note who owns the decision and when it will be reviewed.

Use vulnerability measures as inputs, not as the final decision. CVSS describes technical severity; EPSS estimates exploitation likelihood; KEV identifies vulnerabilities known to be exploited. NIST’s May 2025 paper proposes Likely Exploited Vulnerabilities (LEV) as a possible complement, while noting limitations in EPSS and KEV coverage and the need for industry performance measurements. LEV should not be treated as an established replacement or as proof of better prediction.

How to triage a zero-day advisory

  1. Confirm the advisory. Match the CVE or vendor advisory to the affected products and versions. Check current exploitation evidence, available patches, and any vendor-approved workaround. “Zero-day” alone does not tell you which versions are affected or whether exploitation is active; those facts can change quickly.
  2. Find affected assets. Compare the advisory with your software inventory and vulnerability scans. Identify internet-facing systems, enabled vulnerable services, high-value internal assets, and dependencies. If you cannot tell where the software is deployed, improve visibility before relying on a patch order.
  3. Elevate credible exploitation. Prioritize active exploitation, a KEV listing, credible vendor or government reporting, or signs in your own telemetry. Proof-of-concept availability and exploit automation can inform the decision, but do not treat the absence of catalog evidence as proof of safety.
  4. Adjust for exposure and consequence. Public reachability and critical business or mission functions are practical reasons to move an item up. A lower-scoring flaw on an exposed essential service may deserve action before a higher-scoring flaw on an isolated, low-impact asset; this depends on your environment, not a universal rule.
  5. Select a safe remedy. Prefer the supported vendor patch when it is available and can be deployed safely. Otherwise, consider a vendor-approved mitigation, restricting reachability, disabling the vulnerable function, or isolating the system. For operational technology (OT) and safety-critical systems, coordinate changes with operations and safety owners.
  6. Verify, monitor, and reassess. Validate the fix or mitigation on every affected asset, check for signs of compromise, and reassess as vendor and threat information changes. Keep the decision, owner, residual risk, and next review point in your change records.

How to compare competing vulnerabilities

For each finding, record the same fields so the team can explain why it is being fixed now or deferred:

Decision factor What to record
Exploitation evidence Confirmed exploitation, credible reporting, proof-of-concept availability, or no known evidence; include the source and observation date.
Exposure Publicly reachable, reachable only through internal segmentation, or not reachable in the deployed configuration.
Technical impact Likely attacker access and control, authentication requirements, and whether the vulnerable feature is enabled; verify against the specific advisory.
Asset consequence Effects on safety, mission or business continuity, identity, sensitive data, revenue, and dependent services.
Remediation feasibility Patch or workaround availability, testing and change requirements, deployment window, and rollback plan.
Mitigation strength Whether the measure meaningfully blocks the attack path and can be monitored.

Document why an item is elevated or deferred, who accepts any residual risk, and when the decision will be revisited. The goal is a defensible order based on evidence and local impact, not false precision from a single number.

What to do when patching must wait

Do not leave a known exposure unchanged simply because a full patch cannot be deployed immediately. Choose controls that fit the system and can be maintained:

  • Apply the vendor’s temporary mitigation if one is available.
  • Remove public reachability, restrict access to trusted systems, or disable the vulnerable service when operationally safe.
  • Increase monitoring and look for indicators of compromise. Installing a patch later does not establish that exploitation did not occur beforehand.
  • Assign an accountable owner, document residual risk, and set a specific next review point.
  • For OT or other safety-critical environments, coordinate with responsible operators and use compensating controls if patching could compromise availability or safety.

CISA’s guidance calls for risk-informed handling of known exploited vulnerabilities on internet-facing systems and prioritization of more critical assets; it also recognizes compensating controls where patching could put OT availability or safety at risk. This is guidance, not a universal deadline for every organization.

Verify the change and keep it in force

Patch management is a lifecycle, not just installation. NIST defines enterprise patch management as “the process of identifying, prioritizing, acquiring, installing, and verifying the installation of patches, updates, and upgrades throughout an organization.” Its SP 800-40 Rev. 4 was published April 6, 2022, and treats patching as preventive maintenance that can help prevent compromises, breaches, disruptions, and other adverse events.

After a deployment, confirm coverage across affected assets through deployment status, scanning, or another suitable validation method. For mitigations, confirm that the control remains active after subsequent changes. NIST’s EO-critical software security measures call for rapidly identifying, documenting, and mitigating known vulnerabilities, and for monitoring platforms so mitigations are not removed outside change control.

Make the next decision easier

Maintain an accurate inventory, clear ownership, and a repeatable review process before the next urgent advisory arrives. NIST recommends an organizational patch-management strategy that simplifies patching while reducing risk. For ransomware-focused context, CISA’s StopRansomware guide recommends timely patching of internet-facing servers, especially for known exploited vulnerabilities, and regular scanning with attention to internet-facing devices.
