Some are prescription medical systems for people with paralyzed or weakened lower limbs; others are workplace equipment designed for specific tasks such as sustained overhead work or handling loads. They are not interchangeable, and neither a physically demanding job nor a diagnosis alone establishes that a person needs one.
What a wearable exoskeleton does
An exoskeleton provides external support or assistance for a defined movement. Depending on its design, it may support a body region, help with repeated movements, or offer a resting position during prolonged standing. “Exoskeleton” is an umbrella term: devices vary in whether they are powered or passive, which body parts they support, and what they are meant to do.
That distinction matters. FDA’s classification covers a specific medical category: a prescription, motorized orthosis worn over paralyzed or weakened lower-extremity limbs for medical purposes. It is not a definition for every workplace device. FDA: Product Classification—Powered Exoskeleton
Who may use one?
People with paralyzed or weakened lower limbs
A clinician may consider a prescription powered medical exoskeleton for a particular person and device. The FDA category describes what the device is; it does not establish that a specific person is eligible, that a device will enable independent walking, or what result to expect. Individual candidacy depends on the device and clinical assessment.

People receiving rehabilitation
Exoskeletons are part of medical and rehabilitation technology, but the sources cited here do not establish model-specific indications or outcomes. A rehabilitation team can advise whether a particular system is appropriate for a person’s goals and circumstances.
Workers whose tasks match the support
Occupational devices may be considered for defined demands such as sustained overhead work or particular handling and movement tasks. NIOSH describes shoulder-support devices for overhead work and leg- or back-assist systems for specified tasks. Support for one movement does not mean a device is suitable for a whole job, so assess the actual task, user, equipment, and environment. NIOSH: Industrial Exoskeletons

Healthcare staff handling patients
NIOSH discusses wearable robots as a possible tool for reducing musculoskeletal-disorder risk during patient handling. That potential does not establish universal effectiveness, and an exoskeleton should not be treated as a replacement for safe patient-handling systems. How to decide whether one fits a task
For workplace use, start with the work rather than the product label. A device intended to support a shoulder during overhead work may not suit lifting, walking, or a task requiring frequent changes in posture. A safety assessment should consider:

- Target movement and body region: Identify what the device is intended to support and whether that matches the repeated or sustained movement.
- User fit and movement: Consider fit, range of motion, comfort, and the worker’s familiarity with the device.
- Task and environment: Check compatibility with tools, protective equipment, work sequence, surfaces, and surrounding hazards.
- Load transfer and residual risk: Determine whether the device reduces a load or shifts it to another body area, and whether it introduces new hazards.
- Implementation: Include training and changes to the task or team organization in the assessment.
ASTM’s F3527-24 guide addresses risk assessment for implementing exoskeletons in task-specific environments, including changes involving the user, task, familiarity, and team organization. It supports evaluating the device as part of the work system, not as a stand-alone fix. What the evidence does—and does not—show
NIOSH reports that some devices can reduce physical loading in some work tasks, but many studies are small and conducted in laboratory settings. GAO likewise found limited public evidence demonstrating that workplace wearables reduce injuries; many field studies have been short. GAO also notes that wearables may provide some benefit to workers experiencing musculoskeletal pain or discomfort. These findings support cautious expectations about task-specific assistance, not a general claim that exoskeletons prevent workplace injuries. NIOSH’s occupational-health-equity bulletin summarizes study-specific findings of 10–44% lower back-muscle activity during handling tasks, as well as one study reporting 24% lower hip-extensor muscle activity and 50% lower neck-muscle activity. These are measurements of muscle activity in particular studies—not percentages of injuries prevented. NIOSH cautions that results depend on posture, task, and proper fit. GAO reported more than 700,000 nonfatal injuries and more than 2,000 fatal accidents across warehousing, manufacturing, and construction during 2022 in its March 2024 workplace technology spotlight. Those are industry-wide figures, not injuries attributed to exoskeletons. In its later assessment, GAO reported at least $17.7 billion in employer costs from musculoskeletal injuries in 2021; that is contextual cost data, not an estimate of savings from adopting exoskeletons. Risks and limitations to consider
A device can change how a task loads the body rather than simply removing the load. NIOSH describes examples in which load shifted from the shoulders to the lower back and legs, and in which a heavy tool used with a vest-mounted stabilizing arm increased spinal loading. It also notes that a powered exoskeleton moving beyond a user’s normal joint range may cause muscle strain.
Battery-related hazards can include skin irritation or chemical burns from leakage and thermal burns from sudden discharge. These risks make device condition, fit, task matching, and the work environment part of a safety assessment. An exoskeleton is not a substitute for redesigning a hazardous process or for a professional safety review. NIOSH: Industrial Exoskeletons
When an exoskeleton is not the answer
The evidence cited here does not establish that everyone with back pain, general weakness, age-related mobility changes, or a physically demanding job needs an exoskeleton. In a workplace, the device should be considered alongside task and process changes, not used to excuse an avoidable hazard. For medical use, ask a clinician about a specific device rather than inferring suitability from the broad term “exoskeleton.”
