To find why a state machine entered the wrong state, reproduce the failure from the same starting state and event sequence, then compare the expected and actual execution one event at a time. The first point where they diverge is usually more useful than the final state: inspect which state was active, whether the event was handled, how each guard evaluated, which transition ran, and what actions changed along the way. Turn that reproduction into a regression test.
1. Reproduce the failure before changing the machine
Keep the conditions that determine the path through the machine: its initial state or state configuration, event order, input values, timing, and any queued or deferred events. A failure that depends on timing or event order may disappear if you simplify too aggressively. Reduce the case only after confirming that the smaller reproduction still reaches the same incorrect state.
For a hierarchical or concurrent statechart, record the complete active configuration, not just one leaf state. The machine may have more than one active state, and a transition can depend on which parent and child states are active.
2. Write down the expected trace
Before stepping through the implementation, describe what should happen after each event. Include the active state, the transition expected to be selected, the expected guard result, and the required actions. That gives you a concrete trace to compare against execution rather than a vague expectation about the final destination.

Then capture the actual execution using the same checkpoints. The aim is to find the earliest mismatch, not merely to confirm that the final state is wrong.
3. Inspect the first point where actual execution diverges
Compare the expected and actual trace in order. Check whether the event arrived, which state was active when it arrived, which transitions were candidates, what their guards read at evaluation time, which path was chosen, and which actions ran. A later wrong state can be the consequence of an earlier missed event, unexpected guard value, or side effect.

In MathWorks Stateflow, breakpoints and watched data can help expose execution decisions. Use breakpoints around the transition checks and execution points available in your model, then inspect relevant data as the machine runs. See the Stateflow debugging documentation and the standalone chart debugging guide. Other frameworks expose different debugging controls; choose the equivalent points in the tool and runtime you actually use.
4. Check event delivery, guards, and transition semantics
Confirm that the event can reach the transition
Verify that the expected state was active, that the transition trigger matches the event, and that the event was delivered rather than queued, deferred, or handled elsewhere. If a guard is false, the route may be disabled. What happens next is framework-specific: in QP/C’s documented semantics, a disabled event can propagate to a higher-level state. Do not assume that behavior applies to other state machine libraries. Consult the documentation for the framework and version in use; the QP/C hierarchical state machine reference explains its semantics.

Check guard inputs at the moment of evaluation
Inspect the actual values the guard reads when the transition is considered, not just the values you expected earlier in the event cycle. An action or state update may have changed them. Trace mutations between event receipt and guard evaluation, including actions on transitions and state entry or exit.
Distinguish internal from external or self transitions
Action logs can be misleading if you assume every transition exits and re-enters a state. In QP/C, an internal transition runs its associated actions without executing exit or entry actions. Other frameworks may define internal, external, and self transitions differently. Check the relevant framework documentation before interpreting an absent or present entry/exit action as evidence of a fault.

5. Compare the model with the implementation
Once the divergence is located, check whether the model and implementation agree about the path. Look for a missing transition, incorrect destination, wrong or omitted event or action, extra or missing state, or an event accepted along an unintended route. Trace the fault from the first mismatch rather than editing whichever later state appears most visibly wrong.

Replay the reproduced event sequence in a test. Assert the state at useful checkpoints, and check relevant action or entry/exit effects as well as the final outcome. Cover guard outcomes that could direct the same event along different paths. A test that checks only one output may pass even if the machine is in the wrong state.

If the state is private and cannot be inspected through a test seam, use a follow-on event whose observable behavior differs between the intended state and plausible incorrect states. This tests the machine’s behavior without requiring its internal state to be exposed. Keep the event trace readable so a failure reports the first checkpoint that no longer matches. The QP/C statechart testing guidance discusses testing state machines and observable behavior.
Choosing a debugging view for your framework
There is no basis here for a neutral, current feature-by-feature ranking of state machine tools. When selecting a debugging view, check whether it fits your framework, runtime, and deployment environment, and whether it can show the information needed to reproduce your fault:
- Active state or hierarchical configuration and transition history
- A way to pause around guard evaluation and transition execution
- Inspection of guard inputs and machine data
- Event replay or scripting for the failing sequence
- Visibility into transition, entry, and exit actions
For a team already using Stately, its agent debugging documentation describes visual inspection, trace-based debugging, and scripted reproduction; the page identifies the referenced agent package as alpha. MathWorks documents Stateflow debugging for its own charts, while QP/C’s reference is useful for understanding QP/C transition semantics. These are framework-specific capabilities, not interchangeable guarantees.
