Review Claude Code’s proposed changes and commands before you approve them, then apply work in small increments and verify the result. The exact prompts you see depend on the active permission mode and settings; no mode removes your responsibility to check what the tool will do.

Start by checking the active permission mode

Claude Code does not use one universal approval behavior. Anthropic’s security documentation describes Manual mode as read-only by default, prompting before edits, tests, and commands. Auto mode uses a separate classifier to review actions and block ones it judges unsafe. The CLI reference also documents a flag that skips permission prompts.

Before accepting a proposal, check the mode and project settings for your current session. Documentation and behavior can vary by version and surface, so confirm the setting you are actually using rather than assuming a prompt—or its absence—means the same thing everywhere.

What the modes change

Anthropic’s security documentation states: “You’re responsible for reviewing proposed code and commands for safety before approval.” Treat that as true even when a mode automatically reviews or accepts some actions.

Inspect the proposed changes before applying them

Read the actual diff, not just Claude Code’s summary. Check which files changed and whether the modifications match the task. Pay particular attention to security-sensitive material such as credentials, authentication logic, deployment configuration, permissions, and tests. These are practical examples of files worth checking; Anthropic’s guidance specifically calls for verifying changes to critical files.

  • Look for unexpected files, broad rewrites, or edits outside the intended scope.
  • Check that tests were added or updated where behavior changed.
  • Examine configuration changes for altered access, network, or deployment behavior.
  • Do not approve a change you cannot explain well enough to assess its effect.

Anthropic recommends checking changes to critical files and reviewing proposed code before approval in its security guidance.

Read every suggested command before approving it

A command can have effects beyond the code diff: it may execute a script, access the network, overwrite or delete files, or operate from a directory you did not expect. Read the full command and establish its working directory, inputs, and effects before running it. If any part is unclear, ask Claude Code to explain the command or propose a safer, narrower alternative.

  • Check whether it reads, writes, moves, or deletes files, and whether the paths are expected.
  • Look for network access, downloaded scripts, credential use, or code execution.
  • Be cautious with commands that pipe fetched or otherwise untrusted content directly into an interpreter or another command.
  • In Manual mode, web-fetching shell commands such as curl and wget are not auto-approved by default, according to Anthropic’s security documentation. A prompt is a point to inspect the command, not a reason to accept it automatically.

Understand the limits of permissions and sandboxing

Permission prompts, working-directory limits, and OS-level sandboxing are different protections. Anthropic describes a working-directory boundary for Claude’s file tools in Manual mode, but a Bash command you approve can still write anywhere your user account can. Sandboxing can add filesystem and network isolation for Bash commands; it is not the same as a permission prompt.

For scripts or tool calls involving untrusted content—especially when external web services are involved—Anthropic recommends considering an isolated virtual machine. Avoid piping untrusted content directly to Claude, review suggested commands, and verify changes to critical files. These precautions reduce exposure; they do not make an unsafe action safe by themselves.

Apply changes in small, testable increments

Ask Claude Code to make one coherent change at a time rather than combining unrelated edits. Anthropic’s common workflows separate recommendations, implementation, and test verification, and advise using small increments. Where compatibility matters, ask it to preserve backward compatibility.

  1. Request a focused proposal. State the intended behavior and relevant constraints; ask for a recommendation before implementation if the design is uncertain.
  2. Review and apply one change. Inspect the diff and commands, then approve only the work that fits the request.
  3. Test that increment. Run relevant tests and check their output before asking for the next change.
  4. Review the cumulative diff. Confirm the combined result still does only what you intended.

Verify the result before relying on it or submitting it

Run the relevant tests and inspect both the test output and final diff. A successful command does not prove the change is correct, and a generated implementation is not verified merely because Claude Code says it is complete. Investigate failures instead of assuming they are unrelated.

If Claude Code creates a pull request, inspect the PR yourself before submission or merge. Anthropic’s workflow guidance also recommends asking Claude to call out possible risks or considerations; use that as an additional review prompt, not as a substitute for examining the changes.

A practical approval checklist

  • Have I confirmed the active mode and relevant project settings?
  • Do the files and changes match the task, including any critical files?
  • Have I read each command and understood its directory, file, and network effects?
  • Is the work limited to a small, testable increment?
  • Have I run the relevant tests and reviewed the final diff?
  • If this is a pull request, have I inspected the PR and considered its risks?
