Document AI-generated code like any other engineering change, but make its AI assistance, human ownership, review, and actual validation easy to trace. A label alone does not make code maintainable: the record must preserve why the change exists, how it fits the project, and what future maintainers need to know.
What should a team document?
Use the pull request or equivalent change record as the central place to capture change-level context. Include enough detail for a teammate to understand and verify the work without claiming that checks ran when they did not.
- Intent: State the requirement or problem the change addresses.
- AI assistance: Identify which parts were materially generated or modified with AI, using the team’s agreed convention. The U.K. Home Office gives
[AI-assisted]in a commit message as one example; it is not a universal requirement. - Human ownership: Record who is accountable for understanding the change and who reviewed and approved it.
- Validation: List the tests, builds, static analysis, security scans, and dependency checks actually performed, with outcomes and any relevant failures.
- Maintenance context: Explain non-obvious assumptions, constraints, design decisions, edge cases, and known limitations.
- Dependencies and provenance: Identify new or changed packages and note the required security, maintenance, and license review.
This reflects a core accountability principle: the U.K. Home Office’s engineering standard, SEGAS-00020, last updated 20 March 2026, says teams retain full accountability for AI-assisted code and must understand what they run well enough to assert its security and maintainability. Where should the information live?
Put each kind of explanation where the team will use it. A pull request carries the context and evidence for one change; a durable design decision belongs in project documentation or an architecture decision record; a code comment should explain a non-obvious implementation detail, not repeat the PR description. This is a practical workflow, not a prescribed universal format: the cited guidance supports traceability and clear documentation but does not require a label on every generated line.

Choose a lightweight traceability method
A commit marker is easy to apply and search. A PR template can prompt authors to provide intent, ownership, checks, and limitations in one inspectable record. A broader AI-use register may suit organizations that need centralized oversight, but it can create extra recordkeeping. Choose the least burdensome method that preserves useful context and review evidence for the change’s risk.
How should reviewers verify AI-assisted changes?
Review the code as a proposed engineering change, not as trusted output. GitHub’s guidance says, “Always run automated tests and static analysis tools first,” and recommends compiling, running tests, checking warnings, and assessing whether the code fits the intended behavior and project conventions. Microsoft Learn likewise advises: “Read and understand every change before accepting it” and “Test AI-generated code at least as thoroughly as hand-written code.”

- Read every material change. Confirm a human reviewer can explain what it does and why it belongs in the change.
- Check intent and fit. Compare behavior against the requirement, architecture, naming, readability, and established conventions. Look for ignored constraints, hallucinated APIs, and code that is harder to follow or refactor than a simpler alternative.
- Run the normal validation. Compile or build the project, run relevant automated tests, inspect warnings, and use the team’s applicable static-analysis and integration checks.
- Inspect security and dependencies. Run relevant security and dependency checks. Confirm suggested packages exist, are maintained and appropriate, and have licenses compatible with the project. Apply the usual license-compliance process to generated code.
- Record the evidence. Capture which checks ran, their outcomes, who reviewed and approved the work, and any unresolved limitations in the change record.
AI assistance does not waive ordinary review or testing. Home Office guidance calls for review and approval before production and testing under existing engineering standards. Its standard is specific to the U.K. department; other teams should adapt its example to their own policies.
When should documentation and review be more extensive?
Scale the evidence to the consequences of failure. For a small, low-risk change, a concise PR record and the team’s ordinary checks may be sufficient. Security-sensitive or high-impact work calls for more inspectable evidence, such as detailed review and test acceptance, scan results, dependency review, and provenance review. The U.S. Department of Defense AI4SDLC rulebook describes these kinds of records in a defense acquisition and governance context; it is a useful high-assurance model, not a universal legal requirement.

OWASP’s Secure Coding with AI guidance states, “AI-generated code must have a human owner.” A team should not merge work when no accountable person understands it well enough to maintain it. GitHub also cautions reviewers against accepting code that is hard to follow or would take longer to refactor than to rewrite.

Use only the fields that fit the repository and change. A template improves consistency only if authors fill it with accurate information and reviewers inspect it; it cannot replace understanding, testing, or project-specific policy.
