If your hair is thinning and your scalp is itchy or flaky, choose shampoo for the scalp symptom—not for a promise of regrowth. Use a gentle shampoo and conditioner to reduce damage to fragile strands; use a medicated dandruff shampoo only when flaking or itch calls for it, following that product’s label. Shampoo can help manage scalp symptoms and limit breakage, but it does not identify or treat every cause of hair loss.
Which shampoo should I use if my hair is thinning and my scalp is itchy?
First separate the problem you want to address. For routine cleansing, choose a gentle shampoo and handle the hair carefully. If you have dandruff symptoms such as flaking, scaling, or itch, select a dandruff shampoo by its active ingredient and labeled purpose. If thinning is persistent or unexplained, arrange an evaluation with a board-certified dermatologist rather than relying on shampoo to treat the cause.
For fragile hair without scalp symptoms
The American Academy of Dermatology Association (AAD) describes thinning or falling hair as fragile and easily damaged. Use a gentle shampoo, then apply a moisturizing conditioner after every wash. A leave-in conditioner or detangler can also help; conditioner coats strands and reduces breakage and split ends. The AAD lists zinc pyrithione, salicylic acid, sulfur, selenium sulfide, ketoconazole, and coal tar among ingredients used in dandruff shampoos. Choose according to the product label and your scalp needs. If one shampoo does not help, alternating products with different active ingredients may help, but persistent or severe symptoms call for medical advice rather than simply adding more products. Apply dandruff shampoo to the scalp and follow its own directions for frequency and contact time. Some products need to remain on the scalp for 5–10 minutes; do not assume that applies to every formula. Medicated ingredients can dry hair, so focus application on the scalp, especially if your hair is curly or coily. Use your usual shampoo and conditioner afterward if needed for hair care. How often should you wash, and does hair texture matter?
There is no single frequency for every scalp and hair type. Oiliness, texture, the shampoo’s directions, and any clinician’s treatment plan all matter.

Can dandruff shampoo make hair loss worse?
Dandruff shampoo is intended to manage scalp symptoms, not to treat the causes of thinning. Its active ingredients can dry hair, which may make fragile strands more vulnerable to damage. Keep medicated shampoo on the scalp as directed, condition the lengths after washing, and stop using a product if a rash appears or irritation worsens; follow the label’s advice about seeking medical help.

One U.S. label example: ketoconazole 1%
The U.S. National Library of Medicine’s DailyMed label for Nizoral Anti-Dandruff Shampoo lists ketoconazole 1% and says the product controls flaking, scaling, and itching associated with dandruff. For adults and children 12 years and older, that label directs use every 3–4 days for up to 8 weeks, then as needed. Children under 12 should ask a doctor. The label says to stop use and ask a doctor if a rash appears, the condition worsens, or it does not improve in 2–4 weeks. Check the current package because labeling and availability can change; do not apply this schedule to other ketoconazole shampoos or dandruff products. DailyMed Nizoral label
When should you see a dermatologist?
Flaking and itch can have several causes, including seborrheic dermatitis, psoriasis, fungal infection, or eczema. Consult a board-certified dermatologist if dandruff is severe or does not improve with home care, or if symptoms are persistent, painful, or inflamed. What shampoo can—and cannot—do for thinning
Shampoo selection can make cleansing gentler, reduce irritation from a mismatched product, and help manage dandruff symptoms. Conditioner and careful handling can reduce breakage. Neither a gentle nor a medicated shampoo should be treated as a general hair-regrowth treatment.
AAD describes minoxidil as a treatment applied to the scalp that may help some people with early hair loss; results often take 6–12 months.
