Instead of editing a static document after every outage, the prototype retains confirmed post-mortems and consults them during later triage. Its examples and results are the author’s walkthrough, not an independent evaluation.
What the Hindsight runbook does
In his September 29, 2026 article, “How I built a runbook that learns using Hindsight,” Yelubandi Hari Durga Ramakrishna presents a prototype for production engineering teams. Given an alert, the agent searches prior incident memories and drafts a response aimed at three practical questions: “What is most likely wrong, and how sure are we?” “What should I do, in order, with proven fixes first?” and “What should I not bother trying, because it already failed here before?”
The key difference from a fixed runbook is the feedback loop. An engineer reviews and confirms a structured incident record after an incident; that record can inform the response to a later alert. The same model is used over time, while the memory available to it grows. The author’s interface shows responses with and without Hindsight memory side by side.
The author describes four parts: a Streamlit console, agent orchestration, a memory wrapper, and an OpenAI-compatible language-model client, with Gemini and Groq as configurable examples. In this design, memory.py is the only module that directly knows about Hindsight and exposes retain, recall, and reflect operations. These are the author’s described implementation choices, not a verified deployment recipe.

How the incident-memory loop works
1. Store diagnosis and resolution separately
The author found that one prose post-mortem was not retrieved reliably enough for the wording of a later alert. His implementation therefore creates two records per incident: a diagnosis record and a resolution record. The records can include the incident ID, date, severity, service, title, symptoms, root cause, tags, successful steps, failed attempts, and lesson.
Records are shaped for future matching. The author emphasizes retaining literal error text, configuration keys, and service names rather than paraphrasing away useful identifiers. Incident IDs also provide a way to trace recommendations back to their source.
2. Retrieve memories using the incoming alert
At triage, the raw alert becomes the recall query. The author says Hindsight can return an extracted fact rather than the original stored text, so the application formats returned memories with incident IDs from metadata. The response prompt asks the model to ground recommended steps in those incident IDs.

The Hindsight API quickstart describes three operations: retain stores information, recall retrieves memories, and reflect generates a response using memories. Its documentation says retain extracts facts, entities, and relationships; recall combines semantic, keyword, graph, and temporal search; and reflect uses retrieved memories to produce a disposition-aware response. These documented capabilities explain the intended memory layer, but do not guarantee accurate retrieval or responses in this particular application. Hindsight API Quickstart

3. Make failed actions traceable, too
The runbook is intended to retain not only what resolved an incident but also what did not. The author imposes a hard prompt rule: “do not try” suggestions must be failures literally present in retrieved memories, with the incident ID attached. That constraint makes a negative recommendation inspectable instead of letting the model invent a supposed prior failure.
4. Review before retaining a post-mortem
After an incident, an engineer provides feedback on which suggested steps worked or failed and adds a short post-mortem. The system structures that input and displays it for confirmation; only then does it retain the record. The author summarizes the rule as: “Nothing enters memory unreviewed”.

The review gate addresses a concrete failure described in the article: saving an empty form led the model to invent an incident, which was then retained and influenced later retrieval and answers. The author’s safeguards were human confirmation and instructions to leave missing facts empty or mark the cause unknown instead of guessing. This example illustrates why the quality of retained memories matters as much as the quality of the response prompt.
What the author’s examples show
The demonstration bank is reported by the author as containing 22 incidents across nine services. It includes a recurring payment-service connection-pool problem and repeated auth-service Redis incidents. These are figures for the author’s described demo, not an independently audited incident dataset.

For a payment-service alert, the author contrasts a generic no-memory response—which proposed actions such as rollback and terminating idle connections—with a Hindsight-grounded response. The latter recalled three named incidents, suggested a pool-size or configuration fix and a rollout restart, and noted a prior pod restart that had not addressed the pool-size issue.

The author says the matched incidents were resolved in about fifteen minutes on average, while explicitly describing the comparison baseline as assumed. That figure is an author-reported example, not a measured industry benchmark or evidence of a general reduction in resolution time.
Similar alert wording, different cause
In another walkthrough, an alert containing an expired-certificate error retrieved the certificate incident rather than a superficially similar database issue. The agent recommended renewing the certificate and restarting the gateway. This is a reported demo scenario, not an independently reproduced operational result.

- Did recall surface prior incidents that actually match the alert?
- Can each recommendation be traced to an incident ID?
- Are previously failed actions stated accurately and tied to the memory that records them?
- Does the agent distinguish look-alike alerts with different root causes?
- Do newly reviewed post-mortems affect subsequent triage?
- Can an engineer inspect the memory trace behind a response?
The article demonstrates these as comparison dimensions; it does not report a controlled benchmark across them. Its examples do not establish production reliability, general incident-resolution gains, or that memory will improve every alert response.
Implementation note: asynchronous clients in Streamlit
The author reports an issue with an asynchronous HTTP client and event loop when Streamlit reran across threads. In this project, using a dedicated worker thread with its own event loop and client resolved the problem. Treat this as a project-specific implementation note, not a universal requirement for Streamlit or Hindsight applications.
What this design is—and is not
The design turns incident history into a potential input to future triage, with explicit review and traceability as controls against polluted or ungrounded memories. Its practical value depends on whether stored incidents are accurate, whether recall returns the right ones, and whether recommendations remain tied to them. Ramakrishna’s article is the source for the architecture, examples, incident-bank size, and timing; the Hindsight documentation describes API behavior, not the accuracy of this project or its reported outcomes.
