Use the scanner report to identify the affected asset, confirm whether the finding is real, fix it or submit a documented dispute, then rescan and retain the evidence. If the scan is an external scan required by PCI DSS Requirement 11.3.2, it must be performed by a PCI SSC Approved Scanning Vendor (ASV). A passing ASV scan addresses scan results; it does not establish that your organization meets every PCI DSS requirement.
First identify the scan, scope, and requirement
Start with the report rather than applying a generic patch. Determine whether it is an internal vulnerability scan or an external scan intended to satisfy PCI DSS Requirement 11.3.2. That requirement covers applicable external scans performed by an ASV. PCI SSC’s PCI DSS v4.0 SAQ C specifies scans at least once every three months, resolution of vulnerabilities, and rescans as needed to meet the ASV Program Guide’s passing-scan requirements. Applicability depends on the merchant’s circumstances and validation path; check the current applicable questionnaire and standard with the entity responsible for your assessment.
PCI SSC’s 2024 ASV resource guide also describes an SAQ A case involving specified merchant e-commerce systems: pages that redirect payment transactions to a compliant third-party provider or embed that provider’s payment form. Outsourcing payment processing alone does not remove the scanning responsibility described for that context. Confirm whether your actual architecture and current questionnaire put your systems in scope.
An ASV is an organization whose scanning services and tools have been tested and approved by PCI SSC. If an external scan is required, confirm that the vendor is on PCI SSC’s current approved list. Do not silently remove a host from the scan just because it appears out of scope; resolve scope or inventory discrepancies through the ASV’s process.

Work each finding through triage, validation, and resolution
1. Capture what the scanner actually reported
For each finding, record the report date and version, scan type, affected hostname or IP address, finding identifier, affected service or software version, severity, evidence, and any remediation instructions from the ASV. Verify that the target is the intended system and belongs in the approved scan scope. If the target or inventory is wrong, coordinate a correction with the ASV and preserve the explanation.
2. Check whether the finding is accurate
Compare the scanner’s evidence with the host’s actual software version and configuration. Reproduce or independently verify it in a controlled way that will not disrupt production. If the report does not make the affected component or evidence clear, ask the ASV for clarification.

If the evidence indicates a false positive, an incorrect severity, or a potentially acceptable compensating control, use the ASV’s formal dispute procedure. Provide relevant evidence, such as version or configuration details and the rationale for the dispute. The ASV Program Guide recognizes disputes of these kinds and requires them to be handled and reflected through documented procedures. Do not simply dismiss the finding, relabel it, or assume a compensating control automatically makes the scan pass.
3. Remediate a confirmed vulnerability
The right fix depends on the specific finding and asset. Identify a vendor-supported patch or upgrade, remove a vulnerable service or exposure when appropriate, or correct the configuration that triggered the finding. Check dependencies and business impact before making the change, then apply it through your organization’s change-control process.

Keep the change record and validation evidence, such as the approved change, the version or configuration after the change, and relevant test results. PCI SSC describes vulnerability management as a cycle of scanning, patching, and rescanning; it does not prescribe one patch for every finding. Coordinate technical work with the system owner and use the actual report to determine what needs to change.
4. Rescan and preserve the closure evidence
For an applicable external Requirement 11.3.2 scan, request the ASV rescan needed to confirm remediation and meet the ASV Program Guide’s passing-scan requirements. If the rescan still reports a failure, return to the evidence, correct the remaining issue, and rescan again as needed.

Retain the original scan report, any approved dispute or exception documentation, the change record, and the final rescan report. This lets an assessor follow the finding from identification to closure. An internal scan may have a different process and evidence requirement, so follow the applicable assessment instructions rather than treating every scanner report as an ASV scan.

PCI SSC says periodic controls cannot be performed retroactively or backdated. A successful scan now does not create a report for a missed interval. Complete corrective actions, resume the required cadence, and discuss the gap and supporting evidence with your assessor and the entity that accepts your compliance reporting, such as your acquirer or payment brand. Do not describe a later scan as proof that the missed period was covered.

Know what a passing ASV scan does—and does not—show
An ASV report is evidence about scan results, not an overall PCI DSS compliance determination. PCI SSC’s June 2025 FAQ 1234 states: “The scan report is not an indication that any other PCI DSS requirements have been reviewed or are in place.” Ask the organization that manages compliance acceptance which additional evidence it requires; broader assessment questions may also need to be addressed with a qualified assessor.
