Do not assume the prompt is the cause: model settings, retrieved context, tools, output handling, and runtime boundaries can all shape the result.
Start by defining the failure
Translate a report such as “the AI gave me a weird answer” into an observable behavior and an expected result. Record whether the incident involved a wrong answer, unsupported claim, missed instruction, incorrect tool choice, unexpected refusal, malformed output, latency or cost change, or unsafe action. State what the feature should have done in terms specific enough to check.
Preserve a representative run before changing anything. Capture the user input, prompt revision, model and configuration, relevant retrieved context, tool calls and their arguments and results, intermediate outputs, final answer, and available user feedback. OpenAI describes a trace as an end-to-end record of model calls, tool calls, guardrails, and handoffs: OpenAI trace grading documentation.
Inspect the complete execution trace
Follow the run from input to answer
A copied prompt and final response can hide the cause. Inspect the sequence of model calls, context supplied at each step, routing decisions, tool activity, guardrails, and intermediate messages. In a multi-turn feature, inspect the conversation thread as well as the individual run; an earlier turn may have introduced context that changed a later answer. Compare the whole execution bundle
Compare the failing run with a known-good run or the expected contract. Note the prompt version and runtime configuration, then check model inputs and outputs, retrieved material, tool arguments and results, handoffs, and final formatting. Find the earliest divergence
Ask which step first departed from the expected behavior. Treat each explanation as a hypothesis to test against the trace:

- If the model received stale, irrelevant, or incomplete context, investigate retrieval and data handling.
- If a tool returned an unexpected result or violated its schema, check the tool contract and error handling.
- If routing selected the wrong path, inspect the routing conditions and the information available at that decision.
- If the inputs and tool results were appropriate but the model missed an instruction, examine whether the prompt is ambiguous, incomplete, or in conflict with other instructions.
- If the response was correct internally but malformed or mishandled afterward, inspect output parsing, validation, and downstream application code.
Check whether the failure is reproducible
Re-run the preserved case under controlled conditions and record whether it produces the same behavior. Keep the model and configuration with the case so that a prompt edit is not credited or blamed for an unrelated change in runtime conditions. If results vary, retain the different traces and compare them; the variation itself may be relevant to diagnosing the issue.
Turn the incident into a regression evaluation
Make the expected behavior testable
Once the team has specified what “good” means for the case, preserve the input and relevant setup as an evaluation fixture. Compare the proposed fix against the previous baseline, and check neighboring behaviors so that resolving one incident does not silently break another. Expand from one trace to a dataset
A single production trace is useful for reproducing one failure. After defining the expected behavior, add the case to a dataset and run repeatable evaluations to compare changes across multiple examples. OpenAI describes moving from individual traces to datasets and evaluation runs as a way to make prompt comparisons repeatable: OpenAI trace grading documentation.
Treat prompts as versioned application code
Keep prompt content in named, version-controlled modules. Validate dynamic inputs, review behavior changes, and retain a way to compare or roll back releases. OpenAI’s prompting documentation says, “Treat prompts as application code,” and recommends code-managed, versioned prompt helpers with direct messages through the Responses API for new work: OpenAI prompting guide.

That page also says reusable prompt objects are being deprecated: creation is scheduled to be de-emphasized beginning June 3, 2026, and the v1/prompts endpoint is scheduled to shut down on November 30, 2026. Existing users are directed to a migration guide. These are announced dates and may change, so check the current documentation before planning a migration.
Use a narrow change, compare it with the prior baseline, and ship through a reviewable release path. OpenAI’s guidance identifies Git history, pull-request review, release tags, and feature flags as ways to review, ship, compare, and roll back prompt changes: OpenAI prompting guide.

Do not use prompt wording as a security boundary
A prompt that says a resource is unavailable cannot enforce that restriction if the deployed environment or a tool still provides access. Anthropic’s September 2026 incident assessment describes cyber-evaluation prompts that said there was no internet access even though the environment left access open; it also notes missing scope constraints: Anthropic incident assessment. That account concerns those evaluations, but the production lesson is broader: inspect actual tool permissions, network access, and scope controls alongside prompt text. Enforce permissions in the environment rather than relying on an instruction to the model.
Choose observability tools around the failure you need to diagnose
Provider-native tracing and evaluation, framework instrumentation, and exporting standardized telemetry to an existing backend are all viable approaches. Compare them by the capabilities your team needs:
- Execution visibility: Can you inspect model calls, tools, supplied context, intermediate outputs, and multi-turn history?
- Evaluation workflow: Can a production failure become a dataset item that is scored repeatedly against proposed changes?
- Interoperability: Does the tracing path work with the telemetry and observability systems you already use? LangChain describes OpenTelemetry as vendor-neutral and interoperable across tools: LangChain OpenTelemetry guide.
- Performance and operations: LangChain says its end-to-end OpenTelemetry path has slightly higher overhead than its native tracing format and recommends native tracing when using only LangSmith. This is product-specific vendor guidance, not a universal benchmark: LangChain OpenTelemetry guide.
- Data governance: Decide what inputs and outputs to retain, who may access them, and whether sensitive content requires filtering or restricted capture. Requirements depend on your data and organization; the cited product guidance does not establish one retention or privacy policy for every team.
Monitoring known signals such as latency and errors can show that a service is healthy while its answers are behaviorally wrong. Traces help explain what happened in a run; repeatable evaluations help determine whether a change improves the behavior you care about. As context rather than a benchmark, LangChain’s 2026 State of Agent Engineering survey reports that 89% of teams had agent observability instrumented, 52% ran offline evaluations, and 37% ran online evaluations.
