If you need customers to select paid options that become part of an order, use a product add-on form instead; that is a different workflow.
Choose the right kind of form
Decide what should happen to the information before choosing a tool. A product enquiry form invites a customer to ask a question or request a quote. A product configuration form collects selections—such as text or checkboxes—that accompany a purchasable product.

WooCommerce’s current Product Enquiry Form documentation describes the purpose-built enquiry extension and its settings. Do not confuse it with the separately documented older Product Enquiry Form: WooCommerce says that legacy product is no longer available on WooCommerce.com and its page will no longer be updated. That older page describes a product-page tab that emailed submitted details and recommends Jetpack’s Form Block as an alternative for adding a form to a page. Its installation instructions are not instructions for the current extension. See the current extension documentation and the legacy documentation.
Install and enable the current extension
- Download the Product Enquiry Form ZIP from your WooCommerce account.
- In WordPress Admin, open Plugins > Add New, upload the ZIP, and install and activate it.
- Go to WooCommerce > Settings > Product Enquiry Form > General Settings.
- Enable the extension and select the user roles and products that should see the enquiry button.
These steps follow WooCommerce’s current extension documentation. Check the account and extension availability for your store before starting; compatibility with a specific theme, block setup, plugin combination, or WooCommerce version is not established here.

Choose where the enquiry button appears
Target products
Use the extension’s documented inclusion and exclusion rules to target products, categories, or shipping classes. Additional filters include price range, stock threshold, product types, tags, and attributes. Select the rule that matches the products customers should ask about, then preview the affected pages.
Set placement and purchase behavior
The extension documents button placement inline with or below Add to Cart, as well as product, shop, and cart locations. It can also hide the price or Add to Cart button. Hiding either makes sense only when customers must enquire before buying; otherwise, leave the normal purchase path available.

WooCommerce’s Product Enquiry Form documentation describes these controls, and its extension page describes replacing price or Add to Cart with an enquiry button for quote-led selling.
Configure fields and submission behavior
Build a focused form
Documented field types include text, email, number, textarea, dropdown, checkbox, radio, and file upload. For each field, set its name and label, choose whether it is required, activate or deactivate it, and supply options where relevant. Ask only for information needed to answer the question or prepare the quote.

Set spam protection and the next step
The extension documents reCAPTCHA v2 and v3 settings with site-key and secret-key fields. It also supports a redirect URL after submission. Confirm the notification destination and what customers see after submitting in your own configuration; settings alone do not establish that a particular store’s email or messaging workflow is correctly routed.

Add the button with a shortcode when needed
The documentation lists these shortcodes for placing enquiry buttons:
[product_enquiry_button]for a single product.[cart_enquiry_button]for cart enquiries.[enquiry_button ids="1,2,3"]for specified product IDs. Replace the example IDs with the actual IDs of your products.
Use shortcodes when you need to place a button in a specific supported location rather than relying only on the extension’s placement settings. Refer to the WooCommerce documentation for the extension’s shortcode details.

Review the customer journey before launch
- Check that the button appears only on the intended products and locations.
- Make sure its label clearly signals a question or quote request.
- Try the form on the page customers will use, including any relevant mobile layout.
- Submit a test enquiry and verify it reaches the expected store workflow and that any redirect behaves as intended.
WooCommerce’s documentation describes configuration options, but it does not establish how those options behave with every store’s theme and plugin combination. A test in the actual store is therefore necessary before relying on the enquiry path.
