In Neo4j, relationships are stored with a direction, but you usually do not need to store a second relationship just to query the connection from either endpoint. For a genuinely symmetric connection, store one relationship and use an undirected pattern in Cypher. Store reciprocal relationships when they represent two distinct directed facts, not merely to make traversal possible.
How Neo4j relationship direction works
A Neo4j relationship connects a start node to an end node, has exactly one type, and can carry properties. Its direction is part of how the relationship is stored; an undirected query does not change that stored direction. See Neo4j’s graph database overview and graph database concepts.
Neo4j’s documentation puts the modeling choice plainly: “Relationships always have a direction. However, the direction can be disregarded where it is not useful.” It also says there is no need to add a duplicate relationship in the opposite direction unless the data model requires it. When one relationship is enough
Symmetric connections
If the domain fact is simply that A is connected to B, and the connection means the same thing whichever endpoint you start from, one stored relationship is generally sufficient. Choose a consistent orientation when writing it, then match either stored orientation with an undirected query pattern:

The missing arrow in this pattern means “match either direction.” It does not create an undirected relationship or make the stored connection directionless. Neo4j explains this behavior in its Cypher introduction.

Reading from either endpoint
Do not add a reverse relationship solely so a query starting at the other node can find the same symmetric connection. Use a pattern without an arrow where either orientation is acceptable. This keeps the stored graph from representing one connection twice when the domain only contains one fact.
When direction—and possibly a second relationship—matters
Asymmetric facts
Keep direction when reversing the endpoints changes the meaning. For example, if A follows B, that does not imply B follows A. Express the intended orientation in both the stored relationship and the query:

Creating a second relationship from B to A is appropriate only if B independently follows A. Neo4j’s relationship-modeling guidance recommends choosing relationships that express the domain meaning; its Modeling Relationships lesson provides further context.

Two independently asserted facts
A connection may look symmetric but still represent two separate acts—for example, two independently asserted or separately timed associations. In that case, decide what each fact means and model it accordingly. A pair of reciprocal edges is not automatically correct merely because both endpoints participate; one edge is not automatically correct if the two assertions have distinct meaning.

The second option should reflect the domain, not serve as a workaround for query syntax. When choosing relationship types, keep them meaningful without making them so specific that general queries require an unwieldy list of alternatives. Check undirected query results for duplicates
Undirected patterns can be traversed in both directions, so a query may return the same pattern more than once. Neo4j notes that this can also affect performance. Check the result cardinality of the actual query rather than assuming an undirected match produces one row per conceptual connection. If your result should contain unique pairs, shape or deduplicate the results accordingly; the right choice depends on what the query returns and whether endpoint order matters. When to use an intermediate node
A relationship is a good place for properties that describe a connection between two nodes. If the association needs richer structure, or needs to relate more than two entities, an intermediate node may represent it more clearly. Neo4j’s modeling designs guidance describes this approach. Choose it when the association itself needs to participate in the graph as an entity, rather than adding reciprocal edges to compensate for a modeling mismatch.
