Access governance works when it is recurring operational work—not a one-time deployment or an annual audit. Give each review and approval a named owner, connect identity changes to access updates, and make sure decisions and exceptions lead to follow-through.

Turn governance into an operating routine

Access governance answers two practical questions: which identities should have access to which resources, and what are those identities doing with that access? Microsoft’s Entra governance guidance uses those questions to frame governance. The operational challenge is keeping the answers current as people, applications, and business needs change.

That work crosses teams. Security architecture may define policy, IAM operations may manage platform tasks, application owners may understand resource needs, and business owners may judge whether access is still justified. Assign responsibility for each recurring task rather than treating governance as an undifferentiated security-team duty. Microsoft’s operations reference, last updated September 8, 2024, suggests roles such as these; organizations should adapt them to their own structure.

These are suggested divisions of work, not a universal organization chart. For each item, name an accountable owner and the teams that must contribute; make exceptions and approval decisions part of the same ownership model.

Connect identity changes to access changes

Joiner, mover, and leaver events should flow into provisioning and deprovisioning. When a person joins, changes roles, or leaves, the resulting access should reflect that change in a timely way. Attribute-driven lifecycle automation can add, change, or remove access as organizational status changes, as described in Microsoft’s governance overview. Automation depends on reliable identity data and defined rules: a stale or incorrect attribute can produce an inappropriate assignment or removal.

Set out which lifecycle signals trigger updates, which systems supply those signals, and who investigates failures. Include both routine changes and cases that need human judgment, such as a role change that affects access to a sensitive resource.

Make access reviews produce decisions and action

A review is useful only if it reaches the right reviewer, gives them enough context to decide, and turns the decision into action. Plan the workflow before selecting a calendar interval; Microsoft’s deployment guidance treats cadence and response handling as choices to make for the resources and people in scope. The cited materials do not establish one industry-wide review frequency.

  1. Define scope. Specify the resources and identities covered, including applications, groups, access packages, directory roles, and cloud-resource roles where applicable.
  2. Choose reviewers. A reviewer may be a resource owner, a selected delegate, the user, or a manager reviewing direct reports. Designate fallback reviewers when a workflow relies on resource owners or managers.
  3. Set the schedule and response window. Choose a cadence appropriate to the access and its risk, and state how long reviewers have to respond. Send clear notices and reminders as part of the process.
  4. Define outcomes. Decide what happens when access is approved, denied, changed, or left unanswered. Specify which outcomes can be automated and which require manual follow-up.
  5. Close the loop. Remove or adjust access when it is denied or no longer justified. Route missed decisions and exceptions to an owner for resolution, rather than letting silence preserve access by default.

Retain decisions and follow-up records according to your organization’s control requirements. Microsoft’s guidance supports planning review actions and handling, but it does not prescribe a universal evidence-retention period or escalation process.

Cover more than employee accounts

Reviews should reflect the full access surface, not just employees’ direct application accounts. Depending on the environment, scope may include integrated applications, synchronized or cloud groups, access packages, directory roles, and cloud-resource roles. Microsoft’s deployment guidance also calls out scripted access through service principals.

  • External identities: Confirm that guests still need the resources they can reach and that their access has an accountable sponsor or owner. Where access is tied to a fixed contract, consider an expiry. Remove an external identity when access is denied or the application it was using is retired.
  • Applications and groups: Check that assignments remain aligned with current business need, including access inherited through group membership or a package.
  • Privileged roles: Review elevated assignments explicitly; their operational treatment should not be buried in a broad, routine user-access review.
  • Programmatic access: Include service principals and other scripted access in governance so that nonhuman identities are not overlooked simply because they do not have a person as their user.

Apply stronger controls to privileged access

Microsoft’s Entra operational and secure-deployment guidance recommends least privilege, regular review of privileged access, and just-in-time activation where appropriate. In the described Entra context, it also advises separating everyday and privileged accounts and using MFA for privileged access. The secure-deployment guidance identifies approval for Global Administrator activation as a best practice. These are vendor security recommendations to assess against your architecture and policy, not universal legal requirements.

Decide who may activate an elevated role, what approval or authentication gates apply, how long activation lasts, and how often the assignment and activation process is reviewed. The goal is to avoid unnecessary standing privilege while ensuring authorized work can still proceed through a defined path.

Build a control loop, not a calendar reminder

A practical operating cycle links least-privilege assignments to an accountable owner, a scheduled or event-triggered review, a recorded reviewer decision, and a change to access when the decision calls for it. Exceptions and missed decisions need an owner and a route to resolution. This is an implementation synthesis of Microsoft’s guidance, not a mandated formula or a measured guarantee of effectiveness.

Access packages can help bundle resources and apply controls such as approvals, assignment durations, or recurring reviews. They do not remove the need to decide who owns the package, who reviews its assignments, and how exceptions are handled.

Evaluate the workflow against your environment

If you are choosing or improving an identity-governance process or product, assess the operating burden as well as the feature list. Microsoft’s documentation provides examples of capabilities and planning decisions, but it is not an independent comparison of vendors.

  • Does coverage include the applications and resources that matter, across cloud and on-premises environments where needed?
  • Can joiner, mover, and leaver changes trigger appropriate provisioning and deprovisioning?
  • Can reviews reach knowledgeable owners, support delegation and fallback, and manage response windows and nonresponse?
  • Can you set expiration and approval controls, and check separation-of-duties requirements where your policy calls for them?
  • How are privileged access and just-in-time activation handled?
  • Can the organization retain decisions, resolve exceptions, and identify incomplete follow-through?
  • Who will own the recurring work, and what integrations and coordination will it require?

Confirm current product capabilities and licensing requirements with the vendor before relying on them; those details can change. The governance principles here are grounded primarily in Microsoft documentation and should be adapted rather than assumed to fit every organization unchanged.
